4 major errors in one report!!!! - new but old

Never in the last year have I ever ever gotten 4 major errors in one report, I don't think I have ever gotten 4 in 3 or 4 months.  They will never be able to convince me that something hasn't changed.  I am being marked off for things that are completely ridiculous and some I don't think I even did.  Wow - what a way to start my day - makes me really want to work hard for them.

Mine were on the same report for a doc who is 100% QA because of all the inconsistencies in his dictations and his style is such you can't determine where he intends the sentence to end or continue. Then will change the work type from a consult to a followup by the way he dictates, not by stating such. None of his should be used as a scoring tool .

You'd think they'd get a clue that how they treat us directly impacts our morale and thus our willingness to go the extra mile when needed. Our TSM actually sent an email saying she was embarrased and saddened about our TAT then a little later came an email begging for extra help. What?!?! Yeah, I really feel inspired to give extra after being kicked around by QA, disrupting my home life by having to "flex" my schedule so much, and being brow-beaten by these emails. Sorry about your "major errors". :( QA has gotten so far out of control that it's beyond ridiculous.

Yeah, when you go from having always good QA/QC percentages all the time and then this. I admit that I may make mistakes but I certainly haven't gone off the deep end as it seems they are trying to make me feel. I very rarely have over 2 blanks in any of my reports and I feel like they are just pulling things out of the air to ding me for!!

The QC is trying to make a buck just like the MT. I don't blame them, because I know the quality of my work when it goes in, and they are hard pressed to find a bonafide error. What choice does that leave QC? They have to make it up.

This is a company generated problem. It reminds me of field work. When I was a kid I worked in a tobacco field. We all worked hard, because we made money at it. At 12 years old I made MORE money tying tobacco ($130 a day) than I make in two days of transcription. Then, when the farmer would leave the field one or two of the workers would lay down and not work, and when they saw the farmer coming they'd jump up and pretend they had been working all along. There will always be that kind. Someone complained, so the farmer instead of firing those two decided it would be better to offer a "snitch reward," so we could monitor the work of our coworkers.

This QC incentive to "find errors," is nothing more than that. If they can't find something - - - they'll make it up.

In practice we have Skinner's bell-ringing dogs at work ringing the bell for their rewards.

Is it just QC getting an incentive for making up blanks? - or is QA getting them too?

I ignore QC completely. Never bother looking at the dozens of email alerts I have been getting lately from QC.

But, if QA is getting a bonus for more blanks, we will all be getting 70% or less on our post audit scores very, very soon.

notify the facility when they caught critical errors. Doesn't this make the company look bad to the facilities? I also thought the facility did not have to pay for reports with critical errors. I might be wrong about this but this is what I was told a couple of years ago.

Note that Nuance never published its many changes - to its specifications, just started docking

the errors pointed out by QC? I've been here a little over a year and I get those errors all the time. But I don't get threatened about them. I was just wondering if anyone actually gets fired over them?

I know full well that I will never get their bonus, so I just don't pay attention to them. I type my fingers off, do the best job I can without dwelling on someone else correcting my work, and get paid on PRODUCTION. To heck with those bonuses, especially if they are going to ding and dong you to the point of keeping you from getting one.

I know these don't count towards bonuses etc but it still unnerves me to open my email nearly every day and seeing these stupid alerts with my percentage in the tank.  Some of them I take full credit for others I dispute but it still shakes me up so therefore some of the mistakes I make is just because I am nervous about literally everything I type.  ...

Got 1 last night and 1 this morning.  One I have to admit was definitely my error, the other bul &*&(&(.   Hope the QA people choke on their bonuses.  Have an interview today (non-MT) with benefits and decent pay.  Tired of working so hard to perfect their SR program so they can jack us over either by sending it overseas or finding they need less and less of us as the program gets better (if indeed it ever does).  Wish me luck.  ...

I get dinged for leaving out words like the, and others.  I am now understanding that this is not the correct policy, that others are not getting critical error, major error, off one point for this.  So, am I being singled out? Okay, let's sum it up: 1. Imedx hid information on losing more than one account for months, this is a fact. 2. Number one above is confirmed by Imedx having an acct going to EPIC, and a few days later 3 accounts that we "do not have access to anymore" as stated by manager. 3. I work for a company whose initials are TTS.  I was told that QA works on editing and transcribing too and QA has the ability to assign work to themselves.  In other words not only do they take work away from the MT but they can choose which work/dictators they want to do.  Isn't this a conflict of interest?   I know other companies do not allow double dipping but it was confirmed to me TTS does this and that the QA people have the ability to pick and cho ...

Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE?   All of the sudden, after no feedback for about a month, no audits, nada, zilch, I got my audit and was given critical feedback for not capitilizing a department! (recovery room, radiology suite, cardiac cath lab...)  AND, in a verbatim report, with a client profile that says specifically, DO NOT ADD A HEADING IF ONE IS NOT DICTATED, for NOT adding a heading!  This makes me nuts ...

I work in a surgery center/clinic in medical transcription. I have never worked in the medical field before, so I'm not sure what is acceptable. We get massive amounts of documentation errors from the nurses in the surgery center and in the clinic. Is this usually acceptable?! Is it normal for a transcriptionist to be required to scan over the chart and correct all of their errors or should this be the responsibility of the nursing staff? It's DRIVING me up a wall!
