"Duke Primary Care eliminates transcription costs with SpeechQ."

SpeechQ is the MQ program that eliminates the MT/editor altogether and MDs edit the reports themselves. Duke Family Clinic alone is now doing 8,000 jobs PER WEEK this way, thereby eliminating our job from the whole picture. What doesn't go to India goes to SpeechQ. The writing is on the wall friends. I for one am in the process of Plan B as far as a job goes.

Through all the markers, etc. we have "cleaned up" much ASR -- enough that editing is minimal enough to have dictators do it themselves. So thanks to our efforts (and sweatshop pay), we have optimized SpeechQ for clients and management, leaving us standing in unemployment line soon. There will be a reckoning some day for MQ. :-(

I clean up with the same errors over and over and over again. Speech Q will work the same for them as it does for us, and believe me, most doctors will not take the time to clean it up. This is based on my experience at inhouse at a hospital that bought the SR program, and, of course, since the hospital spent so much money for the system, they stuck with it rather than do what was best for the patient's care. The MR manager was always on the doctor's case about not proofing and correcting their output, and it never worked. Part of this is due to the government's push for quick EMR and in huge part due to ADHI for not standing up or doing anything for US MTs.

In my part-time job, I have to use the medical records and the information has to be accurate. You are completely correct that the doctors do not correct the reports and the reports are, in return, frightening inaccurate. Some information I come across is simply not usable.

Docs will never spend the time looking up all the crap I do and even automation cannot keep up with the advances in gene research.

I too have about 35 years in this business and never thought I would see the things I have seen in the last few years. I worked acute care most of that time, but took a job with a local group of heart physicians (20 in the group) so that I could get benefits. After 2 years, they went to a do-it-yourself platform EMR with Dragon. All but 2 of the doctors are now editing their own dictation (something I thought would never happen). The 2 who still want a transcriptionist's expertise to look things over are being forced to do their own shortly. What a wacky world we live in! The quality of the stuff coming out of that office is atrocious and everyone accepts it because it is cheaper. I think that will not change until huge lawsuits start to happen because of medical mistakes made on the basis of inaccurate recordkeeping. Don't think it will never replace you. That is naive and you will be caught off guard in my humble opinion.

I don't argue that MT is in a sorry state of affairs now and would never recommend it to anyone. Fifteen yrs ago a friend told me eventually this ocucpation would be cut to 20% of its usual workforce.

However, 90% ESL, tertiary care, teaching/research hospitals will be by far and away the last to have MDs edit their own work.

One would think after 35 yrs I would find this job easy...but no. It just gets harder and harder with each and every new medical advancement.

I have 2 yrs until retirement so I'll just ride it out.

Atrocious reports are correct. My hospital went to total doctor correction and the reports hitting the floor are a joke. I do not understand why JACHO has not got involved. I remember when they used to fine Medical Records for any mistakes on reports. What is going on with that? Any reports they see with the initials of VR they probably ignore just like everyone else is!!

in like a newborn baby with no history, see a hospitalist they have never seen before, give a partial history of what they can remember, and get whatever kind of treatment comes along. With these horrifically incorrect documents on file, soon doctors will just not bother reading the historical account, get what they can glean in a hurry, and treat accordingly?

In a part-time job I work (have to because my wages have dropped so much at MQ), I come in contact with the VR program documents that involve no human editing whatsoever. They are terrifying. A lot of the information is nonsensical, incorrect medical words, dosages, you name it. The documents I have seen are not at all edited by the docs - I suspect they don't really care. Sadly, though, you're right. We are almost as much a piece of history (at least U.S. history) as the typewriter.

In my internship I have seen the physicians editing reports dictated on Phillips equipment, and they really hate it. They spend twice as long editing the report as they did to dictate it, and they hate the wasted time. It is impacting patient care; sometimes it takes 24 hours and a walk down to radiology to get a report because it takes so long for the radiologists to edit the reports. All of them say they want humans transcribing reports again. Perhaps just give it some time.

bought an SR system. The doctors too hated it and wanted their work transcribed, but, since the hospital had paid a lot of money for the system, they made the doctors stick with it. And, as I mentioned before, the MR manager was always after them to edit their reports which they never did.

As is usually the case, decisions are made by those who have no idea what the job entails or what is needed.

An HMO I used to go to has the doctors type in the office notes after each patient. The doctor I saw said it cuts down severely on the number of patients they can see because of the amount of time they spend doing their own typing.

These software companies are giving false advertising to these hospitals and they are falling for it. They are paying fortunes for the systems where the doctors do it themselves and they are not as great as the companies claim they are, so the physicians are unhappy but the hospitals have paid so much for the systems that they do not want to waste that money and go back to transcription. I feel this will come back to haunt them when they start losing money on lawsuits and being able to take on less patients and will eventually come back to us. This may take awhile, but I think it will happen. I think somehow the word needs to get out that these systems are not worth the money.

One account I had a couple of years ago left for one of these systems and they were gone for awhile. Now I see reports by them in the DocQManage. I no longer do that account, but it seems like there are hundreds of reports back in there for them. Perhaps if the word gets out how bad these systems are, less hospitals will fall for the false advertising by these software companies. Only time will tell.

I recently changed my health care to Kaiser and have observed first hand and for the first time, the use of electronic medical records; they really are of immeasurable assistance to each provider who is treating the patient. Nevertheless, there are still certain physicians who must dictate, i.e., surgeons, consultants, certain medical specialists. I just ran into a local job already, MOWING GRASS. The lady's lawn mowing man just finished school and is quitting the mowing. Most postings are about how horrible the industry is and how impossible it is to get a job. I haven't been getting a second resevered job for quite some time and was wondering if I was the only one. I have told my manager and she said she would fix it about 2 months ago. It really stinks because after I complete a job it throws NJA at me. If I let if search for jobs long enough it will find me one. It seems they are either extremely low CPL or full of ESLs that means you also cant make any money.
