Someone ought to take these ESL doctors and torture them like they torture us with their unbelievably horrible English speaking skills! We should not be told we're jeopardizing patient care - it's the ESL doctors. I've never had an ESL doctor and I'd refuse to be treated by one who can't speak clearly or speak a complete sentence. Jerks!

The only time I get transcription is when the dictator is too ungodly horrific for ASR. I'm beyond agonizing over every blank with these dudes. Go ahead and audit me - or better yet, fire me! I came "this close" to breaking the report, but I wouldn't do that to you guys.

The ones I can't stand are those that don't even make the effort to be comprehensible. It's so insulting, like they feel dictating the report is beneath them and the MT can just lump it if she doesn't like it. Some of the ESL guys I've got are alright. They speak slowly, spell words they know they mispronounce, etc.

There are 1-2 (out of many, many ESLs I have) who I would refuse to see as a patient because I can barely understand what they are saying.

When my daugher was sick she had 2 ESL NICU doctors and whenever they spoke to me I would have to close my eyes to understand them. God knows we hear accents all day every day but face-to-face was horrible. I finally explained to them why I was doing that and they were honest-to-God surprised that we would have difficulty transcribing them. Amazing.

It's the Indian ESLs that I get in 75% of my reports. I don't know how many dialects there are in India, but I'll bet in their own language they don't pause at all between words or inhale at all when they speak, and it's like their jaws are wired shut. Most don't have a clue how bad they sound and I assume they just don't care, or are just plain cocky that they're making the big bucks to send back the rupees to their family in India. At the end of a sentence instead of enunciating "period," why do they say "pada" or "peda" or "pra." Don't even say the word 'cause when they do, that sound is added to the last word of the sentence which always throws me off. I add a marker, then realize they are just trying to say "period," have to go back and erase that marker, with steam spewing from my ears with yet another needless roadblock to my salary. ESLs are such a handicap to my work, and no I don't find doing them more and more makes it any easier to understand the next dialect as there are so many.

What about dictating the first and second names backwards and then - when you have a patient who

has both names which could be either first or second such as James John or is that John James, holy moly!

You should be able to determine this by the ADT information, or - mt

do a search with both names if trying to fill in the ADT. If in the body of the report, many accounts don't allow patient names, so check you client profile.

There are a few Indian docs on my account who say para meaning paragraph after each sentence. I don't give them a new paragraph, just keep it in my mind that they are starting a new thought.

The worst is when ASR decides to pick that up too and you end up constantly deleting.

Honestly, wouldn't it make sense for the Indian Mts to do their own docs and give us back our good English American docs?

I've been doing this a long time, and I know how oblivious doctors are to anything approaching being polite when they dictate. I've heard them in the bathroom, I've heard them yelling at their kids, talking on the phone, listening to football games on TV while dictating, etc. We all have. I absolutely hate the account I type for....hate it. Is it really THAT difficult to want most of the physicians dictate correctly? I said 'most' too because I do know that to expect all of them to dictate correctly is ridiculous. I can't even average 10 reports an hour on this account because of the errors. It's such a nice holiday gift they're giving us. We all know that anytime you throw a new account or dictator in the mix it drops our LPH. Because of this it will also be dropping our PTO, which is based on our LPH/cpl. I just had a report that could have been dictated and wrapped up in 10 seconds if he'd just dictated it straight through. Instead, each word was separated by about 10-20 seconds of nothing, with only the sound of him eating chips until he said another single word, then smacked and crunched again for another 10-20 seconds.
