I filed with the state DOL online and learned from an investigator 6 weeks later that claims for earnings paid other than salary or hourly (by the line) should go to the US DOL. If PTO involved, that portion of the claim should be filed with the state DOL. That was done a couple of weeks ago, but who knows how long it will take. Keystrokes could just do the right thing and pay us what we have earned; why don't they???
