Of course, we have no idea about the quality of the recording the MT was listening to. This, of course, could have played a factor. Medication dosages should have been flagged for follow-up if they did not make sense, but the physician has the ultimate responsibility to sign off on the records. Of course, the nurse did not follow protocol by referring to a discharge summary to administer medications. The nurse did not question the dosage either, could have followed up on it prior to administering what ended up being a fatal dose. So many things went wrong here. A real train wreck of a situation.
