Transcription errors - dana

Posted: Mar 01, 2013

I have been working for Nuance since November. Not my choice, BTW. Does anyone look at their FIESA alerts when they are sent to your email? I am wondering how much experience some of those people have. I have seen errors that they say I did that, after my 31 years of experience doing transcription, I know I did not or would never do. I had one yesterday that said the error was "will follow with you". The correction they put was "will follow with you". Now correct me if I am wrong, but I think those two are exactly the same. and they posted it as a major error. Unfortunately, there is no way for me to check to see if what I put and what they say I put are the same, as it has already been corrected. I personally think they are trying to screw me over. I can't wait to get another job.

Exactly, you are being screwed over. I have had many major errors lately that I am certain were not mine. As you said, you have no way to prove it. And spending your time to look up the "major" error, copy it to send it to supe, is just not worth the time.

I did take the time to get about 4 reversed because I simply stated these are not 1 point errors, but it takes up to much unpaid time.

I have been transcribing for 20+ years as well. This is ridiculous QA/QC (whatever it is) and I said that in my email too.

WHO IS POLICING THE GRAMMAR POLICE IS MY ONLY QUESTION?

If your TL doesn't respond to dispute who do you go to? - anon

[ In Reply To ..]
I have a report that they gave me a major error on that I know absolutely is incorrect. I keep sending it with the Acct Specs to prove I am correct and that it should be reversed but my TL has not acknowledged it in the 2 months I have been requesting a reversal.

What is the next step? Is there another person I can send it too?

My TL responds to maybe 1 in 5 of my emails.

They do it all the time - NuisanceQA

[ In Reply To ..]
I get them like that all the time. What's even more interesting is how they minus off 1.0 points for that kind of error!

And by the way, what the heck is a "Noncomment" error, that we get back that has points subtracted, but NO corrections on it whatsoever? I've had as many as 5 points deducted with no comments, nothing. I asked my TM, but do you think I am worthy of a reply?

Not on your life.

QA Nightmare - Unbelievable

[ In Reply To ..]
See anything wrong with this "verbatim" SR sentence? According to Nuisance QA it's 100% correct. LOL.

"After a motor vehicle collision through the emergency room, this gentleman was admitted to the hospital."

Is that right? Were any of the hospital staff injured in that emergency room wreck? I don't think that is quite the picture the dictator meant to convey, but for hairsplitting sake, it IS VERBATIM, making it perfect. How apropos.

One thing is for sure, this is not FORD, quality is NOT job one here. LOL.

See thats the thing that drives me nuts and the reason - I never want to work for Nuance.

[ In Reply To ..]
Do they just pull those type of stunts or correct things that dont need to be corrected to make their positions seem more relevant? I mean really, why not just do away with all the layers of QA and downsize that dept a little bit. I think they have way too many people with too much time on their hands. Seriously.

Look on the error and see if it is client feedback. If it is, for some reason we are told they cannot tell us what the exact error is that the client found, but we should just disregard and it does not count against us. Makes really no sense, but that is what I have been told.

This whole thing is ridiculous, we know it, they - know it. Seriously, SM

[ In Reply To ..]
think about getting errors for not typing THE PATIENT but only typing PATIENT, for not putting an "a", stuff like that. When you think about VR and what it puts out, and you think about the fact that VR is going to replace it, it is just laughable. What would happen if VR just put out the word PATIENT and it was dictated THE PATIENT with no editing from an MT? Do you think it would present a problem with the client? Who would the client blame for that? Does the client really care about that? I don't think so.

I have been told, and I agree, that the reason for these idiotic errors is for one thing only and that is to prevent bonus pay. It makes perfect sense. It's quite insulting to all of us.

VR - new but old

[ In Reply To ..]
I have gotten this one dictator a few times in the last few days that I have never gotten before. Strictly VR. Starts off by the fact that it is miscoded in the top. She has put the attending in both slots as the speaker and the attending instead of herself so that has to be corrected, wrong patient was listed, so have to look up the correct patient and input that for her and THEN starts the wonderful dictation. I honestly have no clue how this even got typed, but I would say that at best probably only 1 out of 15 words is correct. It is like the fingers are on the wrong keys, nothing makes any sense at all, I just end up having to delete almost the entire dictation and just retype the whole thing. Completely ridiculous and a lot of wasted time on my part but by the time it goes through it is right!!!! but no thanks to VR
I hope you put in to your TSM to get credit for straight typing pay for it. - and any others like that.
[ In Reply To ..]
That will also probably help get things moving regarding notifying the dictator on how to input ID correctly, etc. Regardless, you deserve straight typing pay for it. I've had to do this on about 3 reports so far and never had a problem getting credit for the lines (they can see what the original SR draft looked like to confirm).
thanks for info - new but old
[ In Reply To ..]
Thanks for the info. I will do that. I thought about it but had not known if they would do anything. I have a great TL so I was just thinking of telling her so they can let them know.

verbatim accounts - Nuance MLS

[ In Reply To ..]

I have worked on a 'verbatim' account. I have been told by both QA and my TL that if a sentence obviously does not make sense, it is to be corrected...verbatim or not. This sentence about the emergency room that you cite is definitely something that I would have corrected because it Does NOT make sense. QA was wrong for not correcting it.

Lawsuit Regarding Errors In Indian Transcription?

Imedx Stuff Your Screaming TRANSCRIPTION ERRORS
Apr 02, 2015

Maybe added to the fact that we are now working on 3 or 4 accounts which are unfamiliar, you will recognize that just maybe the facilities are seeing more errors and becoming frustrated because the work is being sent to India where they are seriously less familiar with the account specifics than MTs who worked an account regularly and reliably up until it was taken away.  ...

1 Point Errors, Critical Errors.
Dec 08, 2012

I get dinged for leaving out words like the, and others.  I am now understanding that this is not the correct policy, that others are not getting critical error, major error, off one point for this.  So, am I being singled out? Because I thought this was the norm.  I'd like to know so that while I am hanging on every word and will continue to do so, I thought I was not the only one with this...thanks in advance for the update.  ...

CRITICAL ERRORS
Jun 15, 2010

Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE?   All of the sudden, after no feedback for about a month, no audits, nada, zilch, I got my audit and was given critical feedback for not capitilizing a department! (recovery room, radiology suite, cardiac cath lab...)  AND, in a verbatim report, with a client profile that says specifically, DO NOT ADD A HEADING IF ONE IS NOT DICTATED, for NOT adding a heading!  This makes me nuts ...

If we get a "syntax error" message on a post we have just made, how do we get back in to correct the error without losing the entire post?  Every so often I forget, and put an apostrophe in the subject line, and end up losing the entire post.  I've tried the "back" button, "Alt-Z" (undo), etc., but it's all gone once that syntax message appears. ...

Documentation Errors
Sep 15, 2010

I work in a surgery center/clinic in medical transcription. I have never worked in the medical field before, so I'm not sure what is acceptable. We get massive amounts of documentation errors from the nurses in the surgery center and in the clinic. Is this usually acceptable?! Is it normal for a transcriptionist to be required to scan over the chart and correct all of their errors or should this be the responsibility of the nursing staff? It's DRIVING me up a wall! I'd say 75 ...

What Is Your Job Like When You Get Overflow? What About Errors?
Mar 08, 2011

I am a relatively new MT with a pretty good track record at my current job. I have been working there for about 6 months. Well, we just got a new acct, and my company was understaffed. I have been working myself to death. I should be able to do more, but I am a new MT, and I do not type very fast (about 55-60 wpm, slower when I am tired). They have a certain amt of mins of dication you are supposed to complete each day, and they have been giving me more than that. I have been completing what I c ...

Common Errors
Apr 12, 2012

PLEASE NO NEGATIVE FEEDBACK!  This is truly meant to be helpful, not snotty, threatening, or negative so I expect the same in return. Military time:  Lately I have seen a lot of reports where people are inserting colons in military time.  Colons are NOT used for military time unless the hour, minute, AND seconds are given.  If the CP states that time is transcribed "3:00 a.m." this is for "regular" time and not military time. Spacing:  TWO spaces after periods that end ...

Medical Errors With EMR
Sep 23, 2012

Has anyone heard of complaints with erroneous information in medical records due to EMR or point and click? My mother recently had to get medical clearance for joint surgery. Her physician went to some form of EMR. They questioned why she was taking a narcotic cough suppressant and insulin listed under her medications in the computer. She was shocked and stated she had never taken those meds. Another family member, who does not drink, got a copy of his most recent physical required yearly f ...

QA And QC Making Up Errors
Mar 23, 2013

I have been working for Nuance for 5-1/2 years now.  I, of course, consider myself a very conscientious and diligent MLS.  Lately, I have noticed "mistakes" that I really DO NOT BELIEVE I made.  Here is an example "800 calorie ADA diet".  Supposedly I did not blank out the 800.  I believe I would have, as 800 calorie diet would have set up an alarm in my head, for sure.  I want to know if others are having the same problem.  Also there is a spot where supposedl ...

ASR Errors/TOO NUMEROUS TO COUNT

ASR Chronic Errors List
Sep 09, 2010

Here are some of my observances of chronic ASR errors, though sometimes it gets it right.  Most are not a biggie, but one could get marked down on an audit. 1.  BiPaP.  Should be BiPAP.  2.  Levothyroxine.  ASR capitalizes the "L" for some reason.  This med is a generic for the brand Levothyroid.  Should be levothyroxine. 3.  FIO2.  Should be FiO2. 4.  Hospice.  In the English dictionary, this is simply hospice. If a specific organi ...

Speech And Editing Errors
Dec 19, 2010

working as a coder, I read all the reports.  It always amazes me what goes through.  today I coded an MRI that had the indication "Left upper Germany tingling!"  It should have been left upper extremity.  This is the stuff that doesn't get caught when editing, and it goes into the patient chart.  Once some doc, who didn't bother to look at it, signs it, it can't be changed easliy.  In fact, it is a major issue to try to change something once the ...

Errors In My Medical Report
