In other words, these are alleged to be his specific findings on examination of the patient, and they are highly unlikely to be true (for each and every patient he sees).

It would be as fraudulent as dictating "HEART: RRR. No rub, gallop or murmur" when you hadn't lifted a stethoscope to the patient's chest.
