One can feel so beaten up and degraded. So much to know and for so little pay. Doc's get things to go faster and their pay is not cut. Difficult dictators don't have to take a pay cut, yet we get a cut in production and a cut in QA if we don't understand them. Are the docs even aware of Account Specifics? If transcription departments are not in-house any longer, who is making the picky rules? If you cannot put qhs, then why doesn't the doc say it that way? No abbreviations in the dx., then why isn't it dictated by the doctor that way? They do have to take a course on dictation in med school and understand do get a grade. Oh, that's right, he is employed at many facilities and he cannot and should not have to remember the different do's and don'ts...he/she might get mixed up!!

Docs have taken a huge cut in pay over the years because of insurance. They have to see 3-4 as many patients to make what they did 10 years ago.

That being said, the docs don't even know who does their transcription. I've asked 2 specialists I see who does their transcriptions, and neither of them knew.

It's an administrative thing in the hospital, not a doc thing. I doubt the docs (for the most part) give a hoot about qhs or q.h.s.

It seems that no one can teach those docs and others who do the dictations. The HIM departments cannot teach them properly, or are afraid to confront them. The MTSOs rarely ask the HIM departments to train or inform the dictators of their errors, for fear of stepping on toes and losing the account (?)

In the good old days, the docs were trained better and received feedback, some were even required to redo a bad dictation. But, those were the good old days.

Now, instead of training the dictators better or requiring them to do a better job, QA departments were developed, along with creative ways to penalize MTs who cannot decipher the worst reports.

Back then, it seems, docs were considered to be normal human beings, capable of making mistakes and capable of improving. They were expected to have basic manners, including not eating while dictating. I guess that's not the case anymore.

Even if MT is not in house, the HIM director usually sets the acct specs. - no message

I used Schedule Source for about a year when they were testing it. It's one of those "benefits" they keep talking about. The problem is, if you swap you will be in somebody else's cesspool - getting unfamiliar accounts for which you have to read maybe multiple CPs and get reports where you may have to leave QA markers and spend extra time researching. The truth is KS is being sued for not paying overtime. KS does not have time records and that is why it is being implemented now. I myself know overtime pay was not offered for anyone taking extra hours on my account. I also have e-mails to that effect.
