Client Profile errors will probably end my career - anon

I never get points taken off for the content of my reports, those are perfect quality.  What kills me are the FULL POINTS taken off for client profile errors.  One single client profile error (it doesn't matter WHAT it is) will fail an audit.  It only takes ONE to FAIL. 

The problem I have with this is, there are soooooo many account profiles in my day and some of them (no joke) are a mile long with absolutely NO consistency to any of them.  One tiniest of details gets by you, and there goes your audit regardless of how perfect the work is, otherwise. 

I've failed my last 2 audits based on client profile errors alone - each audit, I have had one client profile error.  Those alone will probably be the end of my career.

I just failed my audit today with a 99.55%, and not because of the "quality" but because the doc put in the wrong facility code, and I didn't catch it and put it on hold. I was just told today that I cannot fail another audit for at least 6 months, or I will be terminated!! And... to make matters worse, the new passing audit rating is going up to 99.7 next month!!!! What human can possibly achieve that week after week??? I seriously think they are doing this on purpose to "weed out" employees instead of laying off. I just can't even breathe right now, I'm so upset. I don't understand why we have to take full responsibility for physician errors.

They don't want another massive layoff! This is a way to let go MTs that have a higher pay grade, and who have been here a long time. No one can maintain a goal of 99.6 or 99.7 accuracy week after week without their line count taking a hit, so it's a catch 22.

Exactly. It's either fast and sloppy or slow and good. High lines or high QA? Pick one! Grrr - n/m

Hi there,
I feel for you, as I had a "failed" audit of 98%, which shouldn't be too bad, EXCEPT for this company!

Also, they have written me up for lack of Production, NOT for the QA work which is just fine, BUT for the Editing MT work which they say I'm not producing enough. You must produce a LOT of work on this AND have a minimum accuracy of 99.7%!

Who do they think we are?

I know they are trying to get the Client Profiles standardized somewhat. But even on those, the PE comes before ROS. I don't know why they do that. I can scroll down the info as I do the report but I can't scroll past the PE because I haven't typed it yet.

I feel like I'm always messing up the client profile info. Sometimes I think I'm still on the report I did previously from one account but really on a different one. It would be nice if they could put a blue or pink background on patient info so I don't have to search through the info which is not alphabetized. And while I'm at it, why can't they put the age in there?!

Sheesh, I hate to think that I could get fired for these CP things.

I have one account consisting of office notes for one facility. Every office wants their reports formatted differently!!! So, I have to re-read each profile every single time even though it is the same facility.

I resigned from MModal in November.  It was the best thing I ever did.  I had worked for them for close to 30 years but the NJA and the hours and pay cuts really started to get to me, so I started checking periodically at all my local hospitals.  I am lucky to live in an area where there are several major hospitals.  Lo and behold one day a transcription job came up and I jumped on it.  What a joy to go to work every day now.  I thought I would hate working outside the home but honestly it is so much easier.  The hours are regular, the pay is good (17.25) an hour.  A group of about 15 physicians who are all good dictators.  There are 3 transcriptionists in the office.  No one fusses if you hypenate follow up or not or any of the other things we worried about at MModal. So many little things that I used to have to worry about are no big deal now.  The doctors seriously don't care about this little nit picky crap.  Best move I ever made.  Keep looking, there are still jobs out there.  I'm 55 years old - hopefully it will last until retirement.  If it doesn't, at least I get a regular pay check and weekends off for as long as it does.

There is a very rare opening for *first shift* MT at a very notable local hospital here in my town and I've been staring at the application on the website for a few days now. I've been wondering what the pay range might be for a position in-house and I know it varies by city, hospital, etc, but your post at least gives me a good idea, so thank you ... I think me reading about your experience was a sign.

Yikes, this is a little bit scary because I've been with MModal for 15 years but I did it - I just submitted my online application just now!

I go back and forth wondering if it wouldn't just be better to quit instead of being fired for some ridiculous reason. I'd hate for that to be a part of my work history. I've never been fired in my working life, and it would gall me to have this place be the first. Plus, having that on my record might hurt my chances at landing a better job in the future.

Have any of you found it more difficult to find a job after being terminated as opposed to quitting? I appreciate any responses. We sure are all in a big mess here, but heavy decisions have to be made...

I see the writing on the wall. There is no humanly way I will be able to sustain an audit score of 99.7 or higher each audit for the next 6 months. I have actively started to search, and unfortunately there is not much out there. I didn't sleep at all last night, just praying to God that I get through this. We live paycheck to paycheck as it is since taking almost a 25K loss a few years ago when editing came around, and then when MM took over more cuts... we cut back and made due. But making zero $$ will destroy us. I feel like I have let my family down. I also have NEVER been fired from a job.. and NEVER written up EVER!! What a demoralizing situation :(

I knew I needed to quit but did not want to lose the money that I was bringing in. After 3 lower scores of less than 99.6, I was terminated. It wasn't long...only 3 weeks.

I took a job in a small locally-owned clinic for 3 docs and 2 PA's. Wearing slacks instead of jeans and actually driving 3 miles to work are the only drawbacks. I started at $17 per hour and when evaluation came, they gave me double what the other office staff got because they were so thrilled with my performance. Once in a while I start feeling stressed and I take a deep breath and remember "you work by the hour now, not by the line, so take it easy." Keep looking and get out if you can. It will save you literally.

Lucky you! I live in Central California, and pretty much all the clinics and hospitals are outsourcing the work. In-house transcription in this area is unheard of; it's cheaper to send it out. When I did work for a large group, they closed the department because of health care, 401K match, profit sharing and our wages were really good! Oh, those were the good ol' days. I wish you much luck.

I received a work type which has recently been changed over from a mandatory standard to headings. As I get shuffled around so many different accounts, so, of course I haven't seen this work type enough to remember where the body heading goes. So I inform my TSM of the deficiency that needs to be corrected as I spent considerable time checking past reports. It's probably way too much to ask but would it kill them to set up the CPs the same way for each one? Like "qualified verbatim" in bold at the top, patient name in report, phys exam setup, ros setup, ADT instructions, etc.  The CP might just be the thing I despise the most. ...

I get dinged for leaving out words like the, and others.  I am now understanding that this is not the correct policy, that others are not getting critical error, major error, off one point for this.  So, am I being singled out? Well, I certainly feel sorry for those 60 MTs they were able to persuade to go to work for them -- wonder what kind of promises they made them??  They've gotten this procedure down pat over the years -- drawing in unsuspecting MTs with promises of making more money.  All I have to say is good luck, especially since they probably failed to tell them about all of the wonderful changes that are happening, like loss of pay for sending to QA -- make sure you've updated your antide ...

They should have one standard format across all clients and if any client wants it changed, they should charge them per change or addition a recurring fee.  At the very least they should organize them better.  Maybe take advantage of different font colors, so they are easy to spot in the CP panel, like:  Physical exam: Stacked left, all caps subheadings.  and then pick a color for exceptions and put them all in red so its easier to spot.  ...

Thirty different accounts, all with confusing and convoluted CPs.  Follow the DQS, then the TSG, then AAMT, then DQS and throw in the JCAHO dangerous abbreviation list just for funsies.  Wait...  Maybe it's the AAMT, TSG, then BOS, only this client is using BOS3, so disregard the BOS2 unless it is Dr. Bobsmyknob who wants his verbatim, but only when he is at the outreach clinic.  And if the CP doesn't specifically say it, DON'T DO IT no matter what the DQS, AAM ...

Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE?   All of the sudden, after no feedback for about a month, no audits, nada, zilch, I got my audit and was given critical feedback for not capitilizing a department! (recovery room, radiology suite, cardiac cath lab...)  AND, in a verbatim report, with a client profile that says specifically, DO NOT ADD A HEADING IF ONE IS NOT DICTATED, for NOT adding a heading!  This makes me nuts ...

I work in a surgery center/clinic in medical transcription. I have never worked in the medical field before, so I'm not sure what is acceptable. We get massive amounts of documentation errors from the nurses in the surgery center and in the clinic. Is this usually acceptable?! Is it normal for a transcriptionist to be required to scan over the chart and correct all of their errors or should this be the responsibility of the nursing staff? It's DRIVING me up a wall!
