Rule changed? - no name

murmurs, gallops, or rubs  OR  murmurs, gallops or rubs?

I am old school and taught that the first is correct, but I waste a huge amount of time and keystrokes correcting this and other instances like this.  I looked at the AAMT link, but really didn't get a sufficient answer if the second is OK to leave as that is the preferred way ASR puts it; however, every grammar site I looked at online puts the additional comma in.

Doesn't matter what you put, QA will change it - to the other anyway

Yeah, ASR does not put the extra comma in and I AM NOT going to put it in. My CCM said pretend ASR has no brain, duh! It puts a contrast agent with no cap in the body and capitalizes it later, go figure. And sometimes it puts left-sided fracture in one spot and left sided fracture in another spot. And I have a verbatim (spelling?) accout and ASR sometimes puts in There is bla bla bla when it definitely not dictated, so I have to take it out. It sucks. I have been doing ASR for about 3 years and it still makes the same mistakes.

Here is what I found in BOS 2 (page 90 PDF version) - JAZ

"Using a final comma before the conjunction preceding the last item in a
series is optional unless its presence or absence changes the meaning.

Ears, nose, and throat are normal. (final comma optional)

No dysphagia, hoarseness, or enlargement of the thyroid gland. (final
comma required)"

The results showed blood sugar 46%, creatinine and BUN normal. (no
comma after creatinine)

Hope that helps. Personally, I always add the last comma because it just does not look right otherwise to me as I am old school also, drives me nuts. When I asked QA about it, she said either way is acceptable and she would not mark against the last comma on an audit.

The company I work for wants the final comma in a series included, while other MTSOs may choose to leave it out.

client/facility/MTSO wants it done and different depending on who you work for. The client/company I work for goes by the BOS, other companies I have worked for do not use the BOS and have their own rules. Like I said, really no right or wrong way, just have to adjust to each new employer's way of doing things to stay working.

of a day and I certainly take advantage of every little character I can get, LOL. ASR leaving out that last comma drives me insane, so I probably would put it in regardless.

This was published in the Federal Register last week, correct?  I have a friend who is in a battle with an insurance company over a very sick child.  They are always throwing up obstacles for coverage. She told me that out-of-network treatment is covered at 50% of 200% of Medicare rates.  So we are trying to uncover exactly what that rate is.  Is the CMS Final Rule document what  I am looking for to help her? I really don't see what it matters now that we don't get PTO and if we aren't getting paid for it so what. Seems stupid to me that they would care how much time we have off if they aren't paying us anyway but anyone know now the most days you can take for the year part time and full time?  I know they took away PTO for part timers and cut it for full time but wasn't sure of the new rules. Formatting Abruptly Changed

There probably have been answers to my question out here before, but does anyone know what causes the formatting in an EditScript document of which I am either editing or transcribing?  It tabs, bolds, changes font sizes, and messes up the spacing between lines.....things I did not manually do myself.  Is there something I should be changing on my RTF? I've talked with several MTs recently who work for different companies. The companies have to send out reminders to the MTs that they are supposed to be at work at specific times. They also are having trouble with MTs just taking off without arranging for coverage of their accounts. The list goes on and on. What bothers me most though is the attitude of workers.
