FIESA "MAJOR" error - sickened

Patient's age was off by one year, not sure if that is what he dictated or I didn't catch his error or if what VR put and I didn't catch it.  In any event, the fact that the patient's age was incorrect by one year is a MAJOR error. 

It has always been a major error. If off my more - than 1 yr it is critical. Does not matter

The point I was trying to make is I do not believe it should be counted as major. This in no way affects patient care. I do, however, appreciate your sanctimonious reply. Perfection sometimes escapes me, especially for what I am paid.

When I started 20+ years ago, the error was on the dictator, who signed off on the document (though it was appreciated if we caught it). These offshore MTSOs are the ones who put all the responsibility on the MT so they can use it to penalize us.

But the MT. Not doctors, not nurses, not TSMs, not QAs.

Nobody gets their "bonuses" (previously part of our pay) snatched away at the last minute because the idiotic ISR put 67-year-old and you didn't catch that it should be 66-year-old so you lose that $50-100 a week that used to be part of your salary.

Absolutely NO ONE gets sent to the cleaners for minor errors, or any errors at all, but MTs.

A doctor or nurse could pretty much accidentally kill someone and they would get a slap on the wrist, but no pay cut.

Our beloved TSMs can forget about TAT because they are on vacation and no penalty there.

Tech people get their high hourly salaries and can take up all the time in the world and make all the errors in the world and they still get their pay, no penalties, no deductions.

Only the poor, lowly MT, who pays all the MTSO salaries by the work they charge out, gets our pay docked so that many of us are at or near minimum wage. What a racket.

My TSM makes the biggest blunders of blunders and gets away scot-free. If employee information were treated the same as HIPAA information, she'd probably be in the slammer.

Looking at some threads further down the page, specifically one asking about whether Nuance still has TL's, I see that a TSM answered them. First of all, I see at the top of this page, "A community of US Medical TRANSCRIPTIONISTS." It clearly explains the condescending "always been a major error" with a typo and the other post asking why the complaint. Anybody who does this job would understand and have compassion.

Where is the TSM? I assure you I'm a TL and I DO transcribe sm - the TL who answered

I have learned my lesson the hard way - if the age is off by one year (usually because the patient had a birthday while in the hospital), I blank it - it doesn't matter that we know it is the correct patient - that the dictator thinks the patient is 28 and doesn't realize that today is the patient's birthday and they are now 29 - I blank it - and sit back and think fondly of the days when we were respected that we knew the business and weren't treated like idiots.....

The only thing that matters to them is more imaginative ways to cut our pay.

IMHO they should NOT be able to use FIESA as a pay cutting tool, which is the only reason they developed it. They were the first to pit QA against MT and they love to pretend it is all about quality when it is ONLY about ability to cut pay.

If they want, they can get very creative and cut everyone's pay for no reason, think about it. There are ways to mark off a multitude of "errors" so they can achieve their goal of no bonuses for anyone and we have no way of knowing how many ever get the bonus or if they are honest at all in rewarding bonuses.

The whole FIESA thing stinks to high heaven and is only a way for them to get even more pay off of us.

I think we also have to recognize they do not want QA using their brains either. If it is a number error, then it is major/critical/whatever where the QA/QC should be able to determine whether or not something truly compromises patient care, i.e., vitals being off just a little or the age off by a year, etc. I no longer spend a lot of time trying to figure things out. If the doctor is stumbling over labs and it is a bit inconsistent, I blank it, just not worth the extra trouble to try and get right. This was harder when I was on a no-blank account but it is worth it to me now that I have a 3-blank account.

If the patient's age or gender is incorrect in the body of the report, yes, you should be marked off for it.

It's a major error. At some companies it would be a critical error.

The complaint is because the patient's age is wrong by a year, it does not put their life in danger nor affect the care they receive because of my error; thus, I do not consider this a major error. As somebody else stated, I also do not believe I should be held responsible for a mistake made by the physician/dictator. The only clear reason for this is to take even more money away from my paycheck which has been cut in half since Nuance acquired me.

If you hear 17 and the patient is 70, it's a critical error. If the dictator says 55 but the patient is 56 and you leave 55, it's a major error because you had the ability to double check and you didn't.

Stop justifying the fact that you have decided to be the devil's right hand man by QA'ing your fellow workers for anything and everything you can possibly find to take points most of which have no impact on patient quality to line YOUR pocket with more money with the more mistakes you find wrong. AND let's not act like you're perfect either--putting the reports back in under the wrong accounts when your done and the hospital getting upset at what YOU did, putting in words we left blank and nailing us when what you put in was incorrect and yes I could go on. You are not perfect either and it's VERY easy for anyone to nitpick and find mistakes all over a report--difference is your job depends on us failing and YOU get paid MORE to nail us to the wall and keep us from getting that bonus. Take your sanctimonious butt back to work and get off our board. The very fact that you are a QA person for this company shows your lack of integrity as a human being. These ladies are already feeling as low as they can get and they don't need you on this board making them feel even more like it's them that's the problem. It's you and the person paying you that extra bonus to find fault. You pay minimum wage for a job that requires specialized training and if they take the time to find out the answers on top of 30+ pages of account specifics and some of the worst dictators out there since you couldn't convert them to VR--you nail them with the not meeting the lph garbage so they're damned if they do and damned if they don't. You're no better than the person you are grading so get off your soap box. I bet I could pick apart your work and find errors too. Been there-done that.

My fellow MTs' keep your chins up and send out your applications. There are still MANY companies out there hiring and while the pay may be the same or even a smidge less--the headache of working for these people is not there nor do they lie with the "bonuses" for this and that which they ensure only a select few have chance to achieve. Don't feel like your stuck or no good, take the chance and take the test. You'll find your not as bad as Nuance is making you feel. I'm done with this company. I'm sticking with the company I've been with for 18 years and still have a 99% average because they grade on an average not based on one report sent in for an issue. They realize if the report had a blank--we struggled with it. Thanks to our wages falling so low because of this company buying everyone out and VR--I can't afford to be an MT fulltime anymore and went into fast food with a tasty $9000 a year raise. Reading your response to these ladies tells me I made the right decision.

Yes FIESA sucks, but we (QC) have a job to do, just like you. We're paid no more to find errors, and we don't have bonuses. My point was if you can verify information, do it, but I "assume" because that makes sense, my post was disliked...by someone who doesn't even work for Nuance.

It would be nice if there was a more tactful way to remind you. I can see if you were repeatedly not checking the patient's age. It sure would be nice if they could just give you a "heads up" kind of reminder at least once.

I don't want to lecture, but it is important to check the patient's age. Unfortunately, we have doctors, ARNPs, PAs, nurses, etc., who are stressed, dictating in areas with a lot of commotion (and I am not excusing their mistakes) who make mistakes. They put in the wrong patient. I see this happen weekly. You really have to check and double check to catch their mistakes. Once again, it is their mistake, but if you don't catch it... nobody will. Had one today that an MT let go through. I'm QA... Now this age was off by 6 years. But 1 year or 6 years, it could be the entirely wrong patient even if the birth month is off. It's just things we have to check and verify. Unfortunately, it's part of your job and we don't get paid for demographics anymore. Sad.

The age is not the important thing. The important thing is that you verify that it is the correct patient. If you have not verified the age, it can be inferred that you have not verified any of the patient-specific information. I am trying to be nice. I hate Nuance as much as you do, but this is not just a Nuance thing. It is an MT thing. I really do hope you have a good day. I'm not trying to kick you when you're down. If you want, you can dislike this post too. I won't say anything else.

FIESA ERRORS - Definitely worth fighting

Has anybody every thought about retaining legal services for the neglible nature of FIESA. Who is say that they do not change things in reports, especially if they want to get rid of somebody. There is no way even for QC when audited to go see the original report once they get their hands on it. This leaves a whole lot of room for error. I know when I gave notice the errors starting coming in fast and furious on everything, words that I had never used in my life. So.............. whose to say if this thing is 100% fool proof??

You're mistaken. QC, QA etc can see the original report as well as the changes MLS have made. In emon you can see everybody who has touched the report and what they did to it.

Thought I was going to make it through today with no Major error but no such luck.  I typed "There are" and they heard "The"  Trust me either way the sentence says the same thing and in no way changes the meaning of the sentence so I get dinged a major error for this.  I really hope these quality people get good bonuses for these type things.  Just another major error to add to my very long list.  More in the last 2 weeks then in my entire career of being a transcription ...

.....for missing a pronoun at the begininng of a sentence... Symptoms started instead of His symptoms started....major error?  I think not.  Minus 1.0 whole point for me! There is no consistency in the grading of reports.  And again I say, this is a subjective error...someone heard it, I did not and it does not matter anyway.  Doctors do not dictate complete sentences all the time. This just tops of my bad day.  I get on to check previous reports from yesterday and see 1 correction on 1 report out of all of them.  QA filled in my blank, and the error category is "major" with subcategory "not applicable."  Platform is Cornerstone.  Since when is it a major error to leave a blank for something we aren't sure about?  After listening, again, I still can't tell if it is the word QA put in or the word I thought it "might" be; hence, the blank.& ...

Compare job numbers to those jobs that come back with major errors.  Want to bet that most of those jobs we have a hard time with are the very same jobs that we are getting marked down for?  Most likely the very blank or the very problem we had is what the major error is for. ...

This is a new one to me and laughable honestly. I thought we were to spell phoenetically a doctor's name in the text if we cannot find it or verify it.  So this is what I did as usual and got a MAJOR CRITICAL ERROR for putting the wrong doc name??? WTH. Seems like they are grasping at straws now. What's the deal? NEVER Get Error Notifications In Fiesa

I used to almost NEVER get error notifications in Fiesa.  Now that the new "compensation plan" has been put into place, I get a notice every other day that they found an error.  Due to my work ethic established over decades, I have not sped up my QAing of reports at all, accepting my fate that I will now be making third world wages until I find a new employer.  Supposedly post-audit has managed to find these errors in my work.  I don't buy it for a minute.  How much lower can Nuance go ...

After starting with Nuance, getting great QA reviews, great post audit reviews, and thinking I was doing well - I have gotten THREE major alert emails in 2 days.
