It can happen to any of us. I tend to be rushed towards the end of my shift if I get a long report and trying to finish it before time to clock out, and after proofing find errors that spell check will not pick up. I don't have time to stop and send other people's work to QA. Our responsibility is to only report HIPAA violations. JMO
