Just a short addendum to the rather windy paragraph I wrote above: If your state collects state income tax (and most of them do), also go to the State Tax Commission with the same information. Many times the IRS and state tax offices will share information with each other, i.e., if someone cheats on taxes with the state, the state will inform the IRS or vice versa. But if you pay state income tax in your state, they also need to be brought into the loop. It may be that along the way there will be other state agencies that you will be referred to. I say--the more the merrier! :)
