The providers are responsible for the content of their dictated reports, not the MT. If the provider signs the report, then it is his/her problem.. it’s not your responsibility, unless it’s your medical record, to report “outrageous behavior.” On Epic reports generated by voice rec, facilities include disclaimers that they may contain errors. Maybe PF needs to do something like that to show that the reports have been altered.
