Discrepancy on birth date - Southern Belle

I'm new at this, so please don't rail me.
The doc says she's an 87-year-old woman, but dictates her birth date as 2003.
What do you do in cases like this?

Unless there are other clues in the report that point to her either being 7 or 87, then I would either change the age or birth date as indicated, but only if I was 100% sure.

is that he said 8, then corrected himself to 7, and that it made it sound like 87. I agree, though, that it should be flagged for QA unless there are others notes in the report to verify the age.

Nobody has yet suggested seeing if your office allows MTs to flag directly to the client. If they do, you can put the same type of notation as you would for QA--verify patient age/date of birth.
I started the application for Q.E.D. Transcription and saw that the initial section requires a birth date. It won't stop me from applying, but isn't that a form of discrimination? I wish the MTSOs would at least provide a secure site when they require personal information such as physical address and phone. ...
Sounds like Prosidia or Lucidia? Cell phone dictation, which is completely warped. The patient has dysmenorrhea and anemia from heavy menses and is going in for a supracervical hysterectomy (this is a preoperative HP), so I would really appreciate the help. Is anyone else noticing a discrepancy between what the productivity number is during transcription and the weekly productivity number posted to your email? My productivity number during transcription is running 20-30 lines higher than the final number. This has just recently started happening. ...
I have been spending the same amount of hours, doing the same accounts and my lines have been decreasing. Is there something going on that we are not aware of? Has anyone noticed a reduction in lines? I completed 2 speech documents this moring. ESL, but not terrible. I copied/pasted the body text (no headers/footers) into blank Word document and ran the Word Properties after each was completed. I apologize to sound like an Webmedx complainer. Not my intent, but I am extremely concerned regarding the SR jobs I'm transcribing and receiving correct compensation. The way I understand it, an SR/VR draft report already has a tallied line count when it goes to an MT and if during the course of your edit, you delete lines, the line count always reflects this by being less, it does not stay at the original SR drafted line count. Over the last two pay periods, at the end of the period, the amount of hours they tell me I worked is extremely low compared to the hours that I actually worked. I took some pre-employment documents to a notary yesterday. The birth certificate, issued by the hospital where I was born 62 years ago, I was told would not do for ID because it was not "state certified". In order to get a state certified birth certificate, you have to have the notary certify they have seen my birth certificate, then the state issues one they call "certified". How do I transcribe 8th? The patient's last visit was 6-4-10 so I assume the scan was done in June. Since it is not my job to assume, how do I transcribe this?

Does anyone know if there is a new rule in MQ land about never writing out the date, always making it numbers only? I get back reports every once in a while with all the written dates crossed out and replaced with numbers yet the CP does not specify either way. In transcription, when a doctor dictates 12/4 or December 4th what is the correct way to type this? I need this information when I'm taking a test to find a job because the doctor has not been giving the year and from what information I've read I should type out the date as December 4th??
