I would think it is 30 days from the date of the last premium taken out of your check. I would check my pay statements and see when the last one was held out. If you haven't had much work they might not have withheld the premium since the beginning of the cyber attack, so it's possible you've been without insurance for at least a month. Just my thought and I could be wrong.
