1 point errors, critical errors. - MT

I get dinged for leaving out words like the, and others.  I am now understanding that this is not the correct policy, that others are not getting critical error, major error, off one point for this.  So, am I being singled out? Because I thought this was the norm.  I'd like to know so that while I am hanging on every word and will continue to do so, I thought I was not the only one with this...thanks in advance for the update.

I,too, have had points for unimportant words like "the, a" etc. This verbatim stuff is carried way too far. I remember when doctors I used to transcribe for told me to make what they said sound appropriate, i.e. good grammar and sentence structure, but nothing that change the medical content or meaning of the report. That is how it should still be. I have to transcribe sentences that either make no sense or end up backwards from how they should really be worded, all due to typing verbatim. I know there are legal issues these days that make everyone scared to death about law suits, but when MTs are graded severely for missing or adding words that do not change the medical substance in any way at all, it is getting pretty ridiculous. Some doctors pause and while thinking they say "ah" and this sounds like an "a" should be put there instead. QA people need to lighten up with this kind of so called error and focus on what really matters in a report, such as medication, dosages, vital signs, etc. Not prepositions and pronouns. If we do not maintain 98% we are put on full audit and then if we fail, we lose the work! That is serious and not fair at all. Just what an MT needs, with all the other stress that comes with this job, a hammer hanging over her head, waiting to fall and take her accounts away, all for errors that do not amount to a hill of beans!

Any QA who indulges in petty nit-picking like this should be reported for each and every occurrence. Just as all are not cut-out to be MTs, QA is not for those on a power trip and those incapable of differentiating between genuine errors that affect meaning and petty splitting of hairs. It's shameful enough that MTSOs victimize their revenue-producing MT employees, QAs who further hinder their fellow MTs with deductions for trivialities not included in BOS or account specs and which are really their own personal preferences should not be in QA position.

I have been dinged for those same "errors" so it is not just you. NM - anon

I,too, have been dinged for these errors. I, on the other hand, fight the QA on this. It is NOT a major deduction. I get my stuff reversed almost every time I fight the QA on their marks.

I was told by some other MTs from this co that I was wrong saying that I was counted off for 1 point/critical/major for leaving out a the. I also have gotten in trouble for not saying mag when I put magnesium as a critical error. I also get in trouble when I leave an "a" but get into trouble when I put a because the doc stuttered and "changed mind." All these things are from my experience and the truth to the point where I am blanking anything and putting in the QA note (doc changed mind, etc.) I am not getting into trouble for questioning even the smallest thing, but I am not lying in my experience that my e mails from major errors come from these kinds of errors. Thanks for validating me, b/c I started thinking I was picked on and the only one. Though I am not happy b/c misery loves company, I appreciate knowing I am not alone in a vast wasteland of pickiness...

I used to ignore FIESA. Now I guess I have to ignore my Emails. They just make things up and hope that you don't challenge it. I used to, but I don't have time anymore, nor do I care.

The challenges go unanswered. My TL said not to spend time challenging b/c we have to worry about our QA percentage and lph and downtime is not given for challenges. I have very little time in my personal life without getting aggravated over the and, a, etc. If it is something which is informational I ask my TL to clarify so I don't type the wrong thing if the QA ding goes against acct specs, but otherwise I just watch every single little word I hear/see when I am on the job. I won't get the bonus. So, I revert to doing my best and glad to have a job.

This thread so came at the right time for me. Today I received a nasty email from my QA telling me that my reports looked terrible today and they sent them to me to look at. I freaked out and when I looked at them, I had the same types of errors you did...which I thought was pretty good considering none of the doctors could speak English. I'm just sick to death of this, my days are numbered as soon as I can find something else. Not worth the aggravation for me any longer. I can find a job making more money sticking fries in a bag.

As an IC who interacts directly with clients, I will tell you that I DO get reports back from physicians for such little things as this, for example "a" instead of "the," etc., and I am anal with regard to quality and try my best. but sometimes these are not heard, especially on cell dictation, which this physician sometimes dictates on. It is more important than you think. I ended up putting a disclaimer on my log sheet, stating something in regard to this. They are the client, and if that is what they want, that is what they will get (the tenaculum instead of a tenaculum).

However, I do not "ding" my transcriptionist for it but insert it myself.

I work in a surgery center/clinic in medical transcription. I have never worked in the medical field before, so I'm not sure what is acceptable. We get massive amounts of documentation errors from the nurses in the surgery center and in the clinic. Is this usually acceptable?! Is it normal for a transcriptionist to be required to scan over the chart and correct all of their errors or should this be the responsibility of the nursing staff? It's DRIVING me up a wall!
