"the importance of smoking sensation was reviewed" one more time, I'm going to lose my mind!  Doing a string of notes by the world's laziest, fastest dictator.  So tempted to transcribe verbatim and see what happens!

what the words they're mangling actually mean?

I'm hearing a lot of mangling these days from NPs working under/dictating for hospitalists. One such -- a native English-speaking American, BTW -- always dictates hematocrit as "hemacrit". Same person gives me amaRODEOne -- every time -- for amiodarone.

I get "hematopoietic" as a subheading in the ROS quite a lot,too, so obviously she can wrap her tongue around long medical terms.

Lazy dictation, or terminology ignorance? Makes you wonder. I get enough consistent mispronunciation from the same people to hope that I don't encounter them as a patient.

Nobody working in cardiology can pronounce fibrillation it appears. It seems like some of the specialists don't know how to pronounce the terms of their own specialty, and I'm not even talking about the ESLs. I frankly wonder if some of the mispronunciations are a kind of affectation. There was one doctor I used to have who routinely mangled the names of medications, i.e., liosinopril (and that was one of the better ones), although sometimes he would pronounce it right so I knew he was capable of it. However, someone must not have cleaned up his meds for him because he started to spell some of the meds out - saying one med and spelling something else.

... for the specialists, anyway. If their mentor (usually the attending when they are residents) has a distinctive affectation in dictating, they tend to pick it up. The way a doc dictates his/her ROS and physical exam is often "inherited", too.

I used to think it would be fascinating to trace back some of these recurring affectations to the med school/teaching hospital where they originated ... kind of like a genealogy of medical dictation. Of course, that was back when I a) had the luxury of free time to muse about such things and b) loved my profession.

They think that since there is a Cipro for ciprofloxacin, there's a Ceftaz for for ceftriaxone.

Honestly, I get why they use verbal shorthand (easier to say "nonstemmy" than "non-ST elevation MI", and "vanc" for "vancomycin") but lazy dictation certainly makes our jobs as mind-readers harder than it already is.

Just started a new account and have two irks
1) ER doc repeatedly says "tenderness to palpitations."
2) Pain med nurses pronounce "aberrant" as "apparent" w/ the stress on the wrong syllable. This one drove me nuts on my first job for them as I had no clue what kind of behavior the pt had if it were "apparent" behavior.

Someone needs to add "translator" to our job description.

have been tempted to pad my lines by transcribing it as "poor dental attention." At least that way I'd be getting paid extra to interpret her garble and maybe if/when she sees my translation, she'll dictate with more care. Probably not, though.

Hey MTSOs, I think you need to pay attention.  According to a news release, Wal-Mart is INCREASING their minimum wage pay to $9 by April of this year and then no less than $10 by February 2016.  Pretty bad when a local grocery chain knows how to treat their employees better than you do...not bring then down like you do.  They are trying to shed their bad image.  MTSOs...I bet you dont have the grapes to do the same thing WITHOUT raising production requirements.  No grape ...

they blame it on the transcriptionists. I get told that this must be the way the transcriptions are editing the output. Are there really that may bad/horrible transcriptionists in the company??? I don't believe there are that many poor transcriptions..... unless they are using ILP's to train the ASR. Maybe that is the question I should be asking: Is the output so poor because you (the software techs) including ILP editing?? ...

There are some weekends where the dictation is tolerable.  This must be MM's "idiot dictators weekend" because that is all I've had today.  Ranging from the moron who tries to dictate with so much static on the line that I have to leave 15+ blanks because SR didn't "recognize" anything to the dumb one that dictates 300 seconds of a report but leaves the phone off the hook so I get to hear conversations, intercoms, beeping, phones ringing, banging, etc., for 1500 seconds. ...

Is it me or is it that the sound quality is getting worse and worse on these accounts.  Ever since last year when they had the town hall meeting saying they were making improvements (e mails, etc.) and then they got bought, seems all that improving stopped.  Half of the work I get every day or more are simple inaudible, I cannot hear or the static is so bad, or the sound so low, I have to send to QA b/c these are not intelligible. I work for 96 physicians who (rightfylly so) expect the best and fastest quality transciption. However, when seen as a patient, I get the most horrid treatment (and fradulent) treatment possible (most most by clients.) I not only receive "steroid" injections that obviously have no steroids (only receive a few hours of pain relief) to dental care with crowns falling off after 2 weeks (I have had temporary crowns on as long as 3-4 years). No more microscopic QA dissection for me. My new plan is to shoot the job full of blanks and send it in - far less stressful for sure. I know my hearing has not changed but it seems that the sound quality of the dictation lately has changed.  It doesn't matter whether I'm using a cphone or downloaded voice files.  On A LOT of the dictations there seems to be a whooshing sound (for lack of a better description) in the background.   On others there seems to be a squeaking sound as if the doctor is playing with the phone cord.  Is this because the docs are now dictating directly into the computer ...
