Did you ever want to write a letter to the client(s) and say,...Really???......  I don't get paid to sit here and listen to you take phone calls, read the chart, go to the bathroom (disgusting), or have conversations with your staff.  If you are not ready to dictate, hang up.  If you cannot pronounce a drug, spell it instead of murdering it by acting like you do.  You want to dictate on your cell phone in your car, or other noisy places....listen to yourself.  Pay attention to whether you are dictating on a male or a female.  Is it the right leg or left, you have changed your mind 3 times!!  Do not try to use big words to impress people if you cannot say them, and stop MAKING UP WORDS that do not exist.  If you want quality reports, LEARN HOW TO DICTATE!

Also, if I had the nerve, I would write to some of the MTSOs who advertise for jobs and inquire.....why don't you advertise the pay (and the true pay), the hours, the line requirement?  Is your platform a clunker that limits my production?  Do you put anything and everything on VR, even if it does not belong there?  What are your true benefits, PTO?  How is that attained?  Do you have enough work to supply me with 1,800 lines a day consistently?  Are you over-hiring?  Do you pay OT pay when OT is required?

Years ago, in-house, there were 7 MTs. We made a Bloopers Tape of the doctors and the administrator played it over the PA system at the annual Christmas party!! Talk about teaching them a lesson. They went something like this....

Hello, this is Dr. John Doe....(belch)
Hello, this is Dr. John Doe....(belch) (about 4-5 times)

Dr. John Smith here,.......(loud fart)
Dr. John Smith here,.......(loud fart) Again, 4-5 times.

Dr. Jones...........(cuss, cuss)
Dr. Jones...........(cuss, cuss)

Dr. Johnson...... "This is an MRI of the brain on...on...on...MRI of the brain on somebody, hold on....Okay, maybe I need an MRI of the brain."

Dr. Williams........stumbling terribly, and then "Please do not play this at the Christmas party.....never mind, I will erase it." Then he erased most of his dictation and had to start over.

The staff was hysterical!!! They had the back-up feature to erase, but would never use it until we played that Bloopers tape.

we had an ER doc one time ask the nurse "does the carpet match the drapes?" and then go on to ask if she was a "red snapper" HONESTLY...they really need to listen to their dictations. Another doc was getting "pleasured" by his wife and he says "Come on, honey....I'm dictating....Stop..."

dear gabby, or roses some of those similar newspaper articles might would post them. Think it could be a start? Even if you solely did it in a bloopers way as above at least someone would find humor in it and hopefully share those thoughts with any loved ones they know in a dictating position.

And honestly, they need real feedback. Not to mention we need the catharsis of telling these guys what a pack of idiots they are. They would never call their loved ones on a cellphone and mumble like that. In a two way conversation, the other person could tell you they don't understand a thing you're saying, but we don't get that luxury and our MTSOs don't stand up for us. In fact, I chiefly blame my company for originally creating an atmosphere of anything goes dictation. Two decades ago Medquist started a policy of sucking up to the hospitals and taking "trouble" accounts. It's like feeding the bears. All the hospitals got bloated and rebellious.

I used to have this one doctor who didn't speak English and would beg me not to leave blanks. He kept giving his phone number and pleading that I just call him and he would help me with anything I didn't understand. Of course, I couldn't call him. But the bigger issue for me was, why not just learn English? It's not like he was new here. He'd been practicing in America for over a decade. He's supposedly a doctor. Shouldn't someone who can get an M.D. theoretically be smart enough to learn English?

If I moved to a country that spoke a different language and I knew that I was going to be staying there, much less practicing medicine and holding people's lives in my hands, you can be sure I would do whatever it took to learn that language. Especially if I made doctor money and could get all the language tutoring, Rosetta Stone courses and speech therapy necessary to learn even if I was dumb as a sack of doorknobs. It's not like it's just dictation. The other doctors, nurses, patients, family members, etc. probably don't have a clue what they're talking about.

By not allowing us to correspond with the hospital, even by representation, the MTSOs are doing everything they can to keep a human face off transcription. As long as we are a collection of unseen workers, overseas laborers and robots (ASR), even the halfway decent doctors don't have to feel the slightest bit of empathy. They dictate the report however they want and it comes back cleaned and polished. They don't see the blood, sweat and tears we pour into it.

If only we could organize and have all of us send anonymous letters. If a few thousand MTs sent tens of thousands of letters, that would be hilarious to see the hospitals inundated with our helpful criticism;)

Someday, I'd like to write a letter inquiring/requesting to know which drug(s) the dictator is currently whacked out on (Valium, etc). As you say, normal (non-foreign) people don't talk like that, slurring and mangling all their words. It's so unprofessional. They sound like they have speech impediments, but of course they don't really.

One time I saw a show on TV that included the question why doctors always keep patients waiting for so long. The doctor interviewed was honest enough to admit that it made him feel very important to be able to keep people waiting in his waiting room.. An honest moron.

One hospital I worked for in the past required doctors who's handwriting was illegible to take a course so that they wouldn't kill people with their written prescriptions. Should be the same with dictation IMO.

dangerousdictators.com. MTs could send in their "horror stories" anonymously, and I mean name names, names of hospitals, even post their pictures if available to call them out. MTSOs and hospitals seem to have no interest in correcting this situation, so it is up to us to take matters into our own hands!

A few weeks ago, for no reason that I can find, I cannot get the first letter of a sentence to be in all caps automatically, (unless I do it manually.)  The same for the letter " I " by itself when typed.   It was always automatic and now all of a sudden it is doing this and I cannot figure out how to change it back. Well, I certainly feel sorry for those 60 MTs they were able to persuade to go to work for them -- wonder what kind of promises they made them??  They've gotten this procedure down pat over the years -- drawing in unsuspecting MTs with promises of making more money.  All I have to say is good luck, especially since they probably failed to tell them about all of the wonderful changes that are happening, like loss of pay for sending to QA -- make sure you've updated your antide ...

They should have one standard format across all clients and if any client wants it changed, they should charge them per change or addition a recurring fee.  At the very least they should organize them better.  Maybe take advantage of different font colors, so they are easy to spot in the CP panel, like:  Physical exam: Stacked left, all caps subheadings.  and then pick a color for exceptions and put them all in red so its easier to spot.  ...

As I get shuffled around so many different accounts, so, of course I haven't seen this work type enough to remember where the body heading goes.
