I've made lots of 100% accuracy scores, but I just saw that I made a 3.0 Critical error on a VR dictation at Nuance.  It was where the doctor said a medication amt that sounded very much like another amount.  I have been extremely careful with the medication amounts and the laboratory values, but made this mistake!!   Anyway, that brought my accuracy to just under 99%.  I want to go back and listen to that dictation again, bet it was hard to tell the difference in the amounts, although I guess you CAN tell some difference because the QC person obviously heard the difference.     I also could swear I have been adding cc's when dictated, but I missed two of them!!  Those are my only errors, but I know it's really bad to make errors on medications and I know we can't get below 99% accuracy overall.    My self confidence has really taken a hit, not to mention wondering if I will get fired.....    I don't know how to be more careful than I am already!!   Does anyone else struggle with keeping above 99% accuracy???

Don't be too hard on yourself. At most places, you won't get fired for a one-time slip up. What I do if I am at all unsure of med dosages is go to Drugs.com and look up the medication, then check to see what dosages it comes in.

As far as cc requests go, it's better to have mistakenly omitted a cc request than to send a cc to the wrong doc/place. That is a HIPAA violation and you could be in more trouble for that.

If anything sounds unclear to me, I leave a blank, including labs and meds. Didn't realize we get dinged for missed carbon copies though. I could see if you sent one to the wrong person. Sometimes they just rattle them off so fast, you cannot hear them. Also, maybe you added the copy, but it just did not "take". Don't beat yourself up about this. The job stinks and they just push and push and push. It is near impossible to keep on top of this.

The problem I have with VR (aside from the horrid pay) is that it lulls me into a false sense where I just find myself following along at times. It is so monotonous and makes me drowsy.

It really hurts when you have a bunch of 100% jobs and then get dinged for a not so good one. That happened to me last quarter, and I was on pins and needles hoping I stayed above 99%, which I did, but it sure ruined my concentration. This company needs to look at the big picture. They aren't improving our accuracy, they are ruining it with all the push, push, push.

I hate the "99%" thing. The more I worry about it, the worse I am, so I just do my best. If they dont want to give me the extra miniscule for being over the quota, well so be it.. My health is worth more than that bit anyway. Stress will kill you, missing a few cents wont.. ;0)

I struggle with it all the time. Whether it is missed commas or a word (which when you are doing VR it is easy to miss words sometimes) I have the same problem. I can have all these 100% ones and then a couple below, most are brought down because of all minor errors but I have had my share of major ones too. I always wonder how they hear things that I don't hear

I am not even worried about the bonus, just don't want to get fired. I want to be the one who decides when I quit. I am a hard worker, diligent and careful. I am completely stressed out over this.

I was under 99% and had a critical error (maybe 2 actually) and they never said a word. The next quarter, I got over 99% and back in bonus position.
Unfortunately there is no work, so no bonus anyway.

I worked for a company who was owned by Nuance. I had the same issue. I could swear I heard the med correctly. It made sense for the condition the patient had. Like you I always double hear what I type.I knew I heard it correctly. I asked if I could listen to the dictation again for my sake "for learning purposes." I was told, "No, once it is listened to by QA and then input goes back to you it is out of the system" What? Out of the system? No, that dictation is there somewhere? No, one one wanted me proving that the QA person was incorrect. I still believe she was wrong and I was right and I had no recourse. This happened to be several times a week. I am a seasoned professional with over 30 years with a prior hospital with high high QA scores. I know my stuff! Now, just about every day they find a Major Error, which by the way they do not let me "prove" I am right because somehow the dictation is now all gone. Who is keeping check on the QA persons accuracy? I despise Nuance and left them for that very reason. I could never make them happy, never, I worked 13 hour days to get my lines in, because I was being exra careful, looking up every thing cause they were so picky, and even when I found things on line on documented sites, guess what, I was wrong with the way I typed it. Oh Please, this place is off the wall. Anyway, before I could leave they fired me. Yes fired me and my 90 day probation was not even up, only worked for 60 days. They told me I could not get unemployment. Well, I did, because unemployment said "I did not do anything Intentionally wrong, malicious act in my work, and that the company did not allow me time to prove I could do the job. So now they are paying me 3 x as much as they "paid" me if you could call it that, for me on unemployment. I would never work for a Nuance owned company again. it is important for you to ask if their QA goes to Nuance when you apply for a job. My self confidence took a dive. I knew I was right and they were wrong.

I am sorry you had to go through that! It does not make sense that they cut off their nose to spite their face. For instance when we number a list of past medical history, and if they say "Past Medical History includes" but we only transcribe "Past Medical History" and then number it below, we are charged a MAJOR ERROR because we did not put the word "includes". What else would the numbered list below it be other than to infer that it "includes"? Does it "not" include??? of course not. I have transcribed for years and know my stuff. I agree with Elona in that my self confidence is taking a dive. I feel that they will fire me soon.

It means the list "includes" all of that stuff but is not necessarily everything. It is the doc covering his behind.

Having said that, I find Nuance QA to be petty and ridiculous most of the time. My issue, however, is getting a major error for leaving off the word "the" and other tiny non-meaning changing words.

It also ignores "significant for." We might have to recap or change punctuation depending on account (one I have is MEDICATIONS INCLUDE: and another MEDICATIONS: Include:), but they if they set up the recognizer to not disregard and ignore these words the way it does "next," etc., then we'd at least be unable to miss it being dictated. ::banging head against the wall::

Today I had 3 critical errors in 1 report. Two of them were stupid stuff like poster above regarding the "include" word. One was legit. Sorry, but I am so over busting my butt here for peanuts and never doing anything right according to them. I have 30 years in and I'm way over being a MT. Today, my arm is swollen so will be seeing a Dr. for this. I can only hope I can file Workman's Comp.

...correct me if I am wrong, but I believe 99% is for the incentive, 98% minimum is the requirement for employment. I am usually in the 98% to 98.9% range but have been warned a few times about FIESA scores of under 98%. As someone else referred to, a short report with a couple errors can really take your overall score down. Oh, I also called on low production/MUP when I slowed down to be super-careful, and still have not been fired. The worst that should happen to you is getting an e-mail from your supervisor telling you to work on bringing up your score. So, take a deep breath and try not to let a few bad reports get you down. Instead, try to remember all those 100% marks you have.

Oh thank you for the encouragement :) I agree - I need to take a deep breath and hang in there :) Also, I forgot that it's 98% minimum for employment. That is much better and I needed to be reminded of that. :) Thanks---

Yeah thanks for the 98% reminder - I also tend to forget that because they harp so much on the 99%. They are comma happy. I came from Medquist and was told by them they would rather us leave the commas out and just straight type and was never marked off for not leaving a comma in the 10 plus years I worked for them - there were plenty of other problems mainly never any work and being on so many different accounts there was no way you could keep all the specs straight for each acct. No consistency at all. I have been lucky in that I am on an account that I really like and is almost all straight typing and hardly ever out of work so I really can't complain about that but the QA just stinks

Sure 99.7% accuracy is what they strive for in production of goods in a factory..To try for 99.7% accuracy in a job that has as many variables as transcription is a completely unrealistic goal. Speech patterns alone of different people makes errors almost inevitable. What they are striving for is an impossible goal. I just cannot wrap my head around why I continually get 100% accuracy and still get the shaft. This last week?.....I'm over QA by 0.23%, yet I repeatedly get 100% accuracy time and time again. if the 99% accuracy rate also applies to the ILP?  After all, CBay and Medquist have become one company called Medquist (something, it has changed too many times for me to remember), but I would think if it is company policy it should apply to all.  Most of us have an average of 99% or above accuracy rate, but I still have a problem with making it the standard rating.  Especially given the consistent inaccuracy or ASR and the increase of lines per hour for many MTs.  If this ...

Very shortly our accuracy requirement will be 99.6%.  Since this company cares nothing about accuracy, my conclusion is two-fold:  1) As they always have, they are going to use this as a marketing tool.  2) Anyone who falls below 99.6% will have their pay docked thus reducing the company's labor costs even further and will probably result in administrative bonuses.  For new employees - welcome to MM.  For everyone else - get out if you can.  ...

I started at Nuance about a month ago. I am a new MT, graduated from CareerStep with Honors at 94%. I've been transcribing for 2.5 weeks now. I have quite a bit of anxiety because my QC scores are not improving, sometimes even getting worse. I seem to have troubles with numbers in particular. I will listen through labs or vital signs again and again and I am sure that I am hearing 116, not 160. I am just beginning to feel hopeless, like I am not good enough. I just checked the accuracy of one of our best ESL dictators after editing.  He has 28% accuracy.  Total words: 350.  Total errors: 64 (words and punctuation).  This was a full page of report. FlashCount Accuracy

I downloaded FlashCount and love the ease of use.  However, it gives a count which is slightly higher than when I manually do it in Word by dividing characters with spaces by 65.  Is there a way to adjust the settings in FlashCount so it will give me the same count.  I wonder if it is counting things like bold, etc.  Or, is there another counting program that will count batches of files and do it accurately?  It need not be freeware.  I will pay for it.  Years ...

A friend of mine works at Nuance (formerly worked at Transcend, but of course Nuance is gobbling up every MTSO) and said they require 99% accuracy. And rather than an average, they run the accuracy on every single report so that even though, for example, 5 reports may be 100% accurate, if one is 98%, you're screwed. I've been thinking about line count. To figure if Nuance line counts are correct,  just go to the eScription's old reports and do a search for 1 day of your own reports.  Then,  just copy and pasted them in to word.  Then do a line count.  Wouldn't that work? ...

On the account I work I'd say only about one dictator in 20 can achieve 99% accuracy with VR without editing.   This doesn't surprise me in the least. We could have told the developers this 20 years ago, but money talks.  For the doctors forced to use VR, will the errors be acceptable to them?  To the big wigs at the hospitals?   ...
