Piddly Issues - Anon

You're certainly getting some shade thrown your way, and I just wanted to let you know I have experienced the same frustrations with a few past behavioral health accounts. An 11-year-old boy is admitted because he "tried" to hang himself with a shoelace after his mom took his iPod away. Hard to be sympathetic or compassionate in this case, right? Until you realize that this kind of manipulative behavior can easily morph into something way more angry, destructive, and self-destructive as well. There was no apparent reason why an 8-year-old girl woke up at 2 a.m., took a knife from the kitchen, stood by the bed where her mother was sleeping, and proceeded to stab her mother in the face many times until the mother's screams woke the rest of the family, thus de-escalating the attack. I asked a psychiatrist once about the boy using a shoelace to attempt to hang himself, and the doctor had an interesting response. He said he knew it sounded like frivolous, attention-seeking behavior, but it had to be followed up for liability reasons. If the child ever escalated and actually completed a suicide, the doctor could be sued for choosing to ignore the initial self-destructive behavior. He then said the approved standard of care was to admit, administer loading doses of antipsychotic meds as needed, then discharge as soon as possible. When I asked why such a short stay, why no outpatient therapy followup,he said, "Because there are so many. There are just so many." Still, I know that psych work is stressful, and the suicide rate is high among psychiatrists. Probably due to overdose of teen queen drama, and young men with shoelaces.
