Did any of you on the Ambulatory side get the e-mail this morning about the exciting new changes?  What is your take on it?  I, for one, am not "excited" - but more like horrified.

On the Ambulatory side, we have exclusive dictators...but it looks like they won't be so "exclusive" anymore. Sigh.

I did too. Not sure what to think. Plan for full integration with Nuance. They are switching TSMs and now want work completed within the first 4 hours of receiving it, even if its 24 hour TAT.

When I saw "normalization of platforms" on that email they sent earlier this month, that's where I thought they were headed. My suspicions are confirmed. Good luck keeping your quality percentage above 99%, because now we're going to be doing work from every single account Nuance has. What a load of crap.

I used to work at MModal and every day I would get work from dozens of different hospitals from every state in the country. I had a huge stack of account specifics on my desk and every dictation I got was from a different facility. If that happens here, then I am done.

My experience with MModal was a nightmare. Even working as Medquist, it was pretty much a nightmare. I think for our profession to be outsourced to India is a travesty. I am ready to hang it up after over 20 years in this profession, some years as a lead, for one of the largest and most demanding hospitals in So. Calif. I have no idea who to trust anymore.

as the info came out last week. Nobody knows the lines rates yet, but they are sure to not be good. Ambulatory was a good ride while it lasted.

what was the content of it? If I didn't get the email, does that mean I might not have a job? LOL! I saw the email changing our InScribe log in with a bleep about integrating into Nuance, but no where did I receive anything that said we now have a TAT of 4 hours. Had no clue what "integrating" into Nuance meant. Guess we in ambulatory are not Nuance. Quite probable we will all be on FIESA now, but as far as I know, ambulatory has not been told that.

I was wondering if everyone got the email, because I have 3 TSMs and I only received the email from 1 of them. So I'm wondering why ALL the TSMs are not sending out the same email? The email states that the changes are "across the board."

As far as the changes, basically they are now saying that when your exclusives dictate, if you do not transcribe the files within "a few hours" the files will then become available to other MTs to transcribe. So your exclusives are not so exclusive any longer.

How long is a few hours? How would you know at what point other MTs are going to start doing your work for you? I am so frustrated right now. I have only been here a few months, but I was just starting to really get into a groove with my workflow and schedule, and now this?? Really?!?

Only a few ambulatory MTs posted here. most of us have been keeping our mouths shut waiting for our turn, not wanting to draw attention to ourselves. I, for one, left Medquist to come to a small ambulatory company that eventually got gobbled up by Nuance. I am surprised it lasted this long. It was nice.

Well that email did say if we have exclusives now that we will continue to have them because we will pull those accounts as our #1 priority!!! Yay! So that's good news, right?! Oh.. but wait... you say you are pooling the work if we cannot get the work transcribed within that 4-hr TAT, and other MTs will be able to pull that work... so, wait a sec... THAT WORK IS NOT EXCLUSIVELY MINE ANYMORE IS IT?! Jeez... don't sugar coat the bullshit please. Just say it like it is... no more exclusives, work will be pooled, and next we will be picking a set schedule and getting pay cuts. Right, Nuance? I'm right aren't I? Yep... I'm right.

Nothing new. Surely you figured sooner or later they would get around to incorporating you into the big exciting Nuance mix, right? I know a few weeks back one of the ambulatory MLS making thousands of dollars a month just knew this would never happen on that said "cuz they said so." Sorry this is happening to you, but hopefully you took notice a couple of months ago to the posts and made preparations for entering the below poverty level class. Exciting, 'ey?

I am sure most of us have been waiting for the other shoe to fall for a LONG time, waiting to be incorporated into the Borg. Now it is happening fast. I believe exclusives will be quickly phased out even if we keep within TAT. For those dictators who save up their reports and send them all at once, keeping within 4 hours TAT is probably not feasible since they are probably sending more than 4 hours work. Then if they make us follow a set schedule, well, that is the end of that since we never know when our doctors will dictate. I am going to try it and see how it goes. Have been thinking for months what to do next. Have may ideas but so hard to change when you have been doing something for so long.

You will most likely have schedules with other work - thrown in your mix so should be okay.

Exclusives? Man, that must have been nice! But, I do know what it is like to have the rug pulled out from under me on things like that so I am sorry for you too (even though jealous of the exclusives you have been enjoying).

I am now on my 3rd primary within 18 months time, due to my first account going offshore, then being moved to an account that 6 months later was lost to EPIC. Part of speed comes from knowing who you do well. Then, I found out I am now on a list of MTs that may be possibly moved to where they need more coverage.

Why is the most important factor in this (compensation rates) such a big secret? Why the silence. Why are TSMs not allowed to discuss or explain? How can you view this as exciting? It may be exciting for those at the top as they anticipate plan for increased bonuses and more exotic vacations. On the Ambulatory side, how do we find out what our QA% is?  Is it supposed to be in FIESA?  The only report I see in FIESA is from 11/8 to 11/14 payweek and it says Accuracy 100%.  There are no reports for any other pay week. Also, how do we find out what percentage we sent to QA?  I know under the new plan if we send more thn 10% to QA we get a reduced line rate, but I have no idea how much I send to QA.  Help please? ...

I was just wondering if anyone posts here that work for the Nuance Ambulatory Care side of things.  All I see here are what appears to be posts from the acute care side and they are not very favorable.  I will probably get hung on this board for saying this, but so far things are pretty good on this side of the fence.  Same managers, same good accounts, plenty of work.  Flexible schedule.  Not crazy about all the logging in to everything (WorldClient, WebClock, Spark), b ...

I work with the ambulatory team (ortho clinic notes). I have 2 exclusive dictators and 2 that are shared with a couple other MTs. I was originally with On~Time Transcription. We were acquired by Accentus, and Accentus was acquired by Nuance a short time later. The Nuance acquisition took place in November 2012 I believe, and we were switched from IC to employees earlier this year in April. Now I understand how there were glowing reports of being happy and making good money.  Welcome to the way acute care works ambulatory folks.  We have a pool of accounts and reports come in with the one closest to turnaroundtime first. No picking or choosing what you transcribe or edit.  Like I have 7 accounts and get all work types and dictators. At least that is how it works for my group that was swallowed up by the N.  We have a schedule and we have to stick to it unless we ...

Ambulatory is definitely different than acute transcription and probably better in a LOT of ways, but if it makes you acute care feel any better, the 24-hour TAT that is talked about was not a true 24-hour TAT, at least not with my company. If my doctor dictated at 11 pm, my reports were due by 3 pm the next day, so that is more like a 16 hour TAT. If my doctors decided to turn in their reports on the weekend, then I transcribed on the weekend. I worked when my doctors sent reports which ...
