I don't know about the lab value situation, but honestly, they only leave out stuff that is not pertinent to the reason the patient is there. For example, "The patient is proud of her son, who is a business owner in town." I've had something like that where I have had to leave it out. Otherwise, we don't leave out dictation like you are saying. We may put "No" for "Does not have" or something like that, but we don't "leave out" dictation like some are saying.
