Keep track of every job you send to QA with blanks or other. Compare job numbers to those jobs that come back with major errors.  Want to bet that most of those jobs we have a hard time with are the very same jobs that we are getting marked down for?  Most likely the very blank or the very problem we had is what the major error is for.

What we hear in difficult dictations can be different from what QA hears. I've gotten reports back from QA where I left a blank and they have inserted something that simply was not decipherable, so they obviously guessed. Guessing is not okay.

Is the QA being done by Nuance, or is the QA being done by the client by an in-house MT?

Some clients still have in-house MTs and they outsource their overflow to MTSOs. Sometimes Nuance QA can't figure out the blank but the in-house MTs can because they know the dictators so well they can just about read their minds at this point. I've been on both sides of this fence, and when I know a dictator that well, it's not guessing.

I think we might be viewing this through 2 different filters. I am thinking of the many instances I have encountered where QA apparently heard something that was impossible to hear on my end as verified through re-listening to the archived recording. That was puzzling at the most in the environment where they were not deducting for blanks. But when they started deducting for blanks that they can hear for some reason or "know" what was said and I can't but "should be able to" somehow, and those deductions cause me to lose my bonus, then I'm going to call it a rigged game, a stacked deck, if this is going on the way you described. Now if it is done just because of incompetence on the part of the designers of this system of rewards and punishments, then it is just that--a result of bumbling incompetence. But if they purposefully have set things up that way to minimize the amount of bonus money they have to pay, then, yes, that would be a consipiracy to deprive me of my earned income under their own rules that they presented to me when I was hired. Only the suits know which is the case. Either way, I am unfairly knocked out of my bonus.

My post was regarding the accusation of "guessing" by the QAs/QCs. It had nothing to do with bonuses.

Thought I was going to make it through today with no Major error but no such luck.  I typed "There are" and they heard "The"  Trust me either way the sentence says the same thing and in no way changes the meaning of the sentence so I get dinged a major error for this.  I really hope these quality people get good bonuses for these type things.  Just another major error to add to my very long list.  More in the last 2 weeks then in my entire career of being a transcription ...

There is no consistency in the grading of reports.  And again I say, this is a subjective error...someone heard it, I did not and it does not matter anyway.  Doctors do not dictate complete sentences all the time. This just tops of my bad day.  I get on to check previous reports from yesterday and see 1 correction on 1 report out of all of them.  QA filled in my blank, and the error category is "major" with subcategory "not applicable."  Platform is Cornerstone.  Since when is it a major error to leave a blank for something we aren't sure about?  After listening, again, I still can't tell if it is the word QA put in or the word I thought it "might" be; hence, the blank.& ...

This is a new one to me and laughable honestly. I thought we were to spell phoenetically a doctor's name in the text if we cannot find it or verify it.  So this is what I did as usual and got a MAJOR CRITICAL ERROR for putting the wrong doc name??? How about this? For holiday coverage, why not schedule 4-hour shifts instead of 8-hour shifts with hardly anything to do? For example, why not have some MTs commit to the following shifts: 7am-11am 11am-3pm 3pm-7pm 7pm-11pm 11pm-3am 3am-4am I think the MTs would like that idea. They could spend time with family/friends on the holidays without being on call and having to check their queue every 15 minutes or so for some work. They commit and work for 4 hours and are done. Cheap, cheap, cheap. You should pay your peeps. You could tell the clients NTS cannot keep up with work because people are jumping ship like rats from a hot fire. Perhaps management (upper, way upper I mean) could explain instead of the TLs having to do it, that they needed so much money to take to the bank they had to cut the pay of all their employees, now they have green, not as speedy, uninitiated MTs doing the work. Everyone should make a habit of getting a letter of reference from CURRENT employers even before you are actually looking for another job.  All you have to say is that you are updating your resume and want to be sure you have current letters of reference attached as well.  It doesn't matter if you are working as an "employee", if they are happy with you and with your work, they should have no problem giving this to you.  If you are an "IC", it should be even easier since you ...

This is how we got paid at a major department store when I was a hairdresser: Guaranteed minimum wage for hours worked. If our production exceeded minimum wage we were paid a percentage of that production on top of the minimum wage, usually 40%. You know that's your aim anyway. That way you can save money on all the middlemen and enforcers, i.e. you can stop making good QA/QC people into inquisitors. If you want to offer a true bonus, just let everyone know they will make 7 c/l, THEN offer incentives, that way at least we know we can expect the base salary and the bonus will truly be a bonus, not a penalty. I had been using Shorthand for 8 years and then just got a new job where they won't allow us to use anything but Autocorrect.
