I've seen people say drs are fed up with VR. Specifically, how would the poster know that and know it is happening on a broad scale?

I wondered the same thing. - How could anyone know

Doctors aren't commenting on VR on my dictation. I doubt they know they are on VR versus plain voice transcription or who is transcribing or editing their work.

Than for the drs to have figured out that the emperor has no clothes! But I'd like to know it's true and not just wishful thinking on someone's part!

We have acquired several new accounts from doctors who are fed up with voice wreck and they decided they were willing to pay to go back to the old way. Our rates are high and they have to be if the doctors want quality reports that can go directly to the EMR with just a minimal amount of fuss. Our clients are willing to pay our rates because they know the work is coming back as perfect as possible, quickly, and any questions we have about a specific report get highlighted and a detailed note sent so that they are aware when there is a problem of any kind. Plus we are available during business hours for stats. We do not charge extra for this, but business policies are up to you.

Also, my son's best friend from college is now a doctor who practices in another state. He has told my son that his colleagues feel the same way; they just want to go back to the way things were. The want to dictate reports, be done with it, and have work comes back the next day done properly. The problem is they think that there is a lack of qualified MTs to do the work.

If you are an acute care MT, you can certainly transcribe outpatient medical records quickly and easily. I know it is even more boring than hospital work, which I find plenty boring after nearly 40 years, but so what?

So instead of just accepting this miserable treatment from the national MTSOs, make up brochures, get some business cards, do something with Facebook or a website, get listed in the yellow pages, make some business calls, send out mailers, and let those fed up doctors know you are around. And charge enough to make it worth your while, it is still a big savings to them.

Be prepared also, you might be offered an in-house position instead, and unless you are really attached to staying at home, or having your own business, it might be worth considering.

You really don't have much to lose except some free time and the costs of advertising yourself, which are just not that high.

I wouldn't even know where to start, although I do appreciate your advice. Do drs use hand-held recorders or what. Would doing MT myself involve investing in some huge computer system for the drs to dictate into and download to me? TIA.

This would be great except most MDs are forced to use EMR (EHR) now and no need for a transcriptionist. I feel this will be the case for hospitals in 2015.

If this plan is working for you, that's terrific. But the truth is more and more doctors are becoming employees as opposed to individual owners or partners. Even very lucrative practices are being sold to hospital, en masse.
Secondly, there are EHR mandates and deadlines that will force many doctors now dictating into the EHR format, or face significant financial penalties for failure to do so.
The physicians are no longer the masters of their own universes that they have been in years past.

For a great many of them, those who work in employee status, the answer would be no. It would be up to any fed-up doctor(s) to prevail upon their own managers that a change was needed.
But, there is also the legally mandated move to EHR. There is certainly some dictation that will be compatible with EHR, but the quantity is declining. Practices that are not in compliance will see their Medicare reimbursements decrease.
Thus, the grim future for MT.

The days of the solo practitioner and 2-3 physicians together are over in my area. Aside from that, most physicians do not know who is doing transcription or how it is transcribed and/or edited. A business manager gets bids from companies like Nuance and the lowest bid gets the contract for the whole group. Contracts are not easily broken if one doctor decides he does not like the style of the transcription.

I have not commented on it but my niece is a cardiologist and in a private practice but of course has hospital privileges and her husband is a dentist. Neither one of them nor their colleagues use it due to the amount of time spent in having to correct the work. They found it was double work on their staff and just found it easier to dictate and let it be typed from scratch and was much easier on them. This is in Florida and they both said that they don't know any of their colleagues who are using it presently due to it being a pain and wasn't saving them any money

I just don't understand how any of these private practices need an MT anymore. All I know are forced into EMR and no transcriptionist is part of the equation.

I think you are seriously misunderstanding this. EHRs are electronic records ... period. They do not eliminate MTs any more than photocopiers eliminated MTs.

It is the doctor or hospital's choice to eliminate MTs, and a lot of them go this route at first because it seems economical.

We have a full-fledged EHR. All we have to do is upload the Word file to it. Our doctors can choose to type it themselves with or without point-and-click templates (most do), use front-end SR (many do), or use regular transcription.

checking the QA box, it does not go through QA, right?  I know we are not supposed to put in disparaging remarks, but would "speak up" or "stop mumbling" be considered a disparaging remark? I am so tired of these docs who mumble and cannot be understood...how else will they know we can't hear/understand them if we can't tell them...all they see is the nice finished product not what we have to go through to get them that.    ...

or am i getting the feeling that after reading some of these posts that certain ones are "targeted" to have regular audits.  have you done something in the past that puts on a "special" list that you are audited on a regular basis?  I have worked for the Q was not audited before (that I know of).  I have been audited twice now in the last couple of months and have passed, but did I do something to become a "chosen one?"  hhhmmmmm ...

I don't see anyone talking about the test itself or working coders or coding students bringing it up, so I will. I went to school for coding, got certified and could not find a job. That aside, the A and P portion of my schooling was extremely intense. I have been an MT for 15 years and I had no idea how lacking my knowledge of A/P really was. I'm going to test this out myself the next time I use one.  If this is true, sure would explain my slightly plummeting line count from usual no matter how hard I try.  That would be some BS for sure.   Makes you wonder why they make that "rule" that we are not allowed to make our own expanders from their standards!!!!  AHA!  Well, they have no way of knowing, so that is EXACTLY what I am going to do if I find out we aren't compensated for those standards. ...

FIESA Comment "Downgraded As Clear Throughout The Report"

I recently had 0.25 error in FIESA for having "complaining of lateral-sided pain" instead of "complaining of left lateral-sided pain."  The comment that the reviewer made was "Downgraded as clear throughout the report." What's Up With This Ad On Another Board For OB

You see it periodically, OB, radiology, and something else but it's pretty clear the work will be OB.  The test consists of 3 files (getting this from the ad) and you should finish it in less than 2 hours.  3 files?  How long are the files?  So, my real question is (the other two were thiinking out loud), does this ad get run periodically to clear out the backlog or is it really a legit job and why does it come up so often?  It's not the exact same ad everytime ...
