The instructions are on their website:

"If some of my information on MyMercyChart is not correct, what should I do?
If any of your information in MyMercyChart is not correct, please contact Health Information Management at 312.567.2240. Please do not call technical assistance or your primary care physicians."

Even if that is the wrong Mercy, the instructions will be exactly the same. You will need to contact the HIM department, probably where Sarah Jones works/ed. If you don't know, they can find out.

It makes no difference if they know or do not know the person who entered the information. There is a way to correct information. It is required by law.

If you have to, go to the Director of Health Information Management with a written complaint. Copy it to the director of the healthcare system.
