AVN staging...how do I transcribe this? - Megs

Help would be greatly appreciated because I can't locate it anywhere and have not in the last 3 years of doing Ortho had a doctor dictate this way...which I have transcribed verbatim. 

" Mr. X is a 33-year-old male, with a history of Chrohn’s disease, who has a history of increasing left hip pain. X-rays, MRI, and plain films are consistent with a collapsed femoral head (stage IV B2). "

The FICAT classification is used for AVN of the hip. and uses a combination on plain film, MRI and clinical features
â€Â¢ stage 0
o X-ray : normal
o MRI : normal
o clinical symptoms : nil
â€Â¢ stage I
o X-ray : normal or minor osteopenia
o MRI : edema
o Bone scan: increased uptake
o clinical symptoms: pain typically in the groin
â€Â¢ stage II
o X-ray : mixed osteopenia &/or sclerosis
o MRI : geographic defect
o Bone scan : increased uptake
o clinical symptoms: pain and stiffness
â€Â¢ stage III
o X-ray : crescent sign & eventual cortical collapse
o MRI : same as X-ray
o clinical symptoms : pain and stiffness+/- radiation to knee and limp
â€Â¢ stage IV
o X-ray : end stage with evidence of secondary degenerative change
o MRI : same as Xray
o clinical symptoms : pain and limp

The B2 is the type of fracture, stage IV B2 (stage and type)

This is exactly what the doctor says.  I'm unsure how to write it out.  Any help would be appreciated.  This patient is known to have metastatic colon cancer.  He was first diagnosed in 2006 and was originally PT4 PN2 NXG2 R1 and received chemotherapy and radiation therapy but had recurrent disease.  PT4 PN2 NXG2 R1 is what I need help with.    ...

I am working on an account where I have had literally no training, on a program where I have had literally no training so I have no idea if it is verbatim or not. The doctor dictates "He was seen last by me in December of this year." Okay, it was actually December of last year or 2011, I would assume.
