there was one that was absolutely horrible. It stuck out like a sore thumb from the other clear dictations I had before. I took my time with it and ended up leaving only 2 or 3 blanks, but I was told I was wrong by SN anyway because I didn't leave a blank where she thought that I should have. I was able to pull the list of diagnoses from one of the patient's previous reports and she never told me what I typed was actually wrong, but I've never been dinged by QA for actually researching and NOT leaving a blank!
