Quality metrics - Acquitions
Posted: Jul 03, 2014
Received from our TSM this morning...
"I have been advised that the minimum quality score for a passing audit will remain at 99.6% through this third quarter of 2014, which is through September 30th. "
Guess they figured they would lose too many people for not meeting the 99.7%...
Anyone else hear this?
"they would lose too many people for not meeting the 99.7%" - what a surprise... sm

Somebody finally pulled their head out of their A and figured they're pushing people too hard and 97.7 isn't realistic???
99.6 is just as bad as 99.7 - dnr

The same number of minor errors (2) will fail an audit at 99.6.
It seems like some groups have been 99.6 and some 99.7. The group I am in has been at 99.7 for a very long time.

Maybe the profession has a better chance for survival if we are very accurate. Maybe that is MM's motivation for raising the rate.
99.6, 99.7... both idiotic. Industry standard is 98. - nm

98%? - TiggerQA

If you were a PC/QC or auditor, you would see that 98% allows for a LOT of errors, which means we are sending very sloppy reports to our customers. That being said, I think 99 is a more reasonable requirement. It allows for 1 noncritical or 4 minors, which is enough if you pay attention to what you are transcribing.

Of course the post comes from a QC. Does my nickname not tell you what job I do/did? I WAS a QC for many years, and to tell you the truth, I scored a lot harder in that role than I am required to as an auditor. Keep in mind, I started out as an MT, just like most everyone in a QA/QC role, and have moved my way up in the company, and not always by my choice. That being said, if you were to audit reports all day long like I do, you would be amazed at some of the garbage and sloppy editing (and I mean that even about the GOOD dictators, no horrific accents, noises in the background, etc). It is painfully obvious when someone has not listened to the report in its entirety. All you have to do is look at the ASR version of the report, compare that to the report that the MT uploaded, I would say at least 8 times out of 10, the errors were due to someone not editing ASR. Keep in mind, auditors are also on production and there are very stringent rules. When I was a QC, I was a huge advocate for the MT. I felt that mentoring helped a lot of people make less mistakes. Trust me, I am not out to "ding" anyone. Most of the little things, like hitting a tab instead of using 2 space after a period, when not edited properly, make a report look very sloppy, and people are darned lucky not to be scored on those things. No, ASR isn't perfect, but it's our job to fix the errors. I don't like that ASR doesn't hear things right, either. However, when I see whole words or groups of words that make absolutely no sense at all, it's painfully obvious where the errors come from. Take a look at the Error Definitions and Weights document. If you are being scored for punctuation, incorrect spacing or incorrect capitalization, etc., (the small stuff that does not affect the meaning of the sentence), then the auditing supervisor needs to know that you are having points assigned for those things, and you can get those error points back, and they will then go back to the auditor and the AUDITOR gets the points for scoring wrong. If that happens to you, report it to your QC, and your QC will submit a form to the auditing supervisors, who will review the point assignment and restore points if necessary. My former MTs all know that I am all about advocating for the MT, but again, take some pride in what you are uploading. Keep in mind that your transcription is not just about your paycheck, but someone's life. Some of the things I see, if I were an English teacher, would adversely affect your grade. Transcribing medical reports is much more important than a high school English essay, and as much as I disagree with some of MModal's policies, such as the 99.7%, we should still strive to send clean reports, not just spell the medical words correctly. Again, with the way things are scored, a 98 allows for a TON of mistakes, and is unreasonable for a customer to be "ok" with a report with that many mistakes in it. I think 99% is probably a much better requirement. JMHO.
Also, before anyone chimes in about being in a salaried position, trust me... many MTs make more than I do just because they are fast (and still good).
Not editing the entire report explains how some people meet or exceed the minimum lph. - struggling

I am one of those people who have been flunked (remember, it only takes 2 minor errors) for the piddly minor errors. Turns out, extra spaces is on the list of minor errors. I spent 6 weeks on performance management for piddly errors that did not affect patient care, readability or document integrity.
What I don't get is how these people survive audits based on what I see in reports I look at for reference.
Spacing - TiggerQA

You should not be "dinged" for extra spacing UNLESS you put a hard return in or the CP says one space after a period, etc. If you get point deductions for spacing, I would ask your PC to challenge it.
If they cared about the reports they would do - away with ASR

Proven fact, our brains are not wired to pick up every minute detail. Now if you paid me MORE money, because ASR is harder, then I would be able to spend more time proofing and probably finding all the errors..But NO you cut my pay in half make my job harder, and expect perfection..Oh Im sorry, I added an S on that one and missed the ed that ASR added for God knows what reason..I had an old boss whose motto was do your best with the dictators Garbage in Garbage out...and we certainly have Garbage in..

percentage, unlike what MM is doing, just straight point deductions. That allows for a punctuation mistake or something minor, etc. It is the industry standard, period. This 99.7 crap is just a gimmick.

Quality Metrics Since The PODs Feb 11, 2015Is it just my accounts, or has anybody else noticed that their BOB/POD quality metrics have taken a nosedive since they started these PODs? Previously, my BOB would be in the green constantly and we would get praise for it. Now, all I ever see is red..too many criticals, too many major and minor errors. Seems like this whole POD idea is working marvelously. I hope they fail to meet contract and clients start dropping like flies. Of course, that would be our fault. & ...

More Metrics. My God. I Wonder How Many Apr 08, 2015upper level people spend the majority of their time on metrics, raking in the big bucks, while they slash those of us actually earning the company money. What's the point of all the statistics, metrics, charts, tables....... Performance Metrics Coming For Doctors Nov 16, 2012Interesting reading at family practice office this week, publication for Family Physician Practice, saying how software developers are working feverishly on new products for physician offices where they will choose their diagnoses and what-have-you from drop-down menu (yes, that will completely eliminate transcription).
Problems arising already with "trials", where docs weren't making "right" choices with their documentation. (They were not taking enough time with modifiers, etc.)

New Quality Index Vs Old Quality Score Oct 18, 2014I ran an error report for all reports, all accounts on my platform for this quarter, 10/1 - 10/18. From Below, About 99% Quality May 22, 2011slowly sinking here because my quality has always been 98-100 quality, years and years. Now, however, I am expected to maintain 99% quality, on a hideously difficult account, but I must also maintain my LPH.
In other words, I have to listen to reports on the fastest speed I can to get my LPH yet maintain some form of quality. The reports go to QA who in turn listen at the slowest possible speed to give errors for not including 'the' and mind you this cannot even be heard unless you ...
DQS 7 Quality Jun 13, 2012What is everyone's opinion about the new speech engine? I'm not talking about the user side....I mean the quality of it's speech recognition. I thought it was supposed to be so much better, but I'm finding it to be terrible. There are times when it leaves out 3 and 4 words and puts in words that are not even close (i.e., family history when the doc clearly says social history)! That, and the sound quality SUCKS!
Here's another example:
