Bogus errors? - Frustrated

Has anybody ever had the experience on the EditScript or other platforms with Nuance where you got feedback from Fiesa saying you made errors you didn't make-text either inserted or left out? I have been charged with CPSE and major errors with random words stuck in places where I know I would've seen and corrected it but when they check the draft it's showing that I missed it. Maybe I'm going crazy but some of those errors I'm almost positive I didn't make. Did this happen to anyone else or am I just going crazy?

Editscript Malicious Elf. Sometimes, even before I upload the report, some of my edits get reversed when switching back and forth from typing into the header to typing into the body of the report(Alt+E to Alt-Shift-E or vice-versa). I've always wondered if this accounts for those mystery FIESA errors you're talking about; you know, the ones where you know very well you edited out the extraneous text, but lo and behold it re-appears in FIESA and you get dinged for it. This seems to be a relatively rare occurrence, but as we all know, it doesn't take many "omitted or added dictation" errors to thrash your score. This quirk has made me hyper-vigilant (double and triple-checking) and really slows me down; even so, now and again I'll still get one of these FIESA errors and think I've been had by the Elf. Grrrr!
At Nuance, I would save reports I sent to QA - Depressed

and then would get dinged a major error because another QA had filled in the blank wrong. I would get up to 10 of these a week. I kept asking for reversals, and then the guy in India (over QA I guess) locked me out of FIESA.

Never had the issue before using the newest version, Beta, that's come out... and now that Nuance owns Emdat, well, there ya go... Since dl'ing the Inscribe beta version, I've noticed it putting in random words, adding back words I've deleted, changing my autocorrects to read incorrectly and/or inserting words before and/or after the autocorrect text. I can only hope I've proofed well enough to catch that mess, but it's more than likely I've missed at least some of it.

I definitely feel I am going crazy with these errors and/or getting too old for this. I used to fight every error but as I am getting ready to retire, I just put my initials on the dotted line and shrug them off because I am just plain out of energy.
It does seem though you look at some of those supposed words that appear from nowhere in the text and wonder "could I have really added that." It is somewhat reassuring that other people have noticed this,so there must be some kind of explanation, because I am a perfectionist and really hate making erors. I am so slow now and double and triple check, especially numbers, and it does not matter how many times I check or how many times I read over the report, these stupid errors seem to appear in Fiesa. Sometimes I think I have corrected something in VR and then I go back and some word has suddenly appeared or something has been deleted. Yes, I have had the exact same experience you have.

ASR Chronic Errors List Sep 09, 2010Here are some of my observances of chronic ASR errors, though sometimes it gets it right. Most are not a biggie, but one could get marked down on an audit.
1. BiPaP. Should be BiPAP.
