Report audited in Fiesa is one I did not do - Has this happened to anyone?

I checked my Fiesa today just after my shift and saw a report that was supposedly done by me, even had my HRID number, but I did NOT transcribe this report. I only worked 3 hours today and I would have remembered this one. It was a very, very long report, and I ONLY got a bunch of short ones today. I did not do this report. It also was a WT I rarely get and definitely did NOT get today. I tried to be open and so I really looked at it, thinking maybe I did it and forgot, but I really doubt it. It was only QC1, so doesn't affect my pay (this time), failed of course because it's on Fiesa to be checked. But I can't overlook the fact that it had my HRID # on it. I don't think I"m losing my mind, but I'm starting to wonder. Is it possible for a report someone else did get erroneously audited and sent to ME as mine? I will look for the job number on my metrics to see if it is there next time I sign in, but I am sure I will not find it. What does anyone else thing of this? Am I going crazy?

That job number is NOT in my metrics. I looked twice and then two more times. It's not there. I KNEW I didn't do this job. How on EARTH does it have MY HRID number on it???? I can't even call my TSM as she is on PTO for the next week. I guess I shouldn't care as it's not a post audit, but what if it IS next time? ARGH!!

Another MLS' feedback (ding for me!), in addition to QC filling in blanks incorrectly (ding for me!), someone at the hospital adding something against client specs (ding for me!). All were post audits. Luckily, my TSM is fab and helped get all reversed.

I for one will be carefully recording all jobs I do - but it will just take more time....

It's happened to me too. These days in FIESA I pay attention to the facility name and dictator name, as on rare occasion they have not been my jobs, which I can usually spot pretty fast. Or it can even be a job I typed but the name of the facility is wrong. How QC does that, I don't know. Anyway, I just email my TSM & she notifies QC to have them fixed or removed, no problem. If your TSM isn't there, I'd try contacting the TSM's sub, or else maybe do a Reversal Request & state the problem including the fact your TSM is away.

It had three major errors, and it was proven that I didn't type it, but they wouldn't reverse it because I didn't do anything about it prior to the Saturday 5 pm deadline. I logged out of work at 2:30, the FIESA email came about 15 minutes after that, and I didn't see it until Sunday morning. VERY discouraging. I have no idea how this happens.

If it's QC1 and was a long report, it very well could affect you in the QC% allowed to go (10%). Like others have said, contact your TSM sub or even your COM!

I'm definitely contacting him. I have to wait a week because he is on PTO and so is his backup. This is infuriating. I probably won't have time to do anything about it.

Believe me, I considered that and I looked far back in my completed reports looking for that job number, it was not there.

I have always kept a log of each job I do, and came in handy in this case. It took an act of God to get it reversed, but it was a post audit report with many errors that were credited to me, however, the facility was one I had never worked in and had no access to. My TSM insisted it was my report, even though it had been transcribed on a date and at a time I am not scheduled to work, had no webclock punches for, and no other jobs done by me during this time.

I have also had TSM Spark message me about finishing up a stat report ASAP that she showed had been checked out to me...but again, an account I had never been assigned to, never worked in.

The QC or QA had to manually add the MTs ID number back then. I don't know if that is still the case, but it was when I worked there about 4 years ago. The system is not made for QA functions, if you ask me. I much preferred DocQscribe (obviously different company). It was all automatic. You could jump to the blank and didn't have to manually add any IDs.

I have never really bothered to look up any audited reports to see what I did wrong, because my average is always high enough that I don't worry about those little errors, but I wanted to look one up today and I have no clue how to find it after it has been edited. I know how to look it up in the statistics where I can see my output, but not the corrected version. Thanks! ...

I recently had 0.25 error in FIESA for having "complaining of lateral-sided pain" instead of "complaining of left lateral-sided pain." The comment that the reviewer made was "Downgraded as clear throughout the report." What does that mean exactly? I rarely ever see any comments at all from them. Am I being audited? Lately, I have had work show up in my QA'd in box that I have not submitted, but there is no score in the score column. These aren't doctors that have previously been on 100% QA. Just got an audit report. I noticed that the subject line said re-audit. Huh? Didn't even know I was in "trouble". I am the MT who failed an audit last week by 0.15%, and, I am also the person who got dinged for "adding" the word "and" when they claim the doctor said "uh". I passed this re-audit. Wonder if I will be audited again next week. I think I have been audited 3 times in the last 2 weeks. Twice since "failing". So, I have passed 2 out of 3 audits. ...

maybe its my luck to have an extremely anal auditor, but find it hard to swallow I am getting dinged for a's and the's, therefore in a very long report (234 lines), I do not make the ridiculous 99%, like 98.75. it seems like when I request time off, maybe for 4 lousy hours or whether it be a whole day I either get audited or have a meaningless 1.9 long report come back to me within 20 minutes of requesting time off which the report had the word "the" left out which did not change the report "only educational for the MT" . I found out last week that jobs submitted for blanks to QC1 can still be pulled for post-audit. You have to double check your jobs submitted for QC1 against your post-audits because if you don't catch it you can and will be marked off on your post-audit for something the QC1 or QC2 reviewers made a mistake on. Anybody know if there is a "standard" for number of lines to be audited on? All of my audits have been for under 200 lines. I never pass. It seems to me that if you sample such a small number, you are more likely to have errors and thus fail an audit. Are each of us being held to different standards as far as lines audited? Are some people audited on 500 or 600 lines and others to a smaller number? If we have to be held to 99.7%, shouldn't there be a cons ...
