This just tops of my bad day.  I get on to check previous reports from yesterday and see 1 correction on 1 report out of all of them.  QA filled in my blank, and the error category is "major" with subcategory "not applicable."  Platform is Cornerstone.  Since when is it a major error to leave a blank for something we aren't sure about?  After listening, again, I still can't tell if it is the word QA put in or the word I thought it "might" be; hence, the blank.

I know where you're coming from. I just got put on a new acct that they are doing 100% QA on because apparently it just came back from India and the hospital wasn't happy with the quality. There is no consistecy in the feedback from QA - and I finally gave up caring because the things they are picking at are trivial. It makes for a long day when you know someone is reading every one of your jobs - so much of these little things just come down to personal style. In your case, there's no way you should get marked with a major error if you left the blank - obviously if you knew what they were saying..... you would have typed it.

than to guess what they're saying and guess wrong. I don't know what QA is thinking by dinging you for leaving a blank.

There are some QA/MMEs that really don't have a clue because either they are inexperienced at being an MT and by luck passed the test, or they have old ways and they haven't looked at the BOS2 or DQS standards. Personally, if I can't copy/paste the exact rule and where I found it into the comment section, I don't mess with it. If its an incorrect medical term I leave a definition. For incorrect abbreviations, in the comments I leave the expanded form of what I changed it to. Of course, there are some I don't leave a comment for, because its a typo (on/no) or it was an extra couple words from ASR that weren't taken out ("The patient the patient was"), etc.

My goal as an MME is to make it so that the MT/ME passes their audit when it comes around by helping them learn rules that the auditors go by that are new or are silly and easy to forget.

As an MME, we do 75% transcribing/ASR work and we are audited just the same. If a 100% QA job is audited or caught by the client, our head rolls with the MT's for not catching it.

When I was an MT/ME for 9 years, I was dinged inconsistently also and I would look up the rule in the BOS and let my CCM know I was correct. Whether anything was done about it, probably not, but if I knew I was correct, the QA correction didn't bother me because they can tell who does what.

When I became and MME I told my CCM to let me know of any complaints against me so that I can know if I was making a mistake.

Unfortunately, leaving comments costs me money and with it being so impersonal, they may come across as short or mean (they are certainly not), and there are some MTs that don't even check their feedback. However, it is a good way to cover my pattoot when my correction is questioned.

Seriously, I had an MME change Alzheimer dementia to Alzheimer's dementia - how long have we had this drilled in our heads to drop the 's. I had another change hemoglobin 10.0 to 10 and said not to use the trailing zero - that only applies to medications. I had one dictator who wanted his report set up a certain why and the MME changed it and I just feel like I don't know what they want anymore.

Everything you said you were doing correctly. I actually copy/paste the trailing zero rule quite frequently. Everything you need to know is in the BOS2 or DQS Standards. Unfortunately, some MMEs never look.

As long as you are sure you are correct, you have nothing to worry about. Let your CCM know about this MME and hopefully she will educate them on the proper standards.

They are all correct. Don't worry, try to blow it off, and/or just email your CCM. The MME pay was cut by 1/2 per paycheck at least with the Spheris change, then when they started "first time wrong" -- they get hardly any QA work now. A lot of these QA people are old time 20+ year people who can't transcribe anymore (hands/arms are shot) and they are just trying to hang on waiting for retirement to get the heck out of here. All the younger ones got the clue and left MQ a long time ago.

They probably accidentally chose the wrong - reason.

It is extremely easy to do if you aren't observant as it is just 1 key (m) and that key shares the alt key (alt+m) with leaving a comment or not. This is all the more evident from the not applicable subcategory. QA/MME need to go faster than they'd like to make minimum wage also. If no reason was given, it was just a finger slip. QA/MMEs aren't to be using anything but the general "QA Feedback" category.

I feel better about it now that you mentioned that. I've had that happen a couple other times where I was right to leave a blank and it was counted as a major error, yet the subcategory didn't correspond. I wasn't scored on the reports either, so it must have been an MME? Anyhow, I'm still ready to quit. I just feel like I'm hating being an MT. I used to LOVE my job. Now, its all I can do to stay awake while working. I have to get up every few minutes and walk around to energize myself. It's not healthy to be stuck at a computer desk for 8 hours a day (usually longer....), not mention everything has changed so much.

The report happened to be a 30 minute ESL report. I ended up with a major error because I left a blank. It is better to leave a blank than guess.

I had read in their regulations for blanks..etc... that if you leave a blank but QA can hear it and fills the blank it is a major error..to me that is just crap..I can have 4 other people in the OLC group listen and no one will get it but QA fills it (ya not sure if they are even right) and it is a major error..I did quit..not for this reason but it was one of the reasons

Thought I was going to make it through today with no Major error but no such luck.  I typed "There are" and they heard "The"  Trust me either way the sentence says the same thing and in no way changes the meaning of the sentence so I get dinged a major error for this.  I really hope these quality people get good bonuses for these type things.  Just another major error to add to my very long list.  More in the last 2 weeks then in my entire career of being a transcription ...

There is no consistency in the grading of reports.  And again I say, this is a subjective error...someone heard it, I did not and it does not matter anyway.  Doctors do not dictate complete sentences all the time. Not to mention the fact that this NP dictator is ALL over the place...go back here, change ...

Keep track of every job you send to QA with blanks or other. Compare job numbers to those jobs that come back with major errors.  Want to bet that most of those jobs we have a hard time with are the very same jobs that we are getting marked down for?  Most likely the very blank or the very problem we had is what the major error is for. ...

This is a new one to me and laughable honestly. I thought we were to spell phoenetically a doctor's name in the text if we cannot find it or verify it.  So this is what I did as usual and got a MAJOR CRITICAL ERROR for putting the wrong doc name??? WTH. Seems like they are grasping at straws now. What's the deal? We are to be mind readers now and get these names exact every time even when they don't spell, we can't find, etc. etc. Nuance just makes itself look ridiculous ...

HELP QA Pay By Blank???

Hi, I have been speaking with a company who will be hiring me for transcription.  They pay good rates and I asked them about the possiblity of job progression and eventually working as QA.  They said that QA pays by the blank... how is that even possible?  Ive seen being paid by the report, by the hour, or  by production (by the line)... but I have no idea how the QA staff make good money only getting paid for the blank.  any ideas?  Amy I missing something?  ...

I can pick out at least 3 in that list that I know for sure just guessed.  I've QA'd their work and it's horrible.  Way to promote accuracy MM!  You know what would get those blanks down?  Educating dicators, fixing sound quality, keeping MTs in their primaries.  Nevermind, that won't work; it's the MTs that need to improve, obviously.  MM is run by complete boneheads. ...

I was consistently going over the metrics for blanks being submitted to the client and decided to keep track of why I was sending those blanks.  It turns out that 77% of my blanks are due to either discrepancy or audio issues; blanks that I am positive nobody could fill in.  77%!  These metrics either need to be done away with or adjusted for doctors that don't take the time to not be idiots.  The most common discrepancy is incorrect age or something else that I can fill ...
