Rule for adding A - peach

Posted: Mar 24, 2010

Hello. It might just be too early but can someone tell me the rule as to when to add A?  Like the doc says

HISTORY:

72-year-old woman who.....

Should I be adding A to the beginning?

HISTORY:

A 72-year-old...

Never did BOS but sm - Txczech

Most of us just automatically add "A" or the "patient is a" ----

Just my take as an old MT.

that's not the reason. ...see inside - "A"

[ In Reply To ..]
You have to start with an "A" since you cannot start a sentence with a number. And this always.

Sorry. I must have misunderstood the question. - Txczech

Nevermind.
Txczech...you understood perfectly well... :) - brrrr

`
no, she did not, we do not enter an A "automatically" - ??? sm
[ In Reply To ..]
there is a reason for it:
It is the beginning of the sentence, NO numbers!

see message - sm

[ In Reply To ..]
A 72-year-old patient...

An 8-year-old boy...

I would because you do not use a numeric number - SM

[ In Reply To ..]
to begin a sentence. So-
A 72-year-old or
Seventy-two-year-old

Yes...sm - brrrr

[ In Reply To ..]
You should add A or The. Never start a sentence with a numeral that's not written out. Seventy-two-year-old woman, never 72-year-old woman. It's always easier to just add an A. :)

Adding Disclaimers
Jan 27, 2015

Would it not be simply wonderful if we could put disclaimers in the notes to the client, stating "job done to the best of my ability, however, due to your bad dictator habits, either eating, chewing, talking or answering your cell phone, your dictation is utterly and absolutely a pain in the neck to do because you will not dictate properly and/or will not stop answering your page, and/or you are not even familiar with the patient"!!!!....oh how I would just love to attach a disclaimer to many of ...

Question About Adding RAM To A Laptop
Jan 09, 2012

I have 2 GB of RAM on my laptop and want to add 2 more.  There is an open slot to add it.  My question is do I have to take out the 2 GB already there and replace that too?  I have seen the memory sold in pairs, so they are made by the same manufacturer.  Do both cards have to be the same?  Can I just plug in a card by a different maker than the one already there?  Any help appreciated. ...

Who Is Responsible For Adding The Templates?
Sep 13, 2013

Who is responsible for adding the templates?  I am finding a lot of errors lately in these when I pull them in.  For instance, spacing.  Several, if not many, are only single spaced between the end of a sentence and the next and only 1 space after a colon.  Yet another thing to slow us down.  Got "dinged" for pulling one in and not changing.  I assumed (yes we all know what happens when you assume).  That will never happen again if I can help it.  Also noticing 1 space between state ...

Adding More To My Plate - M Modal, Transtech, Accentus, And/or SoftScript
May 11, 2012

I looked through the archives and found not a lot current about any of these places as far as pay and how the accounts run (straight typing or mostly editing) So I am testing or interviewing with all of them right now to add to my egg basket.  Transtech offered 8.5 cents straight typing and 4.5 for editing, said it would mostly be editing.  I hate editing.  Wanted more straight typing.  Only really good thing I could find was the paying $40 for your internet after 90 days.&n ...

Rule Changed?
Jun 29, 2011

murmurs, gallops, or rubs  OR  murmurs, gallops or rubs? I am old school and taught that the first is correct, but I waste a huge amount of time and keystrokes correcting this and other instances like this.  I looked at the AAMT link, but really didn't get a sufficient answer if the second is OK to leave as that is the preferred way ASR puts it; however, every grammar site I looked at online puts the additional comma in. ...

ESL Rule Of Thumb
Nov 17, 2011

As American-born, English-speaking doctors are becoming the minority, I think all companies should openly state what percentage of ESL will be encountered in a new job and then ask 1 year of experience for each percentage of ESL; e.g. 40% ESL (the maximum I can personally tolerate) 4 years experience plus a differential in the line rate, too. What do y'all think? ...

CMS Final Rule
Jan 30, 2013

This was published in the Federal Register last week, correct?  I have a friend who is in a battle with an insurance company over a very sick child.  They are always throwing up obstacles for coverage. She told me that out-of-network treatment is covered at 50% of 200% of Medicare rates.  So we are trying to uncover exactly what that rate is.  Is the CMS Final Rule document what  I am looking for to help her? Is Coder here?  Expert insight would be so appreciated ...

New 6-minute Rule-sm
Aug 14, 2013

When you get NJA you have the 3 options as we did before.  When you choose to "make up time", at the very end of the explaination, it says something along the line of entering a leave request on teamwork for excused no pay?  This only makes sense if in fact you cannot or do not make up the time, but the policy does not explain it that way.  Example:  You work 2 hours and get NJA.  You have 6 hours to make up and can only make up 4.  The remaining 2 hours must be en ...

What Is The New Rule For How Many Days Off Without Pay
Nov 24, 2013

I really don't see what it matters now that we don't get PTO and if we aren't getting paid for it so what. Seems stupid to me that they would care how much time we have off if they aren't paying us anyway but anyone know now the most days you can take for the year part time and full time?  I know they took away PTO for part timers and cut it for full time but wasn't sure of the new rules. Also, when does it start over again if you used up your unpaid time off days. ...

HIPAA Re-release Or Re-disclosure Rule Anyone?

Did They Change The Rule About Not Starting A Sentence With A Number?
Jul 15, 2011

I'm just curious as I'm seeing it constantly now.  For decades, I have always been trained and told never to start a sentence with a number and to spell it out or add something like the work A or The like: The 20-year-old patient... instead of:  20-year-old patient...  I've been correcting it, but since I'm seeing it all the time, I'm wondering if this rule isn't followed anymore and, if this rule is still in place, they need to start training these ...
