QA and QC making up errors - Chavera

I have been working for Nuance for 5-1/2 years now. I, of course, consider myself a very conscientious and diligent MLS. Lately, I have noticed "mistakes" that I really DO NOT BELIEVE I made. Here is an example "800 calorie ADA diet". Supposedly I did not blank out the 800. I believe I would have, as 800 calorie diet would have set up an alarm in my head, for sure. I want to know if others are having the same problem. Also there is a spot where supposedly I wrote 400 2 pills daily (for example). I NEVER put two words together like that - I would have put 400 two pills daily, for the sake of clarity.

These are 2 examples. Please let me know if you have had similar things happen to you. Thank you.

some of the last corrections aren't saving before the report is uploaded. It would be laborious, if not impossible, to try to prove that, unfortunately. My first suggestion would be to uninstall and reinstall EditScript just to be on the safe side. (see link)

Yes, constantly getting "errors" I know I did not make. (Nuance) Employed there 1 1/2 years now.

I have no clue how, but I know stuff I changed was put back and I know I didnt do it, but there it was in the final report like that. I kept saying to the supervisor that I know I changed that, I remember this report. I am for certain something you are doing is changing it back when you upload. I could never prove it though.

Same thing happens to me now at MModal - QC keeps saying

This happened to me all the time and I knew I had not made the mistake and even told my supervisor, but she insisted I did and it was really dumb things that I knew I did not do. Sometimes it would be major errors also, which I had corrected.

It could be you, but more than likely it is the company you keep -- Nuance. Due to its lack in communication and ethics, this company can do nothing but screw up. Pathology, radiology, acute care all done differently when it comes to QA. If you are going to evaluate a group, would you not do it company wide? Does that logic not make sense?

I have been having this problem for years at MM and saying the same thing as others here - first I was blaming QA (not that I think they are above a little finagling with the reports), but I very slowly came to the conclusion that there were some mistakes that QA would have happily jumped on and they went thru anyway. Then I gradually began to see that it was the stupid machinery that was screwing up - and of course nobody would believe me. They were too busy with some version of 99%. Yup - I think this could be the downfall of this junk that we are supposed to "train." ---Waiting for the crash!!

I remember "proving" one to QA in some discussion, I'd written an e-mail about it or something. She didn't seem surprised even before I produced the sort of supporting evidence we almost never have.

Yes, I have had the same thing happen to me too. Tought it was just me. Thanks for sharing your experience. I did tell my supervisor that I feel it unfair that we get dinged and don't have a way to defend or contest the error.

I was beginning to think that it was the QC's had it out for me or something! And we can't save the reports when we're finished to compare, because by showing them that we saved one, that'd be our job!

Anyone else get random y's at the end of their report for hitting "y" and it entering on the report and not answering yes to the pop-up box to finish the report?

Do you seriously think we have the time to make up errors? Everybody makes errors, so do we. Have you never gone back and looked and thought "boy that was dumb"...I sure have. We are audited as well. The big thing is re-read your work, it's very very easy to have a typo in there. If you flagged 800-calorie diet and left a blank it would have been marked as a valid blank and I thank my MTs for catching those errors. They catch a lot, dosage problems, etc., and there are those who just don't care that there's a person on the other end of the report. They put in 20+ blanks and go to the next one. Just had one where they put testicular cancer in the BRAIN (some wreck huh?). Nobody's perfect, NOBODY, and we all make mistakes, and if you get dinged wrong, go to the TSM! We get dinged as well, just got dinged for not using an account I don't even have...lol You just keep on keeping on.

I get dinged for leaving out words like the, and others. I am now understanding that this is not the correct policy, that others are not getting critical error, major error, off one point for this. Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE? All of the sudden, after no feedback for about a month, no audits, nada, zilch, I got my audit and was given critical feedback for not capitilizing a department! (recovery room, radiology suite, cardiac cath lab...) AND, in a verbatim report, with a client profile that says specifically, DO NOT ADD A HEADING IF ONE IS NOT DICTATED, for NOT adding a heading! If we get a "syntax error" message on a post we have just made, how do we get back in to correct the error without losing the entire post? Every so often I forget, and put an apostrophe in the subject line, and end up losing the entire post. I've tried the "back" button, "Alt-Z" (undo), etc., but it's all gone once that syntax message appears. ...

I work in a surgery center/clinic in medical transcription. I have never worked in the medical field before, so I'm not sure what is acceptable. We get massive amounts of documentation errors from the nurses in the surgery center and in the clinic. Is this usually acceptable?! Is it normal for a transcriptionist to be required to scan over the chart and correct all of their errors or should this be the responsibility of the nursing staff? It's DRIVING me up a wall! PLEASE NO NEGATIVE FEEDBACK! This is truly meant to be helpful, not snotty, threatening, or negative so I expect the same in return. Has anyone else noticed the numerous errors on ASR?? Words left out, he/she incorrect, the strangest of phrases being picked up by the computer that are not even dictated and the phrases picked up do not even come close to what is really being said. Here are some of my observances of chronic ASR errors, though sometimes it gets it right. Most are not a biggie, but one could get marked down on an audit. 1. BiPaP. Should be BiPAP. 2. working as a coder, I read all the reports. It always amazes me what goes through. today I coded an MRI that had the indication "Left upper Germany tingling!" Maybe what you are seeing is already the result of many people like yourself who have already left the field for the same reason you did. When you get the responsible, diligent workers leaving because the pay has been cut so drastically they can't afford to work anymore, the field becomes saturated with new or substandard workers. Do any of you feel that errors you've corrected on the edit drafts disappear? Have you gotten marked off for errors that you are sure you corrected? Never in the last year have I ever ever gotten 4 major errors in one report, I don't think I have ever gotten 4 in 3 or 4 months. They will never be able to convince me that something hasn't changed. I rebooted my computer Monday after it had been off for a week. Got exception errors causing Smartype to dump and not allowing speech rec to start. Anyone have any idea what might be causing this or a fix? My solution i to use my clunker, so I'm not quite dead in the water.
