I am about to let loose so stop reading now if you don't feel like hearing it.

I am SICK and TIRED of new MTs in this field who say that all is rosy in the MT world.  Oh, what bright horizons and exciting times for us here!!! Oh how wonderful for those who roll with the punches because technology is gee whiz wonderful.  The stockholders are promised less labor in the future. Well isn't that great, that doesn't affect ME.  That means we are looking forward to bright and new horizons.  Gee whiz isn't my life all sweet and rosy, and aren't I GREAT that I ROLL with the PUNCHES.  Lookee at me.  Lookee at me.  Give me gold stars on my homework that I am so flexible.  I loved being over and look how well I take it. Give me less money!  I can make it.  Gee whiz how wonderful AM I.

OK: I am SICK OF YOU. YOUR EGO. HOW  this is pin money and you have hubby buy anything you want every time you go shopping.

I AM in this 26 years. I am SICK AND TIRED OF YOU and HOW GREAT AND FLEXIBLE YOU ARE> you are an ENABLER.

STOP enabling greed.  Because you are making it bad for the rest of us.  STOP saying you will bend over and take it any which way you can because YOU are so great!

SOME of us remember when our knowledge of science, the correct English language, every drug spelling and lab dosage, every single bone and orifice of the body and the name of each, and we were PAID CORRECTLY.

And for you who are in the higher ups at AHDI I AM SICK OF YOU TOO!

And for you docs who today said it was June 16, 2012.  For you docs who say it is a she when it was supposed to be a he.  For you docs who don't even know the appropriate drug or dosage, or yawn and burp into the phone I AM SICK OF YOU.

I am worth my weight in gold, NOT minimum wage, NOT the nasty e mails and calls from the boss, NOT the threat of firing, NOT the crap I am having to put up with.  I do NOT deserve to have to work 70 hours a week at minimum wage. 

PS: I am also sick of people on these boards who claim they  make 30 dollars an hour and 6000 lines per hour with all their short cuts and their EGO with VR.  I AM SICK OF YOU TOO.

What I want to say is that I personally thank the PROFESSIONALS who are worth their weight in gold, who know all that you know, and do all you do.  For putting up with the NEW MT egos and bragging, the MT liars who do NOT make the 6000 lines an hour on VR.  Thank you for putting up with the doctors who should be FIRED because we would be if we made those kinds of errors.  Thank you for putting up with food stamps, living in hotels, and for hanging in there.  YOU are who I admire. YOU are the ones who are the heros for patient care.  You have always been worth your weight in gold and you still are.

The rest of you who I am SICK OF... well you can take this post and put it in your pipe and smoke it, because we know that you are the ruin of our beloved industry.

I used to work for a hospital and then Edix came along and out the door we went. It was fine for a while, then Spheris took over and little by little it got worse and then they got this new software called Cornerstone and it was horrible; I quit. Now wish I could say, I found a great job, not as an MT, but that was not the case. I went to work for the devil himself, Medquist! For two months, I was so happy, eight months later I quit w/o notice. It took me eight months to find a another job and I hated MQ so much and to this day I hate it so much, M*Modal or whatever stupid name they have chosen, I wish that whole company would burn (not the MTs). I, for the life of me, don't know how they can work for that company in any capacity. Oh yeah back to my finding another job; that was almost three years ago and I have had I don't know how many other MT jobs and each one more-or-less worse than the other. I finally found a good and then a radiology account for the weekends, BUT, I am semi-retired and trying to make it working part time, even with two jobs. They have killed this profession with their greed and all the ridiculous rules and guidelines and the doctors (don't even get me started on that one). I just pray I can hang on as long as I can, because I am very happy with what I have, but it has been a long road and BTW, I do work 7 days a week; yep, for part time work, I work 7 days a week. Now that is my choice, but not really. I had to do that so I could make at least some decent money and I am an IC. I will never see a raise, never have PTO again or sick leave, no insurance (not that I need it now) and have to just be happy to have job! It sucks!

Totally agree with everything you said. This profession has changed so much in the over 35 years since I started in it. You could make a decent living at one time in it. Like I said before here, my income has gone down 50% in six years since I started with Medquist, now MModal. My biggest gripe will always be that they say (whoever they are) that one can edit quicker than one can transcribe a report. I have yet to see that happen.

So true. If they wouldnt have cut the pay half of what it used to be - it would still be a sustainable occupation. Alas,

I agree 100%. I can transcribe any report 3 times faster than I can go thru and edit..having to add "and, the, it, no" etc.in almost every sentence. Not to mention MRIS of the lumbar spine, which after a year still put in "TO space narrowing" instead of "DISC space narrowing" and FRONTAL encroachment instead of FORAMINAL. all that fussy little stuff takes longer to fix than if I had typed the entire thing from scratch! That's a Fact

The problem is all the competition, the cheap hospitals and doctors, the VR, the EMR, all looking for a way to save a penny, so they take those pennies away from us. What these companies need are seasoned transcriptionists who can get the job done accurately and professionally and submit their reports directly to the clients. They need to eliminate most of the so-called QA staff, because they're another reason we've had to take a cut in pay (I'm saying most, not all). They wouldn't need these people if they hired qualified transcriptionists and paid us well for our skills and experience, but no they have to hire inexperienced transcriptionists who are willing to accept less pay, then pay someone to check all their work, then some even have higher QA to check their work. Training is one thing and that is fine, but they should make less. We shouldn't be the ones taking a hit because of trainees. What ever happened to the pay scale? It is SO UNFAIR to pay everyone the same! It's ridiculous, it's discouraging, it's disheartening, and it's downright a slap in the face! And don't even get me started about off-shore outsourcing. Dictation is being sent overseas and coming back to be edited by transcriptionists here in the US, a lot of whom are also not very experienced (think about what might be "lost in translation"), then also to QA. Some hospitals and physician practices aren't even aware that these companies hire off-shore to "save money," but some are and some even request it, and how are they saving money when it takes 2-3 people to do the job. We seasoned transcriptionists perform our jobs the same as we always have and put time into creating an accurate document, learning new software programs, etc., only to see our compensation diminish more and more. I can only pray this someday will turn around and bite them all in the behind. If this keeps up, there are going to be so many inaccurate records (more than there already are!), nonpayment from insurance companies because of the inaccuracies, not to mention malpractice/fraud for documenting exams and tests they did not perform, etc., because of the EMR functions, IT'S JUST CRAZY!!! Well, that felt good, even though nothing may ever come of it :)

I totally agree with this. I have been doing this for almost 20 years. I pride myself in my work and the depth and breadth of knowledge it takes to produce a document that is readable and accurate. I have had to take a pay cut of MORE THAN HALF to do the same type of work, only this time it's mostly VR and part straight typing. In this global economy, it's all about cutting corners and competing with the poor souls overseas who have no choice but to work for pennies on the dollar. It's really pathetic and sad. I have tried to "get with the times," but I sure hate the fact that I have to be demeaned and degraded to the point that working as a Walmart greeter would be a promotion.

Trainees cannot handle today's MT or editing work without a long training and mentoring program. They need to be able to read a "normal PE or ROS template" and be able to pick out inaccuracies for that particular patient. They need to be able to correct all the mistakes made by doctors that they don't have the experience to recognize. This work is NOT just typing what the doctors says or the health care system would be in a lot more legal trouble than it is today.

I hope you feel better after that verbose rant. Now calm down and take a few breaths. What are ya gonna do?

Most of what you described comes with the territory. There is nothing that can be done about the technology or the ignorant doctors, PAs, and NPs. It is what it is. Unfortunately, even if you could abide by all of that, the technology is definitely going to get rid of many of us here, seasoned MTs as well as newbies. So many are already leaving. Many MT schools have suspended classes due to lack of interest. We are in a cycle in this industry that will make it hard for anyone needing to make at living at this to be able to weather the storm. Whether or not I will be one of them remains to be seen.

You have decide if you can weather the changes. Can you live on the reduced income and lack of work to remain an MT. I do honestly believe a tremendous weeding-out process is going on, which will probably equate to eliminaton of about 1/2 to 2/3 of the MTs. EMR will accomplish this, but dictation will not completely go away. There will always be a need for what EMR does not take care of, and honestly many doctors just like to hear themselves talk.

I am afraid because I work for a relatively small company, about 300 MTs, if you consider that small. Has their For Sale sign gone up? How can they remain in this business? Will I end up working for one of the Monsters? Probably so, as a last resort if I want to continue MTing. Even so, after the weeding out process, I believe what comes around goes around. I think QA will be eliminated for the most part. I think there will be many seasoned MTs left who will not accept low pay, unreasonable demands, and to sit at their desk with no work. I figure if I have to change jobs after so many years with my company, I can change again. I will not sit with NJA day in and day out.

Improperly trained MTs are going to have a very difficult time. I believe seasoned MTs will begin to make their own demands, as they are worth it. There will be an evolving process in our favor. Now, how long this is going to take, I do not know. I just hope I am not retired or dead by then.

We all feel your pain. It is frustrating. If you cannot weather the storm, take your time in finding a new position. Don't jump from the frying pan into the fire. If you find you have, do not accept it. Move again, and again, and again until you land in the right spot. This more than anything will send a message to MTSOs left with all the work. Also, hopefully with this evolution, off-shoring will no longer be profitable. I hope this all occurs sooner than later. You are not someone on here just ranting because things do not go your way. You are right. It is sickening. We all feel your pain. The best of luck to you.

I actually work on an account where the hospital uses Epic and I still have a TON of work every day. Proof that MTs cannot be entirely replaced, as the docs have to supplement the machine generated dictation.

My only question is will they do away with American MTs entirely and replace us with all offshore MTs?

I am pushing 50 and need at least 20 more years in the work force and I honestly do not see MT as a viable option.

I don't see wages ever going up due to the offshore competition. I would not recommend to anyone, best friend or enemy, to get into this business as a newbie.

OMG!!! Did you hit the nail on the head!! we who are SPECIALISTS and good at what we do are now treated like slaves and should be ever so grateful for the CRUMBS we are handed! My knowledge and expertise is WORTH SOMETHING..a lot more than what I am currently being paid. I made more money in 1997 than I currently am making...and i'm working 2 jobs to do it (looking for a 3rd job on top of it). I too am single and have no HUBBY or Sugardaddy SHOWERING me with endless gifts and spoiling me rotten (Maybe in my next life!) The problem with healthcare today is it has NOTHING TO DO with PATIENT CARE and EVERYTHING TO DO with Profit.Don't kid yourself, even a NONPROFIT company MAKES MONEY!! And AHDI shove your BOOK OF STYLE were the sun "don't" shine! All the new TECHNOLOGY gives a Rat's *SS about your BOOK OF STYLE! It makes me sick. You get what you pay for..you want to outsource to another country, go for it..when your records get circulated around the world, don't come crying to us! I hear you my friend, and I was sick and tired of it too, so much I sent a letter to our beloved President @ the White House about the horrors of this industry!

Thank you Mr. President for taking time to read my correspondence. I would like to graciously ask for your assistance and intervention into what has become a sweatshop labor industry in America, namely medical transcription. I know what you were able to do for Lilly Ledbetter and there are over 25,000 medical transcriptionists, (predominantly women) in this country who are working from home full time earning wages that would embarrass laborers in a third world country!

Transcriptionists work as much as 60 hours per week from our homes. We provide all the necessary equipment to perform the job (computers, electricity, internet) but we earn as little as $3.00 or $4.00 an hour for a yearly compensation of around $10,500 a year for working time and a half!

Typing medical records for healthcare providers (physicians, nurse practitioners and physician's assistants) is no easy task. As part of our current job we are required to possess as much knowledge as any doctor or nurse knowing anatomy, physiology, pharmacology, all medical imaging/testing, normal lab values in addition to typing at record speeds of up to 200+ words per minute. We perform this work while being held to a standard of quality of work measured at 99% accuracy. Many times the quality of recorded dictation given to us is at best 50% or 60% English proficiency, so we have become translators as well.

The companies in this industry have now taken to classifying the medical transcription job as that of an "independent contractor," primarily to skirt wage and hour laws and avoid paying tax, all the while still making us adhere to rules reserved for those classified as W4 employees. We still have to adhere to a work schedule per the discretion of the companies we work for. Even as independent contractors we are still required to keep time records - albeit even if we are coerced by our employer to falsify those documents to satisfy the needs of the company by reporting lower hours worked.

We are assigned work at the discretion of the company (not having access to all the work available, except specifically what they want us to work on), we are told exactly how to do the job, what equipment we must use to complete the jobs assigned. Lastly, we work on production and as much as 35% of the work we do is completed for free with no compensation, and we are told, "it's your job." It was my understanding that a job was a unit of work for compensation. If I work on production and I am not compensated for my production, how could that be considered "my job?"

I have taken my grievances to the Department of Labor to no avail. For 24 straight months I received a form letter explaining that they were "working on my complaint." On the 25th month I received one final letter saying, "Thank you for contacting the Department of Labor regarding your complaint; however, the statute of limitations in your particularly case has been exhausted." They did absolutely nothing to intervene in spite of wage and hour laws.

In the end, transcriptionists seem to have no recourse to resolve what has become an abhorrent exploitation of labor. If we voice concern or discontentment with our employer they fire us without recourse. Since we are classified as "independent contractors," there is no safety net such as unemployment insurance, and sadly, once we do find another job the terms will be much the same as the previous company because it is the common practice among this industry to operate on the same employment terms.

My work experience in medical transcription is for 18+ years. I have never had a COLA increase, or any wage increase. In fact, I have only seen decreases in my compensation where companies do not want to pay for certain items of production so they have cut our wages.

It becomes easy for someone to say, "Well just get another job," however, that practice is easier said than done. When you have worked in one specialized line of work for a whole career it is hard to find a job outside of your expertise simply because the job experience we have does not meet what other employers are seeking. It is difficult to work even in the fast food industry when all you have ever done is transcribe medical records.

I implore you Mr. President to step in please and help us. It has become so bad that without reservation I can say that if these companies knew how to get transcriptionists to come to work every day and work for free - they would have us all doing that as well. Sadly, in all actuality we are practically doing that now.

As transcriptionists we spend a great deal of our day preserving the integrity of the doctor-patient encounters by correcting incorrect dosages, incorrect medications, and translating poor English speaking doctors who can barely communicate into reports that document patient healthcare accurately. In addition, we are held to a level of performance perfection at 99% on the work we do. Yet our reward for this is to be disregarded and mentally abused so that we become compliant, unable to make a stand for what is honest and fair. Somewhere we were made to feel helpless, and worthless so that we are happy to work for less than a fair wage being thankful that we even have a job at all. Jobs where we can work 60 hours a week in tedious profession and still qualify for EBT benefits just to barely survive.

Medical transcription is the new sweatshop labor in America, and so far it seems no one entity of government powerful enough to change it has cared enough to help. I am truly not sure where else to turn, but I know you can help, and I believe you will. You are by far the most compassionate President of the people we have had in my lifetime. Wholeheartedly, without reservation I can say I know you care about all Americans, job creation, and our prosperity as a country.

I can attest that any medical transcriptionist listening, correcting and typing medical reports 10 hours a day, six days a week for 15+ years has a vast wealth of medical knowledge; enough that they could practice medicine themselves. It would be much the same as attending ongoing medical lectures for 32,000+ hours. How many licensed physicians can say they have done that as part of their education? The only thing medical transcriptionists would require to become practitioners themselves is some clinical instruction on performing practical exams, a licensing authority and a real opportunity!

In the meantime, minimum wage and employer paid FICA and Social Security tax would just be a bonus to our profession.

Please, I implore you to help us!

It will depend on the staff assistant who reads my letter and what they do about it. The problem is that the right eyes with the power to do something have no idea this kind of dirty business is going on. Yes, it's dirty business - when 60% of all people getting foodstamps are working - that's a dirty deal. Walmart is another company who needs a gov't smackdown. They employ 1% of the work force in this country, 80% of their employees get gov't subsidies because they are paid so little, yet in 2011 Walmart was #1 on the list of Fortune 500 companies. So far this year they have earned 15.7 billion in profit. Why can't Walmart pay their employees a living wage out of their profits before taxpayers have to pick up the tab so these people can eek by? Afterall, the hands of Walmart labor are making their company successful. Just owning a company doesn't make the company successful - it is the hands of labor that produce goods and services that keep the wheels of prosperity greased and running. What we see happening today is the second coming of the Robber Barons.

and that's about all he could try to get passed. Everything else.. it all comes down to saving money, cutting corners whenever possible, and we pay the price unfortunately. It's also a matter of doing what is morally right for many workers in this country. We are not the only people who are working hard for low wages. Greed trumps all in this world now, sadly. I just accept that this is the way of the world now, no more wringing hands at the unfairness of it all. It is what it is. No one really cares as long as they have theirs.
Outsourcing Taxes - RF

One law would fix most of what is wrong with America - a law that 25% of gross profits must be shared with the work force in wages and benefits. That still leaves the company with 75% profit. It will increase discretionary spending - prompting a need for more goods and services, i.e. more sales and more jobs.

While I believe capitalism is great - it has no moral conscience, and all we really need is a max cap on greedy profits.

Companies like Walmart with so much of their workforce on public subsidies need to be treated like dead beat dads where the company pays before taxpayers pay to support these working people.


And Yes! They could and they should tax companies that outsource - and the revenue should be placed into a fund for retraining Amaericans who have lost those jobs so they can find new jobs.

The fine should be equal to the difference in the American labor cost and that of the outsourced job.

Two things would happen -

Outsourcing would be minimized - why outsource if there is no labor cost savings?

And those who lose their job to outsourcing will be put back to work doing something else.

I'll tell you like it is, so if you don't want to hear it stop reading now.

Need to break this down for you in simple terms:

1. We were all new MTs at one time, so lay off the new MTs. They probably don't care if you are "sick and tired" of them. They are most likely sick and tired of people with your attitude. I know I am. I've been an MT for 20 years now and I love new MTs. I love the willingness to learn, the upbeat and outgoing attitutes. The willingness to transcribe the more difficult doctors because other MTs with a chip on their shoulder and 26 years in the field think they deserve their pick of who they want. I'm sick of the older MTs with your kind of attitude as though they are better than anyone else.

2. It doesn't matter what field you are in, when you are new, yes you do want things to look up and everything looks great. Even the poster who got a job in collects is looking up and happy and that's THE WAY IT SHOULD BE (see I yell too). Looks like you are the one with the ego. I'M SICK OF IT.

3. Technology IS great. Without technology you wouldn't be sitting there posting that hateful rant you did. Without technology we would not have TV, microwaves, state of the art medical equipment, even Novacaine. State of the art is great. If you can't handle it maybe you need to look for another line of work. I'm sure there is some job out there that does thinks like it was the 1940s so you wouldn't have to deal with new technology.

4. Not all MTs have a hubby that buys them everything. I am an MT and I am struggling to make ends meet. My hubby cannot find a job. But I'm not sitting here moaning and groaning about it. It's just the way things are. I am thankful I can pay my bills and put food on the table. It will be up to me to make my life better, but that's life. YOU NEED TO DEAL WITH IT.

5. NOBODY is enabling greed. We need jobs. We apply for a job. They tell us how much the job pays. WE DECIDE if we want to accept it. WE DON'T CARE if you don't like it. It's our life and we have to make a living and this is what WE WANT TO DO. WE DON'T CARE HOW IT AFFECTS YOU.

7. We have moved forward in our lives. Yes, it's nice to dream about the good ol days when we got paid more, when our fathers worked and and could support the whole family while mom stayed at home to raise us. On $200 pay a week he paid the bills and mortgage, put food on the table, sent us to school and we went on a 2 week vacation every year, but WE have learned that times are changing. It's not just in the MT field. WE have learned to deal with the change. WE have to pay bills and put food on our table. WE decide that we prefer staying at home and getting paid not as much as we used to make, but here's a news flash for ya - it aint going back to the way it was. DEAL WITH IT!

8. If you're so sick of the higher ups at ADHI then do something about it. Get a different job. Don't know what ADHI is. If it is a company name then you are violating the rules of this board. If it is a general term I've never heard of it and I've been in the field for 20 years, but if you are sick of someone or some company then do something about it. Go find yourself the higher paying job you think you are worth. But remember, pay increases each year are based on reviews and if you have this kind of attitude at another job, well I wouldn't expect that big of a pay raise.

9. If you can't handle a doc making a mistake then you are in the wrong field. Here's another news flash. Docs are human too. Docs see many patients. It is the job of the MT to make sure he is she and she is he and the correct date is written, and do the research to know if a drug is the wrong one or dosage and make a note of it. Again, if you don't like it maybe this is the wrong career for you.

10. STOP YELLING (please).

11. Hate to break this to you, but you are not worth your weight in gold. Just because you think you are does not mean it's true. Especially if this post is an inkling of the person I would have to deal with if I was a higher up. You are worth the going rate of what they pay all of us. Again, if you feel "gyped" of that gold price then this is not the career for you.

12. If you are getting nasty emails and calls from your boss, threats of being fired, well that wouldn't surprise me if your post is an inkling of what they have to deal with.

13. If you don't want to work 70 hours for minimal wage don't. You are the only one who can choose what you want to do, but if you think you are going to work a 40-hour week, weekends and holidays off, no working late and get paid $50 dollars an hour ain't gonna happen in this field. Even my instructor at school said if that's what you want then you are in the wrong field.

14. Nobody here claimed they made $30 an hour or typed 6000 lines a minute. I wish I would, but again, nobody here said that. However, I can see that if someone was to have their own accounts and was a go getter and worked directly for a doctor or whatever, I could see someone making $30 or more an hour. If you are out there just want to say kudos to you. You work your booty off hard and deserve all of it, and please know that we are proud of you and hope to aspire what you've done. Not everyone is bitter and angry like the poster. P.S. - if you have any hints on how you did it there are some of us who would like to know how. We are not jealous, but we envy what you have accomplished.

15. STOP YELLING (please).

I've been doing this a long time, and I know how oblivious doctors are to anything approaching being polite when they dictate.  I've heard them in the bathroom, I've heard them yelling at their kids, talking on the phone, listening to football games on TV while dictating, etc.  We all have.  But there's this one foreign female doctor on the account I regularly transcribe who drives me UP the wall.  She apparently thinks dictation time is time to eat.  ...

Even the sound of every deep, gravelly, congested-sounding breath the man takes makes my head and ears hurt.  Sounds like he's dictating with his mouth full of spit or some kind of gunk. I absolutely hate the account I type for....hate it.  Is it really THAT difficult to want most of the physicians dictate correctly?  I said 'most' too because I do know that to expect all of them to dictate correctly is ridiculous.  I can't even average 10 reports an hour on this account because of the errors.  They mumble, slur their words, clip their words, rustle every paper around them... and I'm taliking about 80% of them!  This is a new account ...

Just got a new account a couple of weeks ago and was super excited because its brand new,  straight transcription until VR picks it up.......good deal?  Not so much.  It seems the QCs are all on a different page.  I have sent the 3 "Super" QCs all the same question and got different answers.  Sent it to my lead and she told me to ask the "Super" QCs. Why oh why can't these companies have just a speck of honesty when they hire us?  I was recently hired by a company and was hired for "fulltime" and I get at the most 600 lines daily.  I actually love the job and would probably work it part-time, if asked, but why deceive me into thinking they had full-time lines available?  I see MTs who are actually looking for part-time and most companies say "We are sorry, but we are only hiring full-time."   I am beginning to belie ...

It's such a nice holiday gift they're giving us.  We all know that anytime you throw a new account or dictator in the mix it drops our LPH.  Because of this it will also be dropping our PTO, which is based on our LPH/cpl.  Also, I'm sure we'll see the dreaded "no jobs available" coming our way because everything has been pooled out to the masses to keep a 4-hour TAT instead of the usual 16 to 24 with our exclusives.  Plus, don't forget, as promised, V ...

I just had a report that could have been dictated and wrapped up in 10 seconds if he'd just dictated it straight through.  Instead, each word was separated by about 10-20 seconds of nothing, with only the sound of him eating chips until he said another single word, then smacked and crunched again for another 10-20 seconds.  I hate those kinds of inconsiderate doctors so much! ...
