If the examination is falsified, the billing IS fraudulent. - I do know a bit about this.

...and, incidentally, when I was in the eldercare ombudsman program, among the things we were alert to was fraud, and the first thing we were instructed to look for was "the small things", so let you can go of that mantra.

But besides that, this isn't a "nothing" thing anyway. You obviously have no appreciation of the importance of the retinal examination in the assessment of neurologic disorders.

If this WERE "much ado about nothing", it wouldn't bother me and I wouldn't have mentioned it.
