I can pick out at least 3 in that list that I know for sure just guessed.  I've QA'd their work and it's horrible.  Way to promote accuracy MM!  You know what would get those blanks down?  Educating dicators, fixing sound quality, keeping MTs in their primaries.  Nevermind, that won't work; it's the MTs that need to improve, obviously.  MM is run by complete boneheads.

Some of the winners were simply for less blanks. I don't know about the winners that won $100.00, but that was yet another list they have created.

I agree educating dictators on how to dictate would be a tremendous help. Good dictators don't have you leaving blanks!!!!

that means way more to me than cheesing a report through to try to win an idiotic blanks contest. Much of the time, blanks are out of our control -- either they say it audibly or they don't. Obviously whoever came up with the idea for this contest has no idea what our job entails.

zero blanks. The irony is that when you go too fast & make a typo or push through a guess instead of a blank, then you get severely penalized. You can't win! So I think I'll just keep focusing on quality instead of their games.

I agree with you 100%. There are some dictators, too many in fact, that do not dictate well, for whatever the reason. I have one that watches TV while he is dictating (you can hear CNN in the background), another falls asleep, and yet another simply does not talk loud enough to get a viable report. I really hope in the very near future ALL the dictators are trained to do good dictation. It would save a lot of time.

and won't take the time to make a clean report. They simply don't care. Their time is too valuable to waste on clerical duties - that's what we're for. The greedy hospital administrators dream of getting rid of us & making them do their own clerical work, but I don't think that's going to fly with the doctors...

That is the whole goal of front-end SR. With that, nothing leaves the physician himself, much less the facility. There are no MTs and no "editors."

When the computer the physician is using presents the text to him in real time, as he speaks ... he says "The patient is" and "The patient is" appears on the screen ... he starts speaking just fine.

They do not perceive this as "clerical work" any more than they perceive texting or emailing to be clerical work. To them, it is just technology like iPhones and iPads. They talk to their phones and cars, so what is the big deal?

They quickly recognize something else, too. The first time they try this, they say "Wow, that was instantaneous. I don't have to wait for transcription to do this. I never again have to redictate something they lost. I never again have to proofread their work. I never again have to fill in the blanks they leave. I never again have to write down what I dictated, check for the report, or go back into the record to sign it. I never again have to worry about being suspended because of them. I never again have to write a note summarizing what I dictated so that patient care does not suffer."

I can tell you this, too. All that perfection in capitalization, spelling, and punctuation MTs think is so critical? Nobody else cares. Nobody is the least bit concerned. If does not affect patient care.

I disagree that spelling, punctuation, and capitalization do not matter. I think they do matter.

Also disagree that doctors want to spend time dictating into EMR and editing this. Some will, but I would bet that many won't. Do they have a choice?

The fact that you think doctors even have to "edit" this tells me that you do not have any idea how this works. You are seeing the same SR system that you use.

Why don't you tell us how you think this works?

You know, some MTs here still think their work gets printed out to put in a paper chart or to be scanned into an EHR. Some still think doctors' daily notes and orders get written by hand on pieces of paper.

You seem to be stuck thinking that the doctor dictates by phone into a dictation system, which applies the SR to transcribe it, then sends the report to the doctor, who then has to fix all the blanks that you do.

You can fixate on how much spelling, punctuation, and capitalization matter, but I was not talking about you. I was talking about doctors and hospitals. They care about content. They can no longer afford to pay someone to pretty up reports.

No, I don't sell SR. My facility has it. The majority of our doctors, frankly, prefer to type it themselves or use our FESR than to use the transcription.

This is already here. Your job is VERY time-limited. You need to start transitioning out now. You seem to be in denial about this. Continuing to insist that editors will still be needed does harm to the people who read these boards. You are going to be out of a job and so will they because they listened to you.

Accusing someone of selling things that you don't like, or of being a suit if they say something you don't like is immature.

the time. It takes a lot of work to correct. No doctor is going to be impressed with having to waste their time trying to pain-stakingly correct VR garbage that the hospitals used to make sure was taken care of by professionals. You're either a suit or a salesman, but either way, you're wrong.

It also is not the SR that you see. This is individually trained to the doctor. Errors are much lower because they learn how to speak clearly.

We have it. Our doctors type their own notes, use their own custom templates, and use SR. The middle-aged and younger doctors think doing things that way is normal.

You need to consider something ... most physicians who are not ready to retire have considerable computer experience. They have more than you do. They interact with those computers via a keyboard and voice, and many have done this since childhood. They do not need someone to type a note any more than they need someone to type their emails.

A lot of you misunderstand the value of your work to your MTSO, too. Your value lies in the service you provide to train the SR systems. You are working yourselves right out of a job while drinking the Koolaid that editing will be necessary forever.

You have your eyes closed and your fingers stuck in your ears singing "it can't happen" while it IS happening right in front of you. You imagine it is going overseas. No, it is going away entirely.

Open your eyes and look outside your home office. I am not telling you anything new. This has been going on for 10 years.





I have seen it in action. It's a hassle for them. Go sell your BS on the VR board. - nm

HELP QA Pay By Blank???

Hi, I have been speaking with a company who will be hiring me for transcription.  They pay good rates and I asked them about the possiblity of job progression and eventually working as QA.  They said that QA pays by the blank... how is that even possible?  Ive seen being paid by the report, by the hour, or  by production (by the line)... but I have no idea how the QA staff make good money only getting paid for the blank.  any ideas?  Amy I missing something?  ...

So sick of the emails. I really do not care what number our "team" is in, I know no one on the team anyway. If they want to reduce blanks there are better ways - talk to the dictators like they promised years ago, improve the audio, improve ASR, yada yada yada. But no. Blame the MT instead. Silly contest. I was consistently going over the metrics for blanks being submitted to the client and decided to keep track of why I was sending those blanks.  It turns out that 77% of my blanks are due to either discrepancy or audio issues; blanks that I am positive nobody could fill in.  77%!  These metrics either need to be done away with or adjusted for doctors that don't take the time to not be idiots.  The most common discrepancy is incorrect age or something else that I can fill ...

This just tops of my bad day.  I get on to check previous reports from yesterday and see 1 correction on 1 report out of all of them.  QA filled in my blank, and the error category is "major" with subcategory "not applicable."  Platform is Cornerstone.  Since when is it a major error to leave a blank for something we aren't sure about?  After listening, again, I still can't tell if it is the word QA put in or the word I thought it "might" be; hence, the blank.& ...

From now on, I am blanking HEENT/HENT any time I am not 100%, absolutely, positively sure of what the doctor says.  Especially if they don't say anything at all (which is the usual).  This is utter ridiculousness and I am not going to risk a QA (or QC or whatever it is!)'s subjective opinion about whether a dictator said (or meant) one E or two!  What's next?  If they only dictate a finding on the right lung, do we have to label it as "RIGHT LUNG:" instead of ...


Endometrial Procedure Blank

This is a patient undergoing hysteroscopy, dilation and curettage, and a Mirena insertion.  It is an operative note.  I have came across this blank 3 times now and have spent way too much time searching for it.  Help! The hysteroscope was removed, and the cervix was further dilated.  Bozeman endometrial polyp forceps were then used, and the apparent endometrial polyp was removed.  Systematic curettings were then obtained until a  "s/l cree duh shah" filling in all ...
