I have an ARNP doing a radiation follow up. She dictates "33 fractions of 5040 GRAY" and then goes on to say "that's small c, capital G, small y." She has done this a number of times over a number of reports.

I'm just shaking my head.

Remember long ago transcribing reports for someone who used the "alpha, bravo, charlie" system to spell words--think she was a cop wannabe. Oftentimes she wouldn't even say the word first; you had to guess where she was going with the "spelling". Never heard that before or since [thankfully].

I worked in-house briefly last year (hated it!) and we had an Iranian PA who would spell every thing three times. He'd give his name three times, give the name of the patient three times, give the record number three times and then proceed to spell everything. I know he was just being careful, but still.

Most reports were about 3 minutes in length, if that. His were 5 to 7.

The kicker was one day when he spelled the patient's name. It was Bob.

I had an ER doc that way many years ago -- I am a military widow so understood the spelling that way and I would call him and razz about it -- the ER doc had been a captain in the AF

I have an RN whole will dictate for 5 minutes or so (she always has long discharge summaries) and then will say, "Start a paragraph." Way ahead of you, lady.

Had a funny one today. We have some surgeons who all of a sudden started saying the year the patient was born instead of their age. I think it sounds kind of dumb. The PA that was dictating said, "This is a .... how old is this poor guy...oh, why does he say born in 45?" I was like YES! Thank you. I've been wondering the same thing!

After years (literally years) of saying "micrograms per kilograms per minute", one of my docs got on a kick of dictating: "em-cee-gee backslash kay-gee backslash em-eye-en". Made me nuts.

Um, I don't think the OP is questioning that - I took it as the OP commenting on the fact that this dictator is repeating the same spelling over & over again when once would have been sufficient.

Although, if the dictator means Gy (because she said "gray") and not cGy then the dictator is not spelling it correctly anyway and that could have been what the OP meant.

I'm just chuckling at a PA's dictation. Unless I'm wrong (which certainly would not be the first time) when they dictate GRAY it is typed Gy. When they want cGy, then it is dictated Centigray. If they mean 5040 units of something, then it should be dictated centigray and not simply gray.

Someone ought to take these ESL doctors and torture them like they torture us with their unbelievably horrible English speaking skills! We should not be told we're jeopardizing patient care - it's the ESL doctors. I've never had an ESL doctor and I'd refuse to be treated by one who can't speak clearly or speak a complete sentence. I've been doing this a long time, and I know how oblivious doctors are to anything approaching being polite when they dictate. I've heard them in the bathroom, I've heard them yelling at their kids, talking on the phone, listening to football games on TV while dictating, etc. We all have. I absolutely hate the account I type for....hate it. Is it really THAT difficult to want most of the physicians dictate correctly? I said 'most' too because I do know that to expect all of them to dictate correctly is ridiculous. I can't even average 10 reports an hour on this account because of the errors. It's such a nice holiday gift they're giving us. We all know that anytime you throw a new account or dictator in the mix it drops our LPH. Because of this it will also be dropping our PTO, which is based on our LPH/cpl. I just had a report that could have been dictated and wrapped up in 10 seconds if he'd just dictated it straight through. Instead, each word was separated by about 10-20 seconds of nothing, with only the sound of him eating chips until he said another single word, then smacked and crunched again for another 10-20 seconds. Pulling standards into ASR reports!!!!! This does not work! If we were getting paid hourly maybe, but this is cruel and unusual when we are on production pay and forced to make an ASR report fit into a standard.
