New auditing "rules"...really? - chillywinter

Now in addition to monthly audits, we are receiving WEEKLY audits so they can pick through our work and find more "errors!"  Got dinged for semicolon, dash, and even word that was not clear at all, but it is being "verified" and if "they" (whoever "they" are) can figure it out, I will be held accountable for that also!!!  What is going on here??

It is distressing. You have so many things you are trying to do in a report that is mentally exhausting. Then, you have to worry about these audits. I just hope I can hang on through it all. How long of a process is it when they start going after you? This is so sad.

I have NEVER had weekly audits, only monthly ones. Personally, I'm glad. I failed one audit, and it was because they audited a total of THREE reports, so my percentages were way off. If you have a critical error with 5 or less reports audited, you automatically fail. At least with weekly audits, the percentages are decreased.

It's the only change with this POD crap that I'm happy about.

I just wish we knew what the "rules" were. Seems to me each reviewer makes up their own and there is no standard. And - oh yeah - standardizing CP's - how MANY years have we heard that!!!

Also notice that we never received a note from the BIG MMODAL Corporate about all of these changes.

Don't freak out. These "weekly audits" are not your "official" audit. The weekly ones are sent out as a courtesy so that you can keep track of your mistakes/progress each week so that there are no surprises at the end of the month. This also gives you time to challenge anything that looks sketchy before it is officially counted against you at the end of the month. The reports that are audited in your weekly audit are the ones that will be calculated into your monthly one.

The thought is, that if you are an employee making the same mistakes over and over again, it's best to let you know about it each week, than to let you go until the end of the month for your "official" audit.

This is actually a good thing. The weekly audits don't count against you, only to give you a head's up. There is only ONE official audit and that comes every four weeks.

How would they know if you are making the same mistakes over and over again if they are only taking a SMALL sampling of your work?? I also find it hard to believe that out of the hundreds of reports we do that there would be so many "errors" in the ones they pick.
If you're concerned they are faking errors - Proofread

For a complete day, after you finish a report go back, put the speed all the way up to 2.0 and proofread your work. Find errors? Fix 'em. You might be surprised at how many little errors your eyes miss when you're doing ASR becuase you're picking up on so many major errors.

Once you can go a complete 2 hours with no errors in any report, go 2 hours without re-proofing, then do 2 hours of proofing. No errors again? Go 2 hours without proofing. The following day, don't re-proof anything, and on day 3 re-proof for the first 2 hours of your shift and if no errors, don't re-proof anything. Second week, don't re-proof anything. Third week, re-proof only reports that are longer than 300 seconds. No errors, stop re-proofing.

that turned this once lucrative field into a minimum wage job by constantly having to check and recheck every keystroke and having them look over your shoulder. We are trained professionals and should be treated as such...not some child that deserves disciplinary action!

Name one job where you don't have audits or performance reviews. Name one job where if a mistake is made and someone notices, where disciplinary action won't be taken against you. One...just one job.

You're complaining because you work isn't up to par and you're complaining because your poor work isn't being pushed through because you're a professional. Instead of complaining that people are actually reading your reports, fix the problem...which is your work standard. It's not MModal's fault you aren't passing your audits.

In fact, in spite of all those cuts in pay and higher expectations, they STILL have to take money from their own check every month to supplement supplies for their classroom.

Technically, teachers PAY to work. It doesn't suck any worse than that.

If, when you get your weekly audits, you realize you keep seeing (for example) a specific punctuation error over and over then you will know there is a pattern building that could cost you big on your monthly audit.

Another example is, if you receive 2 weekly audits and you fail them BOTH ... it might be time to buckle in and slow down so you can still recoup the month by doing better on the other 2.

You see, it's kind of a "check up" on how you're doing before your monthly "official" audit comes. Wouldn't you rather know how you're doing before you get slammed by surprise with a failing audit? Or conversely, if you're having a really great month, wouldn't you want to know that too, each week? :)

My last 4 months of audits have been all over 99.7% and I've only had one critical error ... ever ... and it was 6 months ago. When I think of a "really great month", I think of a passing audit score of 99.5%+.

I will confess that part of my audit scores come from:

A) Getting a weekly audit (and reviewing it ... every week) so I can keep tabs on how I'm doing *before* my official monthly audit, and

B) Keeping my audio play at a reasonable speed and relistening to the last 5 or so words in a replay during an edit before moving on with the dictation. I've been able to catch a lot of "minor error" stuff that way.

For me, it comes down to my attitude and my concentration. I could sit here and play on the computer while DQS runs in the background and I get nearly nothing done, or I can set a goal in my mind and meet that goal every day. My audit scores over the last year have been over 99.6% except for once.

I had offshore QA'ing my work and pointing out my 'errors' such as replacing all the commas in past medical history with semicolons, dinging me for "the patient" vs "patient"/over-editing in VR, and other nonsense.

Contrary to what the poster said below, this did not come across as a "courtesy". They were nit-picky, inconsequential, stress-inducing, and frustrating to the point of tears. Add to that the 25+ primaries I had + laughable line count + cumbersome/unpaid time with Fluency + working every holiday, and I quit (and I am not a newbie - have 20 years in the business). I found a wonderful family-owned MTSO, no stress, straight typing, and was promoted to QA within weeks. Go figure.

if they review at all of our work but because most are 100 percent, they don't upload to Fiesa, just choose ones with 99 percent or below and occasionally throw in one that is 100 percent to make it look good!!  I do NOT trust anythng they do and would not put it past them. We have no way of listening to even make sure that what they audit us on is accurate.  I am so paranoid that I think they may even change things that we type to make it an error!  So disgusted..... ...

Seriously, the only person I trusted for corrections was my QC.  If they are not going to be doing the audits, who is?  MMEs are just upper grade MTs who were suckered into working for less pay.  MModal has fired/laid off/chased off most of their competent MTs.   I can hardly wait until I get my first (and probably last) required audit.  A 99.6% accuracy requirement from this company of corporate incompetents is hysterically funny. ...

Late last week I recieved an email communication about how they will only audit reports that were transcribed/edited during the current work week, only to log on this morning (I'm off Fri and Sat, so Sun morning is the 1st day of my week) to find that I had a post audit score below 99% that was audited on Saturday, so obviously it was edited last week sometime.  If I don't start my week until Sunday, how on earth can they audit a report on Saturday that was submitted the week prio ...

Couldn't we just leave a note on any report with a cc to "please verify physician name for copy" which would then take the burden off of our shoulders? Not a QA marker, but by doing Ctrl + Shift + Q. I plan on leaving a note for anything that is questionable at all, ie., only last name given, unclear, cannot verify, whatever... Okay, I got a ding today from my extremely sweet and helpful QATL.  It seems I did not send as a cc a requesting physician dictated.  I looked at the report and I did enter the requesting physician in the ADT and the body of the report.  I thought if they did not specifically request a cc be sent, then you did not.   Am I wrong?   Old dog needs help.  Also, I thought if the Req Physician was in the ADT it automatically received a carbon copy..  Hmmm.. ...

I was wondering if anyone had any knowledge in regard to HIPAA violations and whether certain rules apply only in certain circumstances.  I had a workers comp claim and had my IME closing exam in March.  There were problems - the doctor assumed that he knew it all, didn't take any history from me and then rushed me through the exam and closed my claim.  I wasn't happy with the way the exam was conducted, and then on reading my exam report I found a number of mistakes, so ...

Our department was recently "onboarded" by a transcription service where we are now paid by the line.  We have had several days where we had "mandatory OT" where the boss would email and tell everyone they needed to do 1.5 hours of OT that day.  In my state, VT, the law says if you are required to work over 8 hours in a day you have to be paid OT pay.  Does anyone know how this works for pay-by-line work?  I know many of the practices of this company are questionable and ...

Abbreviations In Impression Rules

I know that according to AAMT we are not supposed to put abbreviations in the Impression/Diagnoses sections, but does that include ALL abbreviations?  Or just ones pertaining to diseases?  For instance, are we supposed to spell out MRI in the impression?  Or just diseases, like PUD or COPD?   I'm a bit confused because I put "MRI" in the impression and QA left that as is, but I also put "PT" and they expanded it out to "physical therapy."
