ASR errors/TOO NUMEROUS TO COUNT - BB

Posted: Jul 01, 2010

Has anyone else noticed the numerous errors on ASR??  Words left out, he/she incorrect, the strangest of phrases being picked up by the computer that are not even dictated and the phrases picked up do not even come close to what is really being said.  It just seems to me as this ASR is getting worse by the day.  You would think after spending all the money on the purchase or Spheris, that some amount of money would be spent in cleaning up ASR.  The CCM/and the Q worry about customer satisfaction and turnaround time as they get penalized if they are late returning report.  We should just send in these reports as translated by ASR and let everyone know just what, we the transcriptionist, has to go through to clean up the report in order for it to be put into a chart.  I just cannot wait until I quit.  I have just had it!!!

You are absolutely right - OHMT

ASR is a joke. It's just a sorry arse excuse to pay us less money. It really is a waste of time and money. It's not worth whatever money they spent developing it.

As if correcting the ASR actually expedites the report to the client. LOL. Sometimes I just highlight the stupid thing and type if for the lowsy ASR rate. At least the client will have their report sooner. LOL. Makes total sense. I can type it in 10 or 15 minutes, or correct it in 1/2 hour-45 minutes. Of course, then there are the ones that are so bad, that it can even take 1-1/2 hours.

They should AT LEAST take the long reports out of there. Anything over 6 mins on ASR is a disaster.

I've been on ASR since they started it. - CA-MT

I've been strictly following the rules that we have to to help ASR learn from the very beginning and on the same account (more or less), thinking that this helps the system learn. It actually showed improvement for about the first year. Then it kind of plateaued, but NOW it is much, much worse, and not only with the new doctors on the system. Some of the doctors it butchers aren't even ESL, but when it is given an Indian MDs, I might as well f11 it right from the start. It's really getting so bad that I'm thinking the dirty Q probably laid off the technicians who were working on it in the beginning, thinking ASR can run itself. When you come to think of it, don't you think MQ is trying to replace us ALL eventually with machines?

Hope this string is not moved. - Adios

Last time comments were made about ASR it was moved to VR page. This is all about MQ, not VR. I have no idea what value there is to the client to have ASR. It does not help them one little bit except for cost. That is where we come in. We are the scape goats. We take less pay for the fact it is ASR, when in fact, and I challenge management to prove me wrong, it is not faster to edit on ASR than to straight transcribe.

It is nothing more than a marketing tool until it is fixed. It is BROKEN. It does not work. It does not make the dictator's life any easier. They get the same report in the end. It CERTAINLY does not make our life easier. Whose life does it make easier? Then client. It is marketing only. Nothing else. It serves no other purpose than to give MQ an excuse to pay us less and pass the savings on to the client. SHAME ON THE CLIENT if they have any knowledge of this.

But we are still turning it into a beautiful report - anonymous

for 40% LESS MONEY. Why wouldn't the Q love it? We are the only ones for whom anything has changed. Reports still go to hospital. Q still gets paid. We get paid less. End of story. 40% less money. Reports still typed and on the chart.

We have class. We care about the quality of our work. Even if we get hit hard, like 40% hard, we believe in what we do. We will not compromise. Therefore, we are taken great advantage of. We have class and ethics. We are a dying breed.

But let's here it for us! We will not compromise! Despite the mistreatment we endure!

I honestly have seen a decrease in ASR - performance in the last

[ In Reply To ..]
few months. It has gone from bad - to ridiculous. Like you said, words missing, jibberish, added words that no matter how you listen just ARE NOT there -- it has added them. Really bad. Putting in words that are not even remotely the same thing - maybe starts with the same letter is all?

I have seen it too. - Adios

[ In Reply To ..]
I still believe it's because nobody is at the wheel.

Watch your next several reports. The dictator will repeat the same word, speaking perfect English, and ASR will pop out 3 or 4 different words.

Medicines especially are very scary. ASR puts in a real medication, but totally the wrong one. Not good. Thank God MQ has those of us who care.

Agree. ASR is apt to put cyclobenzaprine for - cyclophosphamide.

Seems like it just gets close or "guesses." That is scary. That would imply it can THINK.
I'll go you one even better - CyberlandMT

Today I had one where the medication input was so close to the actual one it should have been, it really was scary. But you know what? That med the ASR put in is a commonly used drug in a different country (I researched it)...you know where without me having to say it. Hmmmm! This is not the first time I've run across something like this either. Funny monkey business going on for sure!

Cornerstone - MarineMom

[ In Reply To ..]
I have noticed that Cornerstone is going downhill, too. It will completely leave out the headings for Allergies and/or Medications that it used to put in, and still does put in for other reports. Most of my docs do not have accents, but VR is getting worse. There are some reports that are okay, and then there are other ones that are simply mind boggling.

I can always tell when there are newbie hands in my account because this sort of thing starts to happen. No matter how much training they seem to get, there are those MTs who simply must put their "personal" touch onto a report in spite of client profile, etc.

Probably some newbies are on your account(s) and that's what causing the sudden apparent disintegration of the integrity of the system. Try reporting the incidents and see if they can't be corrected on the backend.

Good luck...It totally annoys me when I can tell there are a few new MTs on board doing their own thing in spite of instructions to the contrary.

Noticed it recently and today was the worst. Reports that had to be - completely retyped.

Wondering If Numerous MTs Have Left
Sep 18, 2013

who were originally on my accounts.  The work used to be feast or famine quite often. Now all the accounts seem to be swamped on a daily basis, constantly approved for OT for FT people (though no incentive for PT or PT progressive people), lots of cheerleading about making a big team effort every day, starting earlier and working later. I don't know how they aren't losing the accounts with them being out of TAT daily.  It never used to be this way and I am wondering if that m ...

1 Point Errors, Critical Errors.
Dec 08, 2012

I get dinged for leaving out words like the, and others.  I am now understanding that this is not the correct policy, that others are not getting critical error, major error, off one point for this.  So, am I being singled out? Because I thought this was the norm.  I'd like to know so that while I am hanging on every word and will continue to do so, I thought I was not the only one with this...thanks in advance for the update.  ...

Character Count/line Count
Feb 23, 2011

Is this correct? I type 70,000 characters a day. I am paid by the 65 character line so I would divide 70,000 by 65 to get 1076, then multiple that by pay per line, say $.10 and get $107. If this is not correct then what is the formula? And what is the average number of lines per day? Thanks so much for the info. ...

CRITICAL ERRORS
Jun 15, 2010

Has anyone at the Q been getting hit by "critical errors" on crazy stuff that has never been an issue BEFORE?   All of the sudden, after no feedback for about a month, no audits, nada, zilch, I got my audit and was given critical feedback for not capitilizing a department! (recovery room, radiology suite, cardiac cath lab...)  AND, in a verbatim report, with a client profile that says specifically, DO NOT ADD A HEADING IF ONE IS NOT DICTATED, for NOT adding a heading!  This makes me nuts ...

Documentation Errors

I work in a surgery center/clinic in medical transcription. I have never worked in the medical field before, so I'm not sure what is acceptable. We get massive amounts of documentation errors from the nurses in the surgery center and in the clinic. Is this usually acceptable?! Is it normal for a transcriptionist to be required to scan over the chart and correct all of their errors or should this be the responsibility of the nursing staff? It's DRIVING me up a wall! PLEASE NO NEGATIVE FEEDBACK!  This is truly meant to be helpful, not snotty, threatening, or negative so I expect the same in return. Military time:  Lately I have seen a lot of reports where people are inserting colons in military time.  Colons are NOT used for military time unless the hour, minute, AND seconds are given.  If the CP states that time is transcribed "3:00 a.m." this is for "regular" time and not military time. Spacing:  TWO spaces after periods that end ...

Medical Errors With EMR
Sep 23, 2012

Has anyone heard of complaints with erroneous information in medical records due to EMR or point and click? My mother recently had to get medical clearance for joint surgery. Her physician went to some form of EMR. They questioned why she was taking a narcotic cough suppressant and insulin listed under her medications in the computer. She was shocked and stated she had never taken those meds. Another family member, who does not drink, got a copy of his most recent physical required yearly f ...

Transcription Errors

ASR Chronic Errors List

Here are some of my observances of chronic ASR errors, though sometimes it gets it right.  Most are not a biggie, but one could get marked down on an audit. 1.  BiPaP.  Should be BiPAP.  2.  Levothyroxine.  ASR capitalizes the "L" for some reason.  This med is a generic for the brand Levothyroid.  Should be levothyroxine. 3.  FIO2.  Should be FiO2. 4.  Hospice.  In the English dictionary, this is simply hospice. Speech And Editing Errors
Dec 19, 2010

working as a coder, I read all the reports.  It always amazes me what goes through.  today I coded an MRI that had the indication "Left upper Germany tingling!"  It should have been left upper extremity.  This is the stuff that doesn't get caught when editing, and it goes into the patient chart.  Once some doc, who didn't bother to look at it, signs it, it can't be changed easliy.  In fact, it is a major issue to try to change something once the ...
