sorry dictation. They might straighten their act up and learn how to dictate properly. It makes me want to go their office and give them my history while talking with my hand over my mouth or while I'm eating or smacking gum or whispering or talking so softly that I can't be heard or out in the middle of the waiting room where all the background noise is, etc. What's just as bad, though, is that the MTSOs don't care about the transcriptionist at all. All they care about is not making the hospital/doctor angry. Just throw the transcriptionist to the wind and let her/him fend for themselves. Sorry, just needed to vent a little.

I have always felt that any dictator, doctor, PA, nurse practitioner, etc. should be held to the same high standards as the MT when doing a medical report of any kind. Wishful thinking, but I would like to be able to transcribe EXACTLY what they say, made-up words and all. I would like to switch him/her and he/she as often as they do. I would like to transcribe the slaughtered names of drugs they can not pronounce. I would like to send all reports with a blank where they did not dictate the physician's first name. I did have one report that I sent to QA with the statement, "I will not even attempt to transcribe this," and in the end it had to be redictated. I do not believe many of them really know how bad they are. Could they? Why is the MT always blamed for their carelessness?

Considering how bad it can get sometimes, it is beyond me how some companies can "ding" the MT for QA submissions. There should definitely be exceptions to that rule.

It's really hard to contain your frustration sometimes at how bad and sloppy some of them are. I just finished one this morning, a female resident who was unbelievably ridiculous. Took her about 10 minutes to dictate a record that should have only taken her two. She was flirting with someone in the background, hadn't read the file before she started dictating so was flipping back and forth reading, stating put this in, then take it out, and she was talking as if she was a little girl.

One thing that has particularly irritated me is that recently I found out that the dictators are being told that they don't have to change their dictating habits at all, that we'll handle everything on our end.

You are so right when you say that they throw us under the bus and we get blamed for many, many issues that should be rightly attributed to the dictator, and it's been like this for as long as I can remember.

I recently saw a NP for endocrinology and she ordered a bone scan -fine, except I wanted a DEXA scan, not a bone scan. Two very different things. I had to argue with her for 30 minutes, go to radiology and verify that I was correct, and then get her to rewrite the order for a DEXA scan, but she still insisted it was a WHOLE BODY SCAN!!! I could not get through to her, and lost all confidence in her treatment of me. If she didn't know the difference between a DEXA scan and a bone scan (with injection of a radionuclide), then I didn't want any part of her. I did tell the MD about this, but of course, it was just poo-pooed. I was furious. What if this was my mother or anyone not knowing the difference? Thankfully, they would have to have prior insurance approval for the bone scan, but STILL???
COME ON PEOPLE...Get on the right page.

I'm amazed by what I hear on this account every single day. We have every right to vent about such bad dictation, and have to do the best we can with it.

I'd never go to the hospital on my account. Slurring, mumbling, speed talking and there are medication/allergy errors or discrepancies, and just send it thru.

Nothing's going to change until one of them gets hit with a huge lawsuit over an error and half bankrupts the hospital system they work for. That may not even help.

With the dictation quality, I'd never trust the quality of care.

I am working on an account where I have had literally no training, on a program where I have had literally no training so I have no idea if it is verbatim or not. The doctor dictates "He was seen last by me in December of this year." Okay, it was actually December of last year or 2011, I would assume. Would you transcribe exactly what he says or would you change it to December 2011 or December of last year? Thanks so much!!! ...

So I've been working for these 2 companies for about the past 2 weeks and I'm just wondering if anyone else who works for either one of them can give me any tips. I really like the work and the flexibility, but I am sooo S-L-O-W. I just did a file for Fox and it took me 3 hours to do 25 minutes of the file (very clear audio). I've never had this before and I'm not sure how on earth to transcribe it. He dictates the pronunciation for someone's name (specifially, the letters as in the first example) and specifically states he wants it included in the report (so he knows "not to mispronounce it next time" ). Should I maybe put: Cecil (pronounced S-E-S-I-L) and I had a long discussion... Cecil (pronounced "Sesil") and I had a long discussion... Thank for your help!! ...
