Have you ever used the break function simply because you had a dictator that was so horrible that you got frustrated and sent it back? I know that we are not supposed to do this and that MQ supposedly keeps track of how many breaks we use but I have to admit I have done this no occasion. I had a report so bad a couple of weeks ago which was over 1000 seconds long and an ESL doctor, not to mention scratchy sound, etc. About halfway through this thing I had already spent nearly 20 minutes on it and I finally gave up.

Sometimes I just cannot mentally/emotionally/physically handle another crappy dictator and I use the break...but it seems to come right back to me! Just can't win! Good luck!

The same thing happened to me a while back while using the break function. Same situation as you, ESL who speaks very softly plus scratchy dictation sounds in background. But, it came back and bit me in the butt! I got the report back about an hour later. LOL. I try not to use it too often but on that particular day, I had just had enough. LOL.

I have complained repeatedly about the poor sound quality to my CCM but nothing ever seems to get done about it. They tell you to report these things but nothing gets done about it. They could care less.

and my horrible sound quality. They said some of us have bad sound quality because of some filters the dictation doesn't go through (don't know if it's my internet connection, my computer or on MQ's end) and there's nothing they (or I) can do about it. So if they can't do anything about it, then I can't either so I send many reports through with lots of blanks. I'm starting to not be so upset about sending in reports with lots of blanks. Hope I don't get too care free!

I report it, I just never hear anything back further about it from my CCM, who is the one who told me to report it in the first place. I have never been told anything concernnig the filtering. They did try to tell me once a long time ago that it might be the earphones I was using. Nope, wrong answer, it was not the earphones. I have cable high speed internet. I think it gas more to do with either the hospital's dictation system or the way it is manipulated through Medquist.

I've never used the break function in the years I've been with MQ. If they don't want us to use it then I don't know why it's even there. However,when I get crappy ESL dictators and poor sound quality, I merely leave LOTS of QA markers. I think I've even left just one QA marker for the entire report. Sorry, don't have time to spend 45 minutes on one report because the dictator totally sucks. The account I just lost to India I rarely left a QA marker, but now that I'm bouncing all over the place with nothing but crappy ESLs and never getting the same dictator twice, I have no qualms about filling up an entire report with lots and lots of QA markers.

I certainly can understand your frustration but all the years I've done MT, we were never allowed to give up a dictation just because the doc was hard to understand and/or poor quality. That's why we have QA and markers here. I've gotten reports that had a paragraph typed with several blanks that someone gave up on and I had very little problem with but I might have had a better ear for that doctor. Someone I might find impossible to understand, another MT might whiz thru it. Do you mean breaking a report or abandoning it?

At least when I worked at hospitals, we were allowed to cherry pick at the VERY end of our shift. Here, if you're finishing a report with 10-15 minutes left to go and a 2000 second report is coming up, what are you going to do? I'm going to go very very slowly on my current document. At the hospital, we could finish the report and keep rejecting reports until we got one that took us the right amount of time. The other benefit of hospitals was other MTs who could listen to your dictation and help out.

I don't know what to tell you. I suppose if you're not abusing it, it probably won't be an issue.

Breaking vs Abandoning - MT

Breaking is for a report type you don't know how to do. Breaking really should only be done if MQ knows you can't do Op Notes, and an Op Note comes that was keyed in as, say, a Progress Note. You change it to Op Note and it goes to an MT who can do Op Notes.

Abandoning is you hit abandon and it goes back into the general pool. MQ has that set up so that it gets slammed back to you unless you're logged out for a long period of time after you abandonded it. The rules say that you must log back in immediately if you have to abandon something. They are watching this, but there is a grace level, depending on who your CCM is. If you have a not-nice CCM--the type that is on a power trip and gets off on writing you up--he or she will write you up the gosh darn very first time you abandon. In my 15 years here, I've only had only one not-nice CCM/manager-type, so that's not bad.

I have never heard that it was grounds for termination. I have been told that it is watched carefully in the event that too many are used. I have only used it a couple of times myself and have never had anything said to me about it.

I noticed today that the break pay section of the timesheet was gone today and when I clocked off a minute ago after working for 7 hours that in the "break" box, it said 0.00, when usually it would be 0.33. For *pain* scale (1-10), i was taught to add the range if not dictated. Example: D: Pain was a 5. T: Pain was a 5/10. I'm not familiar with *function* scale. This is a spine surgeon. This particular patient is having neck and shoulder issues (if that matters). Is there a specific *function* scale? I must hit alt L 20 times a day instead of alt K. Seems like I do it when I'm in front of a word like "in" or "the" - you know, something that is in a document 1000 times.... I usually do alt z to undo it, but once that is done, the red box no longer works until it gets to the last occurrence of the word alt z corrected. How do I disable the alt L function? I don't mind having an unpaid 30-minute break, but the thing that bothers me about some of these companies is the fact that it is tiring sitting at a computer for 8 hours and getting only a 30 minute break. If I want two more 15 minute breaks those can be worked in, but then I have to add back 30 more minutes to my time! I like to at least stretch ever hour or so. :/ ...
