Do you type ALL work types, including tier 1 and tier 2, correct? My CCM implied that tier 3 does ONLY operative reports?? I'm tier 2 and I think I would like to try OP reports, as I've been tier 2 for quite a while. Don't want to NOT do consults and H&Ps though. Not expecting a raise of course, but would just like to try something different. And maybe get NJA less often. What do you all think?

If you do type all, what percentage (guessimate of course) do you think you actually get OPs? Thanks in Advance!

Type everything. Actually not a LOT of op notes. H&P, consults, discharge summaries, and those blessed clinc notes.

10 to 25% op notes, about the same for H&P, DS, consults, rest clinic notes & letters - nm

Yesterday I think I only had 2 Op notes in 4 hours.

That was before NJA, of course.

Those dried up a couple years ago, wonder where they went, hmmm. Anyways, mostly discharge summaries and H&Ps, which slow me down quite a bit. Esp the discharges with sometimes up to 35 medications with dosages etc., and the lab values drag on. Really quite frustrating.

I looked into becoming a tier, read all of the stuff on MQ Central as advised. I contacted my CCM about doing it, she said she would put in for it. That was about a month or so ago. This is very discouraging, but seems to be typical MQ to me.

Try some skeletal dysplasia genetic evals or some of those cardiac procedures where every stinking strangely named piece of equipment is dictated. There are currently about 200 WTs in my account including autopsies.

I am tier 3 and I do everything except Radiology - even get some oddball radiology stuff that got loose thrown in very occasionally, not just ops. You will still get your consults, progress notes, H&Ps - etc. I think you will like it. The great thing about ops is that some of them say the same thing over and over so you can fly if you know how to organize them and make expansions. Go for it.

I was just informed that I HAD to tier up. Has anybody else been told that one? I don't want to tier up. I don't get much NJA and I like the accounts I work on. I was told that I MIGHT get a raise if I did, but there was no guarantee. Why would I want to tier up without getting a raise doing the next tier's work? I just don't get it and I never will. ...

This really sucks because those reports have the highest line counts, while Tier 3 does all the testing reports such as cardiology, eeg, cath reports, and surgeries. While straight typing pay may be higher for Tier 3, everyone makes the same for VR. My vent for the day. Sorry if I offended anyone. ...

What might be happening is MM is keeping high producing Tier 3 employees and India will get all the easier work. Tier 3's in America will do the hardest dictations and worst dictators. The MME's in America will then be correcting more work because India will be doing everything else. Just trying to figure out what is going on. I hit the ceiling last week and quit after 12 years with MQ part-time. I started as a tier 3 and ended as a tier 3. Topped out. I was making close to $20/hr and ended at $10/hr thanks to ASR. I work for another MTSO too that doesn't have the tier system. I get just whatever report comes into my job pool from clinic to ops. I've been at MQ for about 3 years now. CCM keeps wanting me to tier up to tier 3 so I can start doing the ops notes because we don't have enough 3s right now. However, I won't get a raise by tiering up. Has anybody ever passed the tier 2 test?? This is my second attempt at passing it and it seems impossible. You can't slow the transcription. You can't understand the words before and after the privacy tones. It's like they don't want you to pass, succeed, and reach a higher level. My regular work reports are easier to do than anything on that test, even the ESL doctors! ...

Just wondering if it is worth it to test up to tier 3 or not.
