QA errors - newbie

What do I do if QA marks me for a major error but I think they made a mistake? The really wierd part is that the feedback page says it was NOT modified. Seems so strange to me that QA wouldn't modify a report with a major error.

You can request reversal and explain why with - your TSM. They do not

automatically correct/change reports that have been delivered to the facility based on errors found on post audit if that's what you mean by modified.

Depending on the QC people involved, if you do get one reversed you will find yourself being audited like crazy until they find something they can gig you on. Not all QC, but had this happen to me before.

I really just want to learn from my mistakes so if I don't know what they are, I can't learn anything. I think QA should be educational, not punitive.

And I actually was QC - Tried to get a couple of things reversed, always denied. Then the higher up QA's started nitpicking me on every period, comma, etc., until I lost my QC position. About ready to dump this company.

If I ask the TSM and they don't help, who could I ask above them? I really feel strongly that QA should be instructing me on what I did wrong and how to fix it. Otherwise, I will just keep making the same mistake over and over. What is the point of that? I'll probably get fired over this!

instruction. If you're just out of school you should have a mentor doing the instruction. FYI also, what you feel is major and what the company feels is major are probably 2 different things. They have a list of errors and the severity they go by.

I mean I'm new to Nuance. I have many years of experience and could probably be a QA myself. I don't know what a performance plan or SM means. I feel like I have a bag over my head with all this!

Reversals - Butting heads

I get so many errors, but all from 1 QC. So I send almost all of them back for reversals. I figure maybe if my TSM gets tired of seeing them, she might say something. I do have a question though. Some of my errors are I swear things I changed, yet it looks like I didn't change them and I got dinged for it. ??

performance plan is, that's good because that's what people are put on when their quality or production isn't up to par.

If you're looking for "why" something was marked the way it is, you're not going to get that kind of feedback. The assumption is you can see the correction and if you don't understand it, go to the transcription guidelines in FIESA and you're likely to find the answer there or in the account specifics.

I think it just takes out extra spaces or something.

Don't let others responses get you down - the explanations and training are very lacking IMHO. If you are ambulatory, we are not on Fiesa .yet. Maybe that changes tomorrow, who knows.

I have learned much of what I know from finding the manual, and just clicking on things in the program .. LOL

I appreciate your response. After getting halfway through the Fiesa training module I tried to log in but couldn't. Then I sent out various emails and IMs trying to find out why. What a waste of time. I think I need to just go back and redo my InScribe training module to fill in the blanks from what I might have missed the first time. It was a lot to take in all at once anyway. I am so used to minimal, but helpful QA from my old company that this is driving me crazy. I kind of thought it would be since I used to work for a similar big company.

That is exactly what they want you to do, particularly errors that clearly are not putting patient care at risk. The whole set up is to be punitive, not helpful or educational. Smoke and mirrors!

You can click on request reversal right from the - Fiesa screen for that job.

It it's no wonder you don't know why. If you don't know how to use it you need to go to ENRICH and learn. There's an entire tutorial there.

Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE? All of the sudden, after no feedback for about a month, no audits, nada, zilch, I got my audit and was given critical feedback for not capitilizing a department! (recovery room, radiology suite, cardiac cath lab...) AND, in a verbatim report, with a client profile that says specifically, DO NOT ADD A HEADING IF ONE IS NOT DICTATED, for NOT adding a heading! If we get a "syntax error" message on a post we have just made, how do we get back in to correct the error without losing the entire post? Every so often I forget, and put an apostrophe in the subject line, and end up losing the entire post. I work in a surgery center/clinic in medical transcription. I have never worked in the medical field before, so I'm not sure what is acceptable. We get massive amounts of documentation errors from the nurses in the surgery center and in the clinic. Is this usually acceptable?! Is it normal for a transcriptionist to be required to scan over the chart and correct all of their errors or should this be the responsibility of the nursing staff? It's DRIVING me up a wall! PLEASE NO NEGATIVE FEEDBACK! This is truly meant to be helpful, not snotty, threatening, or negative so I expect the same in return. I have been working for Nuance for 5-1/2 years now. I, of course, consider myself a very conscientious and diligent MLS. Lately, I have noticed "mistakes" that I really DO NOT BELIEVE I made. Has anyone else noticed the numerous errors on ASR?? Words left out, he/she incorrect, the strangest of phrases being picked up by the computer that are not even dictated and the phrases picked up do not even come close to what is really being said. Here are some of my observances of chronic ASR errors, though sometimes it gets it right. Most are not a biggie, but one could get marked down on an audit. 1. BiPaP. Should be BiPAP. 2. working as a coder, I read all the reports. It always amazes me what goes through. today I coded an MRI that had the indication "Left upper Germany tingling!" Maybe what you are seeing is already the result of many people like yourself who have already left the field for the same reason you did. When you get the responsible, diligent workers leaving because the pay has been cut so drastically they can't afford to work anymore, the field becomes saturated with new or substandard workers. Never in the last year have I ever ever gotten 4 major errors in one report, I don't think I have ever gotten 4 in 3 or 4 months. They will never be able to convince me that something hasn't changed. I rebooted my computer Monday after it had been off for a week. Got exception errors causing Smartype to dump and not allowing speech rec to start. Anyone have any idea what might be causing this or a fix? My solution i to use my clunker, so I'm not quite dead in the water.
