Is mild-to-moderate hearing loss correct or should it be mild to moderate without the hyphens?

I think the BOS - is moving away from hyphens

but I still put them in for cases such as that, and I have had no problem with my accounts or QA coming back on me for it.

I don't think this case calls for hyphens, BOS rules or not, but I guess if there no preceding verb; i.e., "the patient's mild-to-moderate hearing loss...", it wouldn't sound as bad as if there were a preceding verb; i.e., "the patient had mild to moderate hearing loss."

Or, I could be crazy and off the mark....but either way I wouldn't hyphenate this, and I tend to be pretty hyphen-happy!

Lately, I have been getting dinged for not putting hyphens in mild/moderate to moderate/severe plaque or moderate to severe htn or mild to moderate anything.  I don't understand why we would need hyphens.  In that case, shouldn't "anything to anything" need hyphens? ...

Which way should moderate to severe by typed in this context? 1.  moderate to severe trabeculation 2.  moderate-to-severe trabeculation I was just dinged by QA (would prefer not to say which one yet), and really need to know.  Also, I was always advised when typing a CBC, either all items should be expanded to thousands or none.   It must be consistent.   Is this also true? Anyone out there have any hearing loss? I have developed significant hearing loss. My ENT has encouraged me to keep transcribing, with the use of a boosted headset and I am trying. Telephone use is very difficult and almost non-existent. Boosters do not help. Also diagnosed recently with RA and having difficulty with my fingers, but not letting it get me down.
