I just received my W-2 (not 1099 as we were statutory employees) on Monday. You might give it a few more days before panicking. That being said, the employer's address on the W-2 was given as:
Proscript Documentation Services, LLC
7485 Shipley Avenue
Harmans, MD 21077
This is markedly different from the former address in Florida.
