says, "eighteen slash ten slash three four," and I am not sure how to transcribe the last part.  I am a long-time transcriptionist but have never heard the measurements given that way, so any help from you cardiology gurus would be much appreciated.  Thanks.

so I just sent it through with a blank and will see how they correct it. Thanks for the reply, though. I've been doing acute care for nearly 7 years and have never heard it dictated like that. Oh well, we learn something new every day, right? Have a good one!

Back Extension And Side Bending Degrees Help

I am pretty new and trying to figure out what this doctor might be saying.  He is doing range of motion tests on a patient.  He says "Back extension (s/l 50 or 15) degrees."  Then he says "side bending (s/l 50 or 15) degrees."  The patient has severe neck pain.  I am not sure which value the doctor is saying.  Any suggestions? ...

Those of you who claim to make TONS OF MONEY TYPING....  You are not straight typing MTs under regular circumstances IF I believe you...and I don't.   You probably work 7 days a week, all hours of the day like most MTs.  You DO NOT work a regular 8-hour day, 5 days a week and make that much money.  If that were true...there would be much more competition.  The only perk is being at home.  Period...End of story.  Stop trying to sell me your ocean front pro ...

JVD Measurement

Thus far I have only had doctors refer to the JVD as flat or raised or sometimes at zero.  Tonight, this doc says "JVD is 0 to 9 degrees/90 degrees" [I think].  I have found a site that describes JVD measurements, and one of them is 90 degrees--but there is no "zero to" part. Now I am second guessing my thought about it being 0 to 90 degrees.  This doc is very sweet, but really mumbles the ends of his sentences. Anyone know about the JVD?? ...

Under the musculoskeletal portion of the physical exam, it is dictated, "Crepitus of the bilateral knees.  No ankle tenderness or swelling.  No MTP tenderness.  Last (s/l Sjogren's) was 14.5 cm."  I'm unsure what she would be measuring here. The planned oblique skin incision along the longitudinal axis (or access?) of the mass was sterilely infiltrated with 1% Carbocaine solution and the skin incision made.
