medical errors with EMR - anon

Has anyone heard of complaints with erroneous information in medical records due to EMR or point and click? My mother recently had to get medical clearance for joint surgery. Her physician went to some form of EMR. They questioned why she was taking a narcotic cough suppressant and insulin listed under her medications in the computer. She was shocked and stated she had never taken those meds. Another family member, who does not drink, got a copy of his most recent physical required yearly for his employer. Under his social history, it was stated he consumed alcohol daily. He had to get this immediately corrected, as it could have caused serious problems with his company, which he is close to retiring from. I was appalled. However, not surprised if these types of errors become more common in the future, perhaps with more serious consequences than "We'll have to fix that in the computer."

I was transcribing a report one time, and it's one of those dictators who likes to turn to talk to others in the room as she's dictating. One time, she turned to someone in the room and said she could not believe what the EMR had listed as a medication for this patient she was dictating on.

When my daughter had her last physical for dance team, the paperwork came out as her "testicles and penis" were normal and her "prostate was smooth." We were mortified and had to have it corrected before it was sent to the school. :/

As I left the office, I noticed on my copy of my visit they had me 30 pounds heavier and 3 inches shorter than I am. I don't know what the dr was looking at when she came to see me, but that should have been a concern, if I had ballooned up 30 pounds in 3 months (not to mention shrank 3 inches). Went home shaking my head; whole life is not making errors on people's medical records and I can't go to the dr once without them getting it wrong.

My doctor's office now sends out labs to Quest instead of running them in the clinic, so when I am sick I now wait 5 to 6 days to find out lab results versus the afternoon of the visit. Talk about compromised patient care!

their labs to Quest. It is extremely annoying when you have someone come in to the dr's office with flu symptoms only to find out that the flu screen that was ordered can't be done anywhere but Quest, which of course means no reason to order as by the time the patient actually started having symptoms is usually at least the day before. No Tamiflu if over 48 for symptoms.
I will say, though, that with my experience of working with a lab (hospital) that would have to do send outs to Quest and also working in a physician's office that for the most part we would receive Quest results by the next morning. The only time it caused problems was for flu and strep screens. The tests that took longer also took longer coming from the hospital too.

My PT MT boss went in and the doc rattled off her meds & she just said, whose medical record are you looking at anyway! and then proceeded to give him an ear full
Poor docs, they trust this shit & and it is crap. What about a poor patient in a coma or something who can't tell them that they are wrong?
Lawsuit city here we come. Never thought I would be happy about our litigious society but I think in this case it will be warranted, but it will probably be too little too late and patients will suffer

I think this is going to happen more & more frequently. I just jumped through all of the hoops trying to get a job as a "Medical Scribe." MTs now can do this. After 2 weeks of interviews & testing they hired someone else. I'm sure it was because on my last interview they advised me "I just want you to know that the starting salary for this position is 9.97 an hour." Yikes! I can't even remember making that little. The office was excited I interviewed, but I am sure they hired the person who would take a lower salary. I need a job so bad. It's VERY depressing!

CRITICAL ERRORS Jun 15, 2010Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE? ASR Chronic Errors List Sep 09, 2010Here are some of my observances of chronic ASR errors, though sometimes it gets it right. Most are not a biggie, but one could get marked down on an audit.
1. BiPaP. Should be BiPAP.
2. Levothyroxine. ASR capitalizes the "L" for some reason. This med is a generic for the brand Levothyroid. Should be levothyroxine.
