I have a Creative labs sound card that has a 13-band equalizer so that I can drop down frequency bands and sometimes this will clear up that hiss and hum.
It also permits loudness upper limits of 127 dB (not recomnended for a long document) but that helps with the very soft-spoken words.
I have noticed some benefit from a Grado SR80 headphone which extends out to 20 Hz on the frequency range so I can hear the higher pitches.
A lot of the problem begins at the dictator equipment. So, there is only so much we can do.
