Just a small vent - frustrated

I have one of "those" dictators. I know. . . . . who doesn't, right?

This guy is a native English speaker, but he's a speedy mumbler. I struggle with him, but work on each one of his that I get so that I can get better. Just when I think I'm getting better with him one of his comes back from QA completely marked up. Some are changes that could be debated, but some are just stupid mistakes that I should have caught.

I'm just frustrated with myself this morning. Yes, it would help if Dr. Mumble mouth slowed down and stopped speaking so fast he tripped over his own stupid tongue, but still...... I'm better than that and I know it and I'm just frustrated that I didn't do better on that report. I just cringe when his name comes up, though.

Okay. Deep breath and more coffee and onwards and upwards.

I have some horrendous ESL dictators. Fortunately don't have them often, but even with lots of samples I can't come up with what they are saying at times. I go through the report 3 times and I usually get some blanks, but still have a few. I'm proud of myself that I was able to get some, but then I get it back from QA with lots of dings for errors. I don't get charged for blanks, so get the heck dinged for trying. It is discouraging.

how can you get your minimal line count? - or it does not matter?

one that if it wasn't obvious that I would be cherry picking I would take a break every time her name comes up. Some are just impossible, no matter how many times you have them.

What if you went through the entire report just what you think he is saying. Then go through it again just listening and you can catch more words and also catch minor mistakes. I think the minor mistakes come from struggling with the voice itself. It just makes you all tense. I know the feeling as I have a mumbler, too.

I have pretty much mastered him at this point but I find that if I take a deep breath and just try and relax that I catch more. I no longer do the second listen, but I will do it with difficult dictators just to see what I can catch after the fact. I also think it gives you a better idea of the patient he is dictating on, so you can go back and also make better decisions based on what you've transcribed.

Maybe not go back and listen again, but definitely go back and do more of a solid proofread than what I normally do. That was the problem, I think. I do go back and skim over all of my reports, but only really go back and proofread every word on the really long ones. I'd rather take the hit to my line count than see another report like that come back from QA.

Try adjusting the speed. - It could work

Slow him down or speed him up. I had one doc I did this with for several months, but in the end it was worth it. It just took me a while to get going understanding his mumbles and all, but everyone hated doing his notes, everyone but me, that is. I ended up basically flying through his dictation and his dictation was all mine. :)

Like someone else said, I just completely tense up when I see his name come up. I have to work on getting over that. I think I will try to slow him down to see if that helps.

I have one just like yours - every weekend

He's on one of my secondaries, and stats every word he utters. Slowing him down helps immensely, but I still end up leaving blanks in about 50% of his reports. He destroys my LPH for the week (it is averaged - so this lowers any holiday or PTO pay I get), destroys my line count for the day, but worse, he prevents me from typing my primary until I have typed all of his stats. If other accounts are behind they have to wait for him too.

Personally, I'm flabbergasted that they kow-tow to him and his puny little account this way and let him ruin all the other clients' TAT all weekend. I rue the day I started typing this account, but I guess I'm stuck with it forever - the most my boss will do is peek in once in awhile and "assign" me my own primary jobs - which helps until the assigned jobs run out, then its back to struggling through quicksand with this despiseable dictator.

I refuse to listen to him repeatedly to try to "get" the blanks - he costs me enough in lost production, stress and misery as it is! At least QA stopped sniping at me because since I stop and slow down every report I get less blanks than most MTs that type him. I've also made it my policy to refuse to work any OT on a day he's present - no way will I spend one more instant typing him than I'm forced to. I might attempt OT, but if his jobs start popping up again, I'm gone.

Don't blame yourself. Its the MTSO's fault. They refuse to recognize a problem dictator and treat them accordingly (like arrange for us to type his work LAST instead of FIRST). It is their inflexibility and their denial that causes the other reports to fall out of TAT - the production figures every day he dictates are ample proof - so let them explain to the other clients why Dr. Mumblemouth's work is more important than theirs.

I've always just used "small bowel," but then noticed most put "small-bowel," I assume to differentiate it as being part of the intestine as opposed to a small, or little bowel. Okay, that makes sense, so I stated hyphenating small-bowel. However, I just got marked off by QA for putting in a hyphen! No matter what you do or don't do, you're screwed! Someone ought to take these ESL doctors and torture them like they torture us with their unbelievably horrible English speaking skills! We should not be told we're jeopardizing patient care - it's the ESL doctors. I've never had an ESL doctor and I'd refuse to be treated by one who can't speak clearly or speak a complete sentence. I've been doing this a long time, and I know how oblivious doctors are to anything approaching being polite when they dictate. I've heard them in the bathroom, I've heard them yelling at their kids, talking on the phone, listening to football games on TV while dictating, etc. We all have. I absolutely hate the account I type for....hate it. Is it really THAT difficult to want most of the physicians dictate correctly? I said 'most' too because I do know that to expect all of them to dictate correctly is ridiculous. I can't even average 10 reports an hour on this account because of the errors. It's such a nice holiday gift they're giving us. We all know that anytime you throw a new account or dictator in the mix it drops our LPH. Because of this it will also be dropping our PTO, which is based on our LPH/cpl. I just had a report that could have been dictated and wrapped up in 10 seconds if he'd just dictated it straight through. Instead, each word was separated by about 10-20 seconds of nothing, with only the sound of him eating chips until he said another single word, then smacked and crunched again for another 10-20 seconds.
